Extraserosal invasion in advanced gastric cancer: evaluation with MR imaging.

Purpose: To evaluate the usefulness of magnetic resonance (MR) imaging in the diagnosis of extraserosal invasion in advanced gastric cancer.

Materials And Methods: MR imaging was performed in 48 patients with advanced gastric cancer, and the results were correlated with the histopathologic findings. Images perpendicular to the gastric wall with cancer were obtained with spoiled gradient-recalled acquisition in the steady state (GRASS).

[Early detection of non-palpable (T0) breast cancer: the diagnostic procedure for pathological discharge from the nipple].

Detection of non-palpable (T0) breast cancer by pathological nipple discharge is possible 3 years or more earlier than tumorous breast cancer. However, the definite diagnosis of T0 breast cancer has been considered to be very difficult because the standard diagnostic method such as exfoliative cytology and ductography were not totally reliable. In 1985 we first demonstrated the significance of CEA measurement in nipple discharge for diagnosis of T0 breast cancer.

Criteria for differentiation of normal and abnormal uroflowmetrograms in adult men.

Objective: To study parameters which serve as criteria for differentiation of normal and abnormal uroflowmetrograms.

Subjects And Methods: A total of 105 subjects were investigated, 43 of whom had no symptoms of dysuria, 57 of whom had dysuria demonstrated as an abnormal uroflowmetrogram, subjective symptoms and a small residual urine volume, and five patients who were about to undergo transurethral resection of the prostate. The three parameters used to specify the shapes of the uroflowmetrogram curves were: (i) the ratio of the maximum flow rate (Qmax) and the voiding time (T100), Qmax/T100; (ii) the ratio of the time to peak flow (TQmax) and the voiding time, TQmax/T100; and (iii) the newly introduced voiding efficiency (epsilon), measured by the ratio of the voided volume (VV) and T100 x Qmax, epsilon = VV/(T100 x Qmax), were taken for each patient.

ISDN application for image transmission.

Image transmission using ISDN is not yet popular in Japan, but the number of ISDN terminals is rapidly increasing since 1991. We have two systems for image transmission using ISDN. One system uses an image workstation of the mini PACS.

[A case report of ureteral stump metastasis from renal-cell carcinoma].

This is a case report of the 21st documented case of ureteral metastasis from renal cell carcinoma. A 75-year-old woman was admitted because of asymptomatic gross hematuria. Right radical nephrectomy for renal cell carcinoma had been performed 2 years and 9 months prior to admission.

Effective cases of transcatheter arterioportal chemoembolization with high-dose iodized oil for hepatocellular carcinoma.

By administering an excessive amount of iodized oil via the hepatic artery, anticancer drugs in the iodized oil flow into the portal vein through the arterioportal communication. This phenomenon permits chemotherapy against extracapsular infiltration by a hepatocellular carcinoma (HCC) nourished by the portal blood flow. From May 1983 through July 1992, 240 cases of HCC underwent transcatheter arterioportal chemoembolization (TAPCE) with more than 5 ml of iodized oil (mean, 15 ml) in our hospital.

Follow-up study of combination treatment (TAE and PEIT) for unresectable hepatocellular carcinoma.

The subjects were 35 patients with unresectable hepatocellular carcinoma. The patients were divided into a transcatheter arterial embolization group (TAE group, 18 cases) and a combination therapy group receiving both TAE and percutaneous ethanol injection therapy (TAE+PEIT group, 17 cases). The 50% survival period was 21.

Dynamic MR imaging in the head and neck.

Purpose: To determine the utility of dynamic magnetic resonance (MR) imaging in evaluation of head and neck lesions.

Materials And Methods: Dynamic MR imaging was performed in 79 head and neck lesions. Five types of time versus signal intensity curves were obtained: type A, curve peaks 0-30 seconds after administration of contrast material; type B, curve peaks at 30-60 seconds; type C, curve peaks at 60-210 seconds; type D, gradual upward slope; and type E, flat.

[CT assessment of residual contrast medium in the kidney 12-24 hours after angiography].

Residual contrast medium in the kidney on delayed film is sometimes reported in patients with problems in the urinary system following the administration of contrast medium. However, we frequently found residual contrast medium in the kidney of patients with normal renal function on delayed CT after angiography. Thus we carried out CT examination 12-24 hours after angiography with four kinds of contrast media to prospectively study the incidence of residual contrast medium in the kidney.

[Studies on combination therapy with TAE and percutaneous ethanol injection therapy for unresectable hepatocellular carcinoma].

We studied and analysed the effects of combination therapy with TAE and PEIT for unresectable HCC. The subjects were 28 patients (21 males, 7 females) with 31 tumors treated in the Department of Surgery, Osaka Teishin Hospital. The patients were divided into four groups on the basis of the tumor size (< 3 cm, 3-5 cm, 5-8 cm, > 8 cm).

[Potential laser hazards to bystanders during photocoagulation].

The authors measured reflected laser beams delivered by slit-lamp photocoagulator and binocular indirect ophthalmoscope laser delivery systems, to evaluate potential hazards to the eyes of bystanders. 514 nm laser beams were used; reflected laser beams were measured by a light power meter (Advantest: TQ8210). In the case of a patient who had Charcot's joints in her left hip and right shoulders, multiple osteonecrosis in the joints were disclosed by MR imaging. This suggested that osteonecrosis may be a predisposing condition for Charcot's joints.

