Chronic granulomatous disease (CGD) is a disorder of immunity characterized by phagocyte dysfunction. Mold infections in patients with CGD are often severe and disseminated. We present patient characteristics, microbiological data, and outcomes for 26 patients with CGD who received hematopoietic cell transplantation (HCT) or gene therapy-modified cells (GT) between 2008 and 2019, with proven fungal infection either before or during their transplant.

Tuberculous otomastoiditis in children complicated by homolateral sigmoid sinus thrombosis: a case report.

Pan Afr Med J

November 2024

Department of Otolaryngology and Head and Neck Surgery, Faculty of Medicine and Pharmacy of Fes, Hassan II University Hospital, University Sidi Mohamed Ben Abdellah, Fes, Morocco.

Tuberculosis is a major public health problem in Morocco and most of the developing countries but tuberculous otomastoiditis is quite rare. Here we report a case where a seven-year-old patient had chronic right otomastoiditis and retro-auricular fistula, whose specificity is its persistence after prolonged unsuccessful medical and surgical therapy for more than 2 months, and its complication by homolateral sigmoid sinus thrombosis. Computed Tomography (CT) scan of temporal bones showed complete destruction of right mastoid cells and a hypodense complement in the right middle ear.

Clinical results of locking compression plate external fixation for open ulnar and radial shaft fractures.

Int J Surg Case Rep

October 2023

Department of Orthopaedics Surgery, Tianjin First Center Hospital, China. Electronic address:

Introduction And Importance: Open ulnar and radial shaft fractures refers to the simultaneous rupture and injury of the skin and subcutaneous soft tissue at the fracture, and is easy to cause wound infection, local soft tissue necrosis or osteomyelitis. The purpose of this paper is to observe the efficacy of external locking compression plate in the treatment of open fractures, so as to provide reference for clinical practice.

Case Presentation: From January 2019 to January 2020, 11 patients with open ulnar or radial shaft fractures underwent surgery by a single surgeon.

>25 % of the patients who receive orthopedic implants have been reported with implant-associated osteomyelitis, which can result in inflammation, osteolysis, and aseptic loosening of implants. Current treatment methods doesn't ensure defect healing and prevention from reinfection. Thermoplastic-based 3D-printed scaffolds offer a bioresorbable, biocompatible, and mechanical strong implant system.

  • Charcot foot, a serious complication of diabetes, can lead to high mortality and treatment costs, with amputation associated with a 70% 5-year mortality rate.* -
  • A study compared outcomes between amputation and circular external fixation (CEF) for managing infected diabetic foot ulcers, revealing significantly lower mortality and better quality of life scores for CEF patients compared to those who underwent amputation.* -
  • Although the overall treatment costs were similar for both methods, the quality of life improvement was much greater in the CEF group, indicating that CEF may be a preferable option for diabetic foot management.*

  • Pressure injuries (PIs) are common in bedbound or immobile patients and require a multidisciplinary approach for effective management and prevention of recurrence.
  • The study analyzed data from nine patients who underwent lumbosacral rotation flap (LSRF) surgery for sacral PIs, excluding those with severe systemic health issues.
  • Results indicated that LSRFs are a viable primary treatment option for sacral PIs, with most flaps healing successfully and the possibility of addressing recurrences with readvancement.

Objectives: To elucidate the existence of bacteria in situ and its influence on the clinical and pathological features of synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome.

Methods: A total of 34 patients diagnosed with SAPHO syndrome were allocated to two groups (bacterial positive and bacterial negative) based on next-generation sequencing (NGS) and microbiological culture through CT guided percutaneous puncture. The clinical characteristics, biochemical markers, imaging data, and pathological results of the patients were analysed and compared between the two groups.

Osteomyelitis is a refractory disease of orthopedics, part of which is caused by medical implants. The main difficulties in treatment are the barrier effect after the formation of bacterial biofilm, and the difficulty in achieving sustained antibiotic intervention. In view of this situation, we studied a hydrogel coating that can release CaCl and vancomycin in pH-responsive manner.

Salmonella is a rare causative pathogen of osteomyelitis and is commonly detected in patients with sickle cell disease. We described 2 consecutive cases of nontyphoidal Salmonella osteomyelitis identified in immunocompetent children. It is evident that even children without apparent risk factors for immune dysfunctions can be affected, so maintaining clinical suspicion is crucial for early diagnosis and prompt treatment.

Background: An intramedullary rodding is the preferred fixation method in limb stabilization of OI patients. However, the intramedullary rod may not provide adequate fixation and rotational stability, especially in adolescents and adults. The incorporation of adjunctive plate fixation alongside intramedullary rodding has been introduced to enhance this stability, although its complications remain insufficiently understood.

Bone infection evolution.

Injury

November 2024

Department of Chemical and Biological Engineering, Center for Biofilm Engineering, Montana State University, Bozeman, MT, USA.

The present minireview aims to provide a context for imagination of the timespan for bone infection evolution from the origin of cellular bone tissue to modern orthopedic surgery. From a phylogenetic osteomyelitis-bracketing perspective, and due to the time of osteocyte origin, bacteria might have been able to infect the skeleton for approximately 400 million years. Thereby, bone infections happened simultaneously with central expansions of the immune system and development of terrestrial bone structure.
