Background And Aims: Gastrointestinal (GI) symptoms occur among patients diagnosed with coronavirus disease 2019 (COVID-19), and there is clear evidence that SARS-CoV-2, the causative pathogen, infects the GI tract. In this large, multicenter cohort study, we evaluated variations in gastrointestinal and hepatic manifestations of COVID-19 throughout the United States (US).

Methods: Patients hospitalized with a positive COVID-19 test prior to October 2020 were identified at 7 US academic centers.

Article Synopsis
  • The CODA trial found that outpatient management of acute appendicitis using antibiotics is a safe and effective alternative to immediate surgery, with about 50% of participants discharged within 24 hours.
  • The cohort study analyzed data from 776 adults with imaging-confirmed appendicitis who received antibiotics and assessed the safety and outcomes of outpatient versus inpatient care.
  • Results indicated that most treated participants managed their appendicitis without needing surgery, and outpatient management could reduce hospital visits, increase convenience, and lower healthcare costs.

Objective: This study aimed to use a large population-based sample to investigate age-associated differences in mental distress among sexual and gender minority (SGM) adults compared with their heterosexual, cisgender counterparts.

Methods: Data were pooled from five cycles (2014-2018) of the Behavioral Risk Factor Surveillance System (BRFSS) survey (N=762,541) and included states that administered the optional SGM module during that interval. Mean days of self-reported mental distress and the rate of frequent mental distress (≥14 days of mental distress per month) were calculated for each age and SGM identity stratum by using linear and logistic regression, respectively.

  • Ki67 is a key biomarker in breast cancer but struggles with consistent measurement between labs, prompting research into automated digital image analysis (DIA).
  • The study assessed Ki67 levels using specially prepared slides from 30 breast cancer patients, with analysis conducted by 17 labs to evaluate consistency in scoring.
  • Results showed a strong correlation between consecutive sections, but core biopsy samples had notably higher Ki67 scores than whole sections, indicating that core biopsies may provide a more accurate biological status of the tumor.

Gastrostomy tube placement is a procedure that achieves enteral access for nutrition, decompression, and medication administration. Preprocedural evaluation and selection of patients is necessary to provide optimal benefit and reduce the risk of adverse events (AEs). Appropriate indications, contraindications, ethical considerations, and comorbidities of patients referred for gastrostomy placement should be weighed and balanced.

Importance: For adults with appendicitis, several randomized clinical trials have demonstrated that antibiotics are an effective alternative to appendectomy. However, it remains unknown how the characteristics of patients in such trials compare with those of patients who select their treatment and whether outcomes differ.

Objective: To compare participants in the Comparison of Outcomes of Antibiotic Drugs and Appendectomy (CODA) randomized clinical trial (RCT) with a parallel cohort study of participants who declined randomization and self-selected treatment.

Article Synopsis
  • A nationwide study analyzed the correlation between patient-reported gastroesophageal reflux disease (GERD) symptoms and endoscopic findings using data from the National Endoscopic Database from 2000 to 2014.
  • A total of 167,459 esophagogastroduodenoscopies (EGDs) were evaluated, with the majority of patients categorized by typical reflux symptoms, airway symptoms, or both.
  • Results showed that patients with both reflux and airway symptoms had higher rates of reflux esophagitis and hiatal hernia, while those with only reflux symptoms had a higher prevalence of Barrett's esophagus, suggesting implications for GERD management and treatment strategies.
