Highly selective inhibitors of cyclin-dependent kinase 4 and 6 (CDK4/6is) have emerged as a standart of care for first- and second-line therapies in combination with endocrine therapy (ET) for HR+/HER2- metastatic breast cancer (MBC) patients. It has been reported that combination therapy is more effective than ET alone and is safe in elderly patients as well as young patients. Nevertheless, elderly and very old patients with HR+/HER2-MBC treated with CDK4/6 inhibitor (CDK4/6i) combinations are relatively underrepresented in randomized controlled trials.

December 2024

University of Health Sciences, Ankara Oncology Training and Research Hospital, Department of Hematology & Apheresis Unit, Ankara, Turkey; Ankara Yildirim Beyazit University, School of Medicine, Department of Internal Medicine, Division of Hematology, Ankara, Turkey.

Objectives: Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a potentially curative treatment for hematological diseases, with success rates improving due to advancements in conditioning regimens and new anti-graft versus host disease (GVHD) drugs. Ruxolitinib, an oral selective Janus kinase (JAK) 1 and 2 inhibitor has been used to mitigate the effects of various inflammatory and myeloproliferative syndromes, given the JAK kinase pathway's central role in cytokine signaling during inflammatory and immune processes. In this study we aimed to assess ruxolitinib's efficacy in patients with chronic GVHD (cGVHD).

Evolving Clinical Features of Diabetic Ketoacidosis: The Impact of SGLT2 Inhibitors.

Pharmaceuticals (Basel)

November 2024

Department of Endocrinology and Metabolism, Faculty of Medicine, Recep Tayyip Erdogan University, Rize 53100, Türkiye.

: The antidiabetic effect of SGLT2 inhibitors (SGLT2-is) is based on their ability to increase glucose excretion through urine by inhibiting the kidney-resident SGLT2 protein. Euglycemic diabetic ketoacidosis (EuDKA) is an uncommon but potentially life-threatening adverse effect of these medications, which are notable for their antidiabetic, cardiovascular, and renal protective properties. This study aimed to clarify the impact of SGLT2-is on demographic, clinical, and biochemical characteristics in patients with DKA.

Article Synopsis
  • Accurate staging of prostate cancer is crucial for treatment decisions; this study compares the effectiveness of Ga-68 PSMA PET/CT and mpMRI in preoperative assessments.
  • In a study involving 78 patients, mpMRI showed greater sensitivity for detecting extraprostatic extension and bladder neck invasion, while Ga-68 PSMA PET/CT was more effective for identifying seminal vesicle invasion and lymph node metastasis.
  • When both imaging methods were used together, there was a significant improvement in the accuracy of tumor localization, reaching over 72%, highlighting the benefits of using complementary imaging techniques.

Article Synopsis
  • This study investigates the effects of extensive surgical resection on outcomes for patients with wild-type IDH glioblastoma, considering the influence of genetic mutations like TERT and EGFR.
  • The analysis included 41 out of 723 patients who underwent either partial or extensive surgeries from 2014 to 2022, focusing on tumor recurrence-free periods and overall survival rates.
  • Results showed that extensive resection led to longer tumor-free intervals and better quality of life scores, although it did not significantly improve overall survival rates despite the presence of TERT mutations and EGFR amplification.

The incidence and prevalence of neuroendocrine neoplasms (NENs) in many organs are increasing. Although such NENs have similar grades, they may exhibit quite different behaviors. In this multicenter study, we aimed to investigate the prevalence and distribution of different morphological NEN variants in the non-pancreatic gastrointestinal (GI) tract and determine whether they can guide prognosis prediction.

Article Synopsis
  • The study aimed to evaluate how different riboflavin solutions and soaking times affect corneal crosslinking outcomes in patients with progressive keratoconus.
  • Patients were divided into four groups, each receiving different riboflavin formulations and soaking durations, followed by UV irradiation, and their corneal structures were assessed at one month and correlated to clinical results after one year.
  • Results showed that while the corneal structures varied, particularly in demarcation line depth with dextran vs. HPMC solutions, visual and refractive outcomes remained consistent across all treatment groups after one year.

HbA2 levels in children with β-thalassemia trait associated with iron deficiency: A perspective for pediatricians.

Am J Clin Pathol

July 2024

Section of Hematology, Department of Pediatrics, Faculty of Medicine, Gazi University, Ankara, Turkey.

Article Synopsis
  • Hemoglobin A2 (HbA2) levels of 3.5% or higher are crucial for screening β-thalassemia trait, but iron deficiency can lower these levels, complicating diagnosis, especially in children.
  • A study examined 213 children with β-thalassemia trait to compare HbA2 levels in those with iron deficiency versus those who were iron-sufficient, finding that iron-deficient children still showed significant HbA2 levels to be diagnosed.
  • The results indicate that low HbA2 levels may relate more to thalassemia mutations than iron deficiency, suggesting that screening for β-thalassemia should proceed regardless of iron status.

Background: Aortic aneurysms, particularly those affecting the ascending aorta, pose significant health risks due to their potential to cause life-threatening complications such as rupture and dissection. While the etiology of ascending aortic aneurysms has traditionally been associated with non-inflammatory processes, emerging evidence suggests a potential role of inflammation in their development.

Methods: This study investigates the relationship between inflammatory markers and ascending aortic aneurysms, focusing on high-sensitivity C-reactive protein (hs-CRP) and the monocyte-to-HDL ratio (MHR).

Familial Hemophagocytic Lymphohistiocytosis Screening in Neonatal Sepsis.

J Pediatr Hematol Oncol

August 2024

Department of Pediatrics, Division of Neonatology, Faculty of Medicine, Karadeniz Technical University, Trabzon, Turkey.

Objective: Neonatal sepsis and familial hemophagocytic lymphohistiocytosis (fHLH) have similar clinical and laboratory symptoms and the possibility of overlooking fHLH diagnosis is high in newborns with sepsis. History of consanguineous marriage and/or sibling death, hepatomegaly/splenomegaly, and hyperferritinemia (>500 ng/mL) are likely to support fHLH in newborns with sepsis. Therefore, in newborns with sepsis in whom at least 2 of these 3 criteria were detected, genetic variants was investigated for the definitive diagnosed of fHLH.
