Background The detection of in-stent restenosis (ISR) with coronary CT angiography (CCTA) is challenging, but CT perfusion (CTP) has demonstrated improved diagnostic accuracy over CCTA in patients with stents. However, there are limited data on the performance of dynamic CTP, which allows noninvasive adjudication of regional myocardial blood flow. Purpose To compare the diagnostic performance of regadenoson-stress dynamic CTP with that of CCTA, using fractional flow reserve (FFR) and the index of microvascular resistance (IMR) as reference standards for epicardial coronary circulation and coronary microcirculation, respectively.

In the last three decades, ablation of atrial fibrillation (AF) has become an evidence-based safe and efficacious treatment for managing the most common cardiac arrhythmia. In 2007, the first joint expert consensus document was issued, guiding healthcare professionals involved in catheter or surgical AF ablation. Mounting research evidence and technological advances have resulted in a rapidly changing landscape in the field of catheter and surgical AF ablation, thus stressing the need for regularly updated versions of this partnership which were issued in 2012 and 2017.

Article Synopsis
  • Many patients with atrial fibrillation (AF) are not receiving or are stopping oral anticoagulation therapy, despite its importance for stroke prevention.
  • While direct oral anticoagulants (DOACs) are available, issues like bleeding risks, poor compliance, and aversion to treatment contribute to this problem.
  • A recent expert consensus guide highlights left atrial appendage closure (LAAC) as a safe alternative for stroke prevention in patients who cannot use long-term anticoagulation, detailing the devices, implantation technique, and follow-up requirements for non-implanting physicians.

  • Robotic-assisted coronary interventions (R-PCI) have shown safety and effectiveness in treating various coronary lesions, but their real-world clinical outcomes remain unclear.
  • A study evaluated 111 patients to assess major adverse cardiovascular events (MACE) and found a low occurrence of 5.4% during a median follow-up period.
  • The research indicated that while procedural complexity increased time and radiation exposure, it did not impact in-hospital or long-term outcomes, and CCTA-guided procedures allowed for a higher rate of same-day discharge (64.6% vs. 44.2%).

Article Synopsis
  • This study assesses the outcomes of adrenalectomy for pheochromocytoma in obese versus nonobese patients, using data from 46 centers from 2012 to 2022.
  • It finds that while obesity does not significantly increase the rate of complications or comprehensive complication index (CCI), it does lead to a longer hospital stay for patients.
  • Furthermore, minimally invasive surgical techniques, like laparoscopic and robotic surgeries, are associated with less morbidity in obese patients, and a retroperitoneal approach may provide additional benefits.

Cardiovascular magnetic resonance imaging in mitral valve disease.

Eur Heart J

November 2024

Department of Cardiovascular Medicine, Gagnon Cardiovascular Institute, Morristown Medical Center, 100 Madison Avenue, Morristown, NJ 07960, USA.

Article Synopsis
  • - This paper discusses how cardiovascular magnetic resonance (CMR) imaging is used to evaluate patients with mitral valve disease, specifically focusing on mitral regurgitation (MR), a common heart valve issue that often has no symptoms but can lead to serious complications.
  • - CMR is particularly recommended when traditional ultrasound imaging doesn't provide clear information, as it offers detailed insights into the mitral valve structure and function, including aspects like leaflet shape and how MR affects heart remodeling.
  • - The review also aims to present both current and innovative CMR techniques for assessing mitral valve disease, including flow and myocardial evaluations, and suggests a simplified flow chart for healthcare providers to follow when performing CMR assessments.

The androgen receptor signaling inhibitor apalutamide is used successfully for the treatment of prostate cancer. An increased risk of hypothyroidism, mostly subclinical, has been reported in the SPARTAN and TITAN trials. We present three cases of subacute deterioration of previously known but well-controlled hypothyroidism treated with levothyroxine, occurring shortly after the initiation of treatment with apalutamide, resulting in severe hypothyroidism.
