Well-leg compartment syndrome after laparoscopic low anterior resection in the lithotomy position: a case report and literature review.

J Surg Case Rep

April 2024

Faculty of Medicine, Department of Digestive and General Surgery, Shimane University, 89-1, Enya-cho, Izumo City, Shimane 693-8501, Japan.

Well-leg compartment syndrome (WLCS) develops in healthy lower limbs because of surgical factors such as operative position, lower limb compression, and long operative time during abdominopelvic surgery. WLCS can lead to irreversible muscle and nerve damage if a prompt diagnosis and appropriate treatment are not provided. We report the case of a 57-year-old male who developed rectal cancer immediately after laparoscopic low anterior resection and was successfully treated with fasciotomy without sequelae.

An 82-year-old man was treated with ipilimumab and nivolumab for malignant pleural mesothelioma. Although he was previously treated with prednisolone (1 mg/kg/day) for immune-related adverse event (irAE) hepatitis by a previous doctor, he still had worsening liver function and was transferred to our hospital. Blood tests and imaging findings were negative for autoimmune and infectious hepatitis, and liver biopsy results were consistent with irAE hepatitis.

Article Synopsis
  • Multiple acyl-CoA dehydrogenase deficiency (MADD) is an inherited metabolic disorder linked to mutations in specific genes (ETFA, ETFB, ETFDH) and has symptoms that can be confused with riboflavin metabolism disorders.
  • A study of 37 Japanese patients revealed that most MADD cases were due to ETFDH deficiency, with a high mortality rate—15 out of 15 neonatal patients died by age two, and 5 out of 22 later-onset patients died by age three.
  • The study highlighted the p.Y507D genetic variant as a common and dangerous mutation, which is associated with severe outcomes, indicating that many affected patients in Japan do not respond well to riboflavin

We report a case of diffuse large B-cell lymphoma (DLBCL) in the ampulla of Vater, causing painless obstructive jaundice in a 78-year-old woman. Duodenal endoscopy revealed a mass in the ampulla of Vater and narrowing of the second portion of the duodenum, although diagnosing DLBCL from an endoscopic biopsy was impossible because there were several kinds of leukocytes in the infiltrate. We performed pylorus-preserving pancreatoduodenectomy to establish a histological diagnosis, relieve the obstructive jaundice, and remove the narrowed second portion of the duodenum.