Two cases of urothelial tumor with high serum level of carcinoembryonic antigen and TA-4.

Two rare cases of urothelial tumor with high serum carcinoembryonic antigen (CEA) and TA-4 levels are presented. One is a case of lymph node metastasis due to bladder tumor: pathological diagnosis revealed mucoepidermoid carcinoma. The other is a case of left renal pelvic tumor: pathological diagnosis revealed transitional cell carcinoma with glandular and squamous differentiation.

Prognostic factor of telecobalt therapy for early glottic carcinoma.

Background: Local control rates of T1 and T2 glottic carcinoma treated with radiation alone were reported as 80-91% and 63-76%, respectively. The authors investigated the factors that affect the local control rate for early glottic carcinoma.

Methods: From 1967 through 1982, 330 patients with early glottic carcinoma (T1: 274 patients; T2: 56) were treated with telecobalt therapy at the Department of Radiology, Osaka University Hospital, Osaka, Japan.

Clinical assessment of tumor clearance during radiotherapy as a prognostic factor of early glottic carcinoma.

From 1967 through 1985, 358 cases of early glottic carcinoma were treated with telecobalt therapy at the Department of Radiology, Osaka University Medical School. Among 278 cases treated with 2 Gy a day, the tumor response of 262 cases at 40, 50 and 60 Gy were evaluated by direct or indirect laryngoscope. The five-year local control rates of these evaluable cases of T1 and T2 glottic carcinoma were 79% and 70%, respectively.

[CT assessment of gallbladder opacification 12-24 hours after angiography].

After the injection of conventional contrast agents that are largely excreted in urine, gallbladder opacification is observed in patients with renal dysfunction. However, some reports have noted that gallbladder opacification on plain radiographs occurs frequently in patients without renal impairment after the intravenous administration of Ioxaglate, one of the new low-osmolality agents. We carried out CT examination 12-24 hours after angiography with various kinds of contrast media in 437 patients to examine the incidence of gallbladder opacification.

[Necessity and limitations of effectiveness of oral beta-stimulants, theophylline and oral antiallergic drugs in asthma treatment].

Oral beta-stimulants, theophylline and oral antiallergic drugs are frequently used for asthma treatment in Japan. However, inhaled beta-stimulants and inhaled steroids are becoming the first and second choice of drugs in asthma treatment in European countries, and these three categories of drugs are becoming third or fourth-line therapeutic choices. Accordingly, we investigated the effectiveness of these drugs, which is limited by their pharmacological actions and effects.

[Intra-arterial infusion chemotherapy in the treatment of advanced breast cancer--doxorubicin versus epirubicin].

Doxorubicin (ADM) and epirubicin (epi-ADM) were tested at a dose of 150mg given intraarterially to 31 patients (ADM 14, epi-ADM 17) with advanced breast cancer. The clinical response rate was 78.6% in ADM (1 CR and 10 PR) and 76.

A new model of plastic ankle foot orthosis (FAFO (II)) against spastic foot and genu recurvatum.

A plastic ankle foot orthosis (AFO) was developed, referred to as functional ankle foot orthosis Type 2 (FAFO (II)), which can deal with genu recurvatum and the severe spastic foot in walking. Clinical trials were successful for all varus and drop feet, and for most cases of genu recurvatum. Electromyogram studies showed that the FAFO (II) reduced the spasticity of gastrocnemius and hamstring muscles and activated the quadricep muscles.

[Image-transmission using 64 kbps-ISDN (Integrated Services Digital Network) and mini-PACS].

We carried out the image-transmission using 64kbps-ISDN between two image workstations (IWS) of mini-PACS installed at the Department of Radiology, Osaka Teishin Hospital and the Kashiwa Works, Hitachi Medical Corporation. The distance between Osaka and Kashiwa is about 600 km. The subtraction of two images before and after transmission revealed no change of the data of the image during transmission.

[Combined effect of TNF and anticancer chemotherapeutic agents on murine bladder tumor (MBT-2)].

The antitumor activity of combination therapy of recombinant human tumor necrosis factor alpha (rHu-TNF alpha) and chemotherapeutic agents against murine bladder tumor (MBT-2) was studied. Tumors were intradermally transplanted into the hind leg of C3H-HeN mice and allowed to grow until they reached a diameter of 8 to 10 mm 10 days after transplantation. TNF (3,000 units) was given intraperitoneally from day 10 to day 19, and the chemotherapeutic agents, 5-FU (60 mg/kg), CPM (100 mg/kg), CDDP (10 mg/kg) and ADM (5 mg/kg), were applied once intraperitoneally on day 10.

Thymus lymphocytes in uraemic rats and the effect of thymosin fraction 5 in vivo.

Thymus lymphocyte subsets in uraemic rats were studied using monoclonal antibodies. Severe and moderate uraemia was induced in rats, and sham-operated and normal rats were used as the controls. As a result, the weight of the thymus decreased in uraemic rats.
