BACKGROUND Virtual reality (VR)-guided GC simulation for patients with anatomical anomalies using cardiac computed tomography (CT) has been recently reported. Rotational atherectomy (RA) for the left circumflex (LCX) ostium is challenging due to the tortuous anatomy, acute angulation, and variable vessel size compared to other lesions. The appropriate positioning and coaxiality of the guide catheter (GC) are key factors for safely performing RA.

Article Synopsis
  • Chronic total occlusion (CTO) PCI is complex and varies in success rates among the three major coronary arteries due to differences in lesion characteristics.
  • The study analyzed 6,408 first-attempt CTO patients from a Japanese registry to identify predictors of unsuccessful PCI and developed a difficulty score for surgical outcomes.
  • Results showed that success rates were lowest for the left circumflex artery, with key differences in predictors of failure across arteries, and the new difficulty score outperformed the J-CTO score in forecasting PCI success.

  • Directional coronary atherectomy (DCA) is a method to remove plaque from coronary arteries, but it typically requires an 8Fr guiding catheter, which poses certain limitations.
  • The study explored using a transradial approach (TRA) to perform DCA with an 8Fr sheathless guiding catheter, which can be inserted if a 6Fr sheath fits easily.
  • In five successful cases, this new approach proved effective and not uncomfortable for patients, potentially reducing bleeding complications during the procedure.

Article Synopsis
  • Directional coronary atherectomy (DCA) is a technique revived in Japan in 2014 that removes plaque from coronary arteries during procedures to unblock them.
  • A case study on a 91-year-old woman with angina illustrated how DCA was used to treat a complex artery blockage without placing a stent, due to her high bleeding risk.
  • The use of optical coherence tomography (OCT) provided detailed imaging of the blockage, allowing for successful removal and subsequent balloon treatment, suggesting that this approach is viable for patients who are at risk for bleeding.
