Article Synopsis
  • A 73-year-old man with diabetes treated with sitagliptin developed widespread, itchy red skin lesions suspected to be a drug eruption due to the medication.
  • Histopathological analysis revealed significant eosinophilic infiltration, leading to the discontinuation of sitagliptin and improvement with treatment, including prednisolone and phototherapy.
  • Five months later, he experienced blistering lesions, and further tests indicated he had anti-laminin γ1 pemphigoid, a rare condition linked to the DPP-4 inhibitor, with some supporting autoimmune response findings.

Calorie restriction (CR) can prolong human lifespan, but enforcing long-term CR is difficult. Thus, a drug that reproduces the effects of CR without CR is required. More than 10 drugs have been listed as CR mimetics (CRM), and some of which are conventionally categorized as upstream-type CRMs showing glycolytic inhibition, whereas the others are categorized as downstream-type CRMs that regulate or genetically modulate intracellular signaling proteins.

Article Synopsis
  • This study evaluated the long-term impacts of tofogliflozin, an SGLT2 inhibitor, on atherosclerosis and key health markers in type 2 diabetes patients without cardiovascular disease.
  • Over a 2-year period, both tofogliflozin and conventional treatments reduced carotid intima-media thickness, but with no notable difference between the two groups.
  • Tofogliflozin showed advantages in improving blood sugar, cholesterol, BMI, waist circumference, and blood pressure compared to conventional treatment, while safety metrics were not significantly different between groups.

Purpose: To evaluate the correlation between the position of a ureteral stent and stent-related symptoms in a single-center randomized study.

Methods: A total of 113 patients who required ureteral stent placement after lithotripsy were randomized at a 1:1 ratio into groups with stents crossing and not crossing the bladder midline. The ureteral stent remained in place until postoperative day 14, when we obtained each patient's International Prostate Symptom Score (IPSS), overactive bladder symptom score (OABSS), and visual analog scale (VAS) pain score.

Acute focal bacterial nephritis (AFBN) refers to the bacterial infection of the renal parenchyma without abscess formation. Although AFBN has mainly been reported in pediatric patients, it may be underdiagnosed in adults as it resembles acute pyelonephritis in its clinical presentation. However, the symptoms suggesting acute abdomen is an important clue to diagnose AFBN, which requires additional imaging studies such as contrast-enhanced computed tomography (CECT).

Article Synopsis
  • Autosomal dominant tubulointerstitial kidney disease (ADTKD) is a hereditary condition linked to various genetic mutations that can lead to end-stage kidney disease, often going undiagnosed due to non-specific symptoms.
  • A study analyzed kidney biopsies from 13 patients with uromodulin (UMOD) gene mutations, focusing on protein accumulation and common pathological features like fibrosis and atrophy.
  • Findings revealed that all patients had interstitial fibrosis and tubular atrophy, and significant UMOD protein accumulation was found, indicating a potential correlation between these findings and kidney function.

Background: Acute coronary syndrome (ACS) with high-grade atrioventricular block (HAVB) still has a poor mortality risk, even in the current percutaneous coronary intervention (PCI) era. However, early PCI for ACS with HAVB is associated with improved in-hospital survival and a 6-month survival similar to that of ACS without HAVB.

Case Presentation: A 70-year-old man was admitted to our hospital for ACS with HAVB.

Background: Takotsubo cardiomyopathy (TC) is a cardiomyopathy that shows distinctive clinical conditions first described more than 20 years ago. Because clinical features of TC mimic those of anterior acute myocardial infarction (AMI), the differential diagnosis is important in selecting the appropriate treatment strategy in the acute phase. But it was difficult to differentiate those two diseases because the TC-like findings; such as the electrocardiogram (ECG) changes and left ventricular wall motion abnormality can occur in AMI especially with the anatomical variance of the coronary artery.
