Study Objective: To estimate the efficacy of the vasopressin injection technique for laparoscopic cystectomy of ovarian endometriomas with regard to operative time and coagulation events.

Design: Prospective study (Canadian Task Force Classification II-1)

Setting: Osaka Central Hospital in Osaka, Japan

Patients: Fifteen women with single lobular similarly-sized ovarian endometrioma.

Interventions: Women who planned to undergo laparoscopic cystectomy of endometrioma were allocated randomly to 3 groups: (1) ordinary laparoscopic cystectomy without injection, (2) laparoscopic cystectomy with the injection of saline solution, and (3) laparoscopic cystectomy with the vasopressin injection technique.

Introduction: A purpose of the surgical treatment for pelvic organ prolapse (POP) is not only the anatomical restoration of the patients but also the improvement of their quality of life (QOL). The aims of this study were, using fixed and fresh cadavers, to clarify how closely the pelvic splanchnic nerve, levator ani nerve (LAN), and sacrospinous ligament (SSL) are located and to examine how to avoid nerve injury during the posterior tension-free vaginal mesh procedure (TVMP), in which mesh is applied through the SSL. Macroscopic dissection of fixed cadavers demonstrated that the LAN crossed the inside of the SSL at a point 0-18 mm medial to the ischial spine and entered the muscle at a point 12-26 mm inferior to the ischial spine and 40-55 mm anterolateral to the coccyx. Dissection after TVMP of fresh cadavers revealed that the point of penetration of mesh through the SSL was close to the LAN.

Objectives: The objectives of this clinical study is to demonstrate our experience of the abdominal approach (from the abdominal wall to the vagina) for the tension-free vaginal tape (TVT) operation and to identify factors responsible for complications relating to the puncture.

Materials And Methods: One hundred and fifty-one cases of patients with stress incontinence who underwent the tension-free vaginal tape operation were analyzed. Among them, 75 cases underwent the TVT procedure with the vaginal approach (from the vagina to the abdominal wall) and the remaining 76 cases underwent the TVT procedure with the abdominal approach (from the abdominal wall to the vagina).

Purpose: To evaluate the safety and efficacy of a new vessel-sealing device (LigaSure) for laparoscopic varicocelectomy.

Materials And Methods: To evaluate the safty, after elective varicocelectomy, the internal spermatic veins (n = 8) were sealed ex vivo with a 5-mm laparoscopic Maryland-style LigaSure (LigaSure Lap), and bursting pressures were measured. To evaluate the efficacy, a retrospective review of our clinical experience with LigaSure (n = 13) and clip (n = 13) in laparoscopic bilatelal varicocelectomy from June 2000 to October 2002 was performed and certain parameters were abstracted, including operative time, estimated blood loss and perioperative complications.

A 51-year-old male with a recurrent medullary thyroid carcinoma (MTC) showed high serum levels of calcitonin (CT) and carcinoembryonic antigen (CEA) but with concomitant normocalcemia. The CT activities of the patient's serum in vivo (hypocalcemic activity in rats) and in vitro (cyclic AMP-increasing activity in cultured human mammary tumor, T47D) were much lower than those expected from the patient's CT level. High performance liquid chromatography (HPLC) with radioimmunoassay (RIA) detection gave a broad spectrum of molecular weights of CT-like polypeptides in the 3 kDa to 200 kDa range with separation from CEA, suggesting that the mechanism for CT production in MTC is an obscure one.

A prospective uncontrolled study of the safety and efficacy of transrectal hyperthermia was performed on 15 patients with chronic abacterial prostatitis or prostatodynia. A total of 6 (1-2 per week) 1-hour sessions of hyperthermia were performed. Subjective improvement was fair in 2 cases and slight in 13 cases (about 87%).
