Article Synopsis
  • - This study investigated whether starting vasopressor treatment early (within 1 hour of recognizing sepsis) improves the outcomes for patients in septic shock compared to starting the treatment later.
  • - Conducted in 17 intensive care units in Japan with 97 adult sepsis patients, the results showed that the early group had a slightly higher in-hospital mortality rate (32.8%) compared to the delayed group (26.7%), but the difference was not statistically significant.
  • - While the findings weren't conclusive on the overall benefits of early vasopressor use, they suggested it might help prevent volume overload during prolonged sepsis treatment.

Background Modification of arrhythmogenic substrates with extensive ablation comprising linear and/or complex fractional atrial electrogram ablation in addition to pulmonary vein isolation (PVI-plus) can theoretically reduce the recurrence of atrial fibrillation. The DR-FLASH score (score based on diabetes mellitus, renal dysfunction, persistent form of atrial fibrillation, left atrialdiameter >45 mm, age >65 years, female sex, and hypertension) is reportedly useful for identifying patients with arrhythmogenic substrates. We hypothesized that, in patients with persistent atrial fibrillation, the DR-FLASH score can be used to classify patients into those who require PVI-plus and those for whom a PVI-only strategy is sufficient.

Aim: We aimed to identify subphenotypes among patients with out-of-hospital cardiac arrest (OHCA) with initial non-shockable rhythm by applying machine learning latent class analysis and examining the associations between subphenotypes and neurological outcomes.

Methods: This study was a retrospective analysis within a multi-institutional prospective observational cohort study of OHCA patients in Osaka, Japan (the CRITICAL study). The data of adult OHCA patients with medical causes and initial non-shockable rhythm presenting with OHCA between 2012 and 2016 were included in machine learning latent class analysis models, which identified subphenotypes, and patients who presented in 2017 were included in a dataset validating the subphenotypes.
