Acute bacterial orchitis (AO) is a prevalent cause of intrascrotal inflammation, often resulting in sub- or infertility. A frequent cause eliciting AO is uropathogenic Escherichia coli (UPEC), a gram negative pathovar, characterized by the expression of various iron acquisition systems to survive in a low-iron environment. On the host side, iron is tightly regulated by iron regulatory proteins 1 and 2 (IRP1 and -2) and these factors are reported to play a role in testicular and immune cell function; however, their precise role remains unclear.

The male reproductive system has a standard immune response regulatory mechanism, However, a variety of external stimuli, including viruses, bacteria, heat, and medications can damage the testicles and cause orchitis and epididymitis. It has been shown that various RNA viruses are more likely to infect the testis than DNA viruses, inducing orchitis and impairing testicular function. It was found that local injection of the viral RNA analog poly(I:C) into the testes markedly disrupted the structure of the seminiferous tubules, accompanied by apoptosis and inflammation.

In the western North Pacific, prominent granulomatous testes have been detected in many Brucella-infected common minke whales (Balaenoptera acutorostrata), but there have been no reports in toothed cetaceans. We found severe orchitis with granulomatous lesions in a rough-toothed dolphin (Steno bredanensis) stranded on the Pacific coast of Japan in 2011. Histopathological examination revealed leukocyte infiltration of the lesions.

Background: Epididymitis is a common cause of scrotal pain in adults, with coliform bacteria being the most common isolated organisms in patients older than 35.

Case Presentation: A 51-year-old healthy patient presented with scrotal pain and swelling, and was found to have epididymo-orchitis and bacteraemia caused by , which has not previously been reported as a cause of epididymo-orchitis and bacteraemia in immunocompetent patients.

Discussion: Diagnostic studies can help confirm the diagnosis and detect the causative pathogen.

Inflammation in the testes induced by infection and autoimmunity contributes significantly to male infertility, a public health issue. Current therapies using antibiotics and broad-spectrum anti-inflammatory drugs are ineffective against non-bacterial orchitis and induce side effects. This highlights the need to explore the pathogenesis of orchitis and develop alternative therapeutic strategies.

  • Epididymal orchitis is a common urological condition typically treated with medication, but can lead to serious complications like testicular infarction, especially in adolescents.* -
  • A case study of a 13-year-old boy showed that a urinary tract infection caused acute epididymal orchitis and subsequent testicular damage, leading to an imbalance in testicular volume and hormone levels.* -
  • The report highlights the need for prompt antibiotic treatment to prevent severe testicular damage and recommends monitoring the function of the unaffected testis during recovery.*

Background: Infection and inflammation of the genital tract are major potentially treatable factors contributing to male infertility. The profile of small non-coding RNA (sncRNAs) in spermatozoa can be altered by environmental exposures and inflammatory conditions.

Objectives: Experimental autoimmune epididymo-orchitis (EAEO) is a well-established model of autoimmune-induced chronic testicular and epididymal inflammation.

Varicocele and testicular cord torsion: immune testicular microenvironment imbalance.

Front Cell Dev Biol

November 2023

Universidad de Buenos Aires, Facultad de Medicina, Departamento de Biología Celular e Histología/Unidad Académica II, Buenos Aires, Argentina.

The main functions of the testis, steroidogenesis and spermatogenesis, depend on the endocrine axis and systemic and local tolerance mechanisms. Infectious or non-infectious diseases may disturb testicular immune regulation causing infertility. Literature has illustrated that bacterial and viral infections lead to autoimmune infertility: either sperm antibodies or autoimmune epidydimo-orchitis.

Article Synopsis
  • Epidemiological analysis of 1,590 human brucellosis cases revealed that most patients (72.08%) were male, with a significant number being farmers and having contact with livestock, mainly sheep and cattle, which were identified as primary risk factors.
  • The most common symptoms included fatigue, joint pain, and fever, with arthritis and spondylitis being prevalent complications, alongside notable alterations in laboratory markers indicating a decline in nutritional metabolism and immunity.
  • Antibiotic therapy proved effective for 86.73% of patients, although 13.27% experienced treatment failures or relapses, highlighting the challenges in diagnosing and managing this disease.

  • It commonly impacts organs like the skin, nervous system, and gastrointestinal tract, and distinguishing features include the lack of granulomas and specific antibodies present in other vasculitides.
  • While historically associated with hepatitis B virus, PAN is now more frequently linked to genetic syndromes and neoplasia, with this article reviewing its clinical progression over time.

Antimicrobial resistance is a major global problem that is primarily driven by the excessive and inappropriate utilization of antibiotics. Urinary tract infections (UTIs) are frequent in primary health care (PHC) and are typically treated with antibiotics. There is ample evidence on the management of this condition in women but not in men.

Background: Testicular torsion (TT) is a urological emergency that needs early diagnosis and intervention to prevent testicular death and necrosis. This study aimed to determine the efficacy of testicular scintigraphy (TS) in confirming the clinical diagnosis of TT and how this imaging method correlates with the surgical findings.

Methods: A retrospective cohort review of clinical data was performed for 68 patients referred for TS from January 2016 to December 2021 to rule out possible TT.

  • The study aimed to identify missed diagnoses of sexually transmitted infections (STIs) among patients experiencing lower genitourinary tract symptoms (LGUTS) within a specific timeframe in the US (2010-2019).
  • Researchers analyzed insurance claims data from over 23 million episodes, focusing on a GAP cohort of 833,574 cases where STIs may have been overlooked during initial evaluations.
  • Results revealed low rates of chlamydia and gonorrhea testing (4.6% in men, 5.4% in women) and high incidences of misdiagnoses as urinary tract infections (UTIs), indicating a significant lack of STI consideration during treatment.

Article Synopsis
  • Furan is a harmful food contaminant that can damage organs, including the testis, and the study investigates how salidroside (SAL) can protect against this damage in mice.
  • The research involved treating mice with furan and different doses of SAL, finding that SAL prevented testicular damage by restoring vital protein functions and reducing inflammation via specific biological pathways.
  • SAL also improved gut health by altering gut microbiota and metabolites, which helped prevent intestinal stress and apoptosis, thereby protecting the blood-testis barrier and reducing overall testicular inflammation.
