Fibroblast activation protein (FAP)-targeted radionuclides are known as a promising step in cancer diagnosis and treatment (theranostic), although it is still a subject under investigation. We present a patient with anaplastic thyroid carcinoma who underwent 99mTc-FAPI-46 SPECT/CT for the evaluation of metastatic disease. Several metastatic sites were found with considerable 99mTc-FAPI-46 uptake.

Utilizing 99mTc-FAPI-46 scintigraphy, we observed a high target-to-background ratio in the breast mass and metastatic axillary lymph nodes. Notably, a benign follicular nodule was also detected in the thyroid which showed absent 99mTc-FAPI uptake.

We present a 74-year-old man with a history of very high-risk prostate cancer (Gleason score: 5+4 in 11 of 14 cores) who was referred for initial staging with 99mTc-HYNIC-PSMA. The scan was reported as miT2m N2(REI, ROB) M1b(uni) M1c(skin?), PRIMARY score: 4, PSMA expression score: 1-3. We incidentally discovered increased uptake in the left skull region on whole-body images.

A 34-year-old woman with a history of renal transplantation and cervical cancer underwent lymphoscintigraphy using 99mTc-phytate before surgery. Initial planar images revealed multiple foci of uptake, which were later precisely localized using SPECT/CT imaging. Notably, uptakes in the right lower quadrant were attributed to the pyelocalyceal system of the transplanted kidney, highlighting a potential pitfall in patients with ectopic or transplanted kidneys.

We present the case of a 58-year-old man with metastatic medullary thyroid carcinoma refractory to conventional therapies, including peptide receptor radionuclide therapy. Despite multiple interventions, serum calcitonin and carcinoembryonic antigen levels continued to rise. Subsequent evaluation with 99m Tc-FAPI-46 revealed remarkable uptake in metastatic lesions, suggesting a potential role for FAPI-labeled radioisotopes in the management of medullary thyroid carcinoma.

Breast cancer lymphoscintigraphy is a crucial tool in pre-operative assessment, typically revealing sentinel lymph node drainage patterns within axillary and extra-axillary regions. However, rare cases challenge conventional understanding. Since the 1940s, 131-I radioiodine therapy (RIT) has been the primary treatment for metastatic differentiated thyroid cancer (DTC). Approximately half of these patients respond favorably to RIT, achieving partial or complete remission or maintaining long-term stable disease, while the other half develop radioiodine-refractory DTC (RAI-R DTC). The main genomic alteration involved in radioiodine resistance is the activated mitogen-activated protein kinase (MAPK) pathway, which results in the loss of sodium iodide symporters (NIS).

Equilibrium radionuclide angiography (ERNA) has long been assumed as the preferred method to assess cardiac volumes as well as left ventricular systolic and diastolic indices. ERNA was used to diagnose subtle changes in cardiac function during chemotherapy or early stages of heart failure. Gated myocardial perfusion SPECT (GMPS) was introduced as a more feasible and versatile alternative to ERNA, but the precision of GMPS to assess systolic and diastolic indices has not yet been fully reviewed.
