Background: Patients with acute ischemic stroke and active cancer have more severe neurological symptoms, elevated risks of stroke recurrence, and death compared with the general population. We examined whether von Willebrand factor (vWF) antigen levels at stroke onset were associated with the poor outcomes of patients with stroke and cancer.

Methods And Results: Using data from 90 patients with acute ischemic stroke and active cancer who were registered in the SCAN (Ischemic Stroke in Patients With Cancer and Neoplasia) study, a prospective multicenter, observational study in Japan, we divided patients into 2 groups according to their median vWF antigen levels (high, n=46; or low, n=44).

  • Hybrid emergency room systems (HER) allow for CT scans, surgery, and interventional radiology to be done without moving the patient, reducing critical time for treatment in severe trauma cases.
  • A patient with significant injuries, including artery occlusion and organ damage from blunt trauma, initially suffered cardiac arrest after a CT scan but was resuscitated through quick interventions performed within the HER.
  • This case demonstrates that the HER system effectively facilitates rapid diagnosis and treatment, improving survival rates in patients with severe blunt trauma.

Disseminated intravascular coagulation (DIC) is a frequent but lethal complication in sepsis. Anticoagulant therapies, such as heparin, antithrombin, activated protein C, and recombinant human-soluble thrombomodulin, were expected to regulate the progression of coagulopathy in sepsis. Although a number of randomized controlled trials (RCTs) have evaluated the survival effects of these therapies over the past few decades, there remains no consistent evidence showing a significant survival benefit of anticoagulant therapies.

Article Synopsis
  • - This study investigated whether starting vasopressor treatment early (within 1 hour of recognizing sepsis) improves the outcomes for patients in septic shock compared to starting the treatment later.
  • - Conducted in 17 intensive care units in Japan with 97 adult sepsis patients, the results showed that the early group had a slightly higher in-hospital mortality rate (32.8%) compared to the delayed group (26.7%), but the difference was not statistically significant.
  • - While the findings weren't conclusive on the overall benefits of early vasopressor use, they suggested it might help prevent volume overload during prolonged sepsis treatment.
