Fibroblast activation protein (FAP)-targeted radionuclides are known as a promising step in cancer diagnosis and treatment (theranostic), although it is still a subject under investigation. We present a patient with anaplastic thyroid carcinoma who underwent 99mTc-FAPI-46 SPECT/CT for the evaluation of metastatic disease. Several metastatic sites were found with considerable 99mTc-FAPI-46 uptake.

This case report describes a 41-year-old man with a progressively enlarging, painful vegetative lesion in the perianal region, ultimately diagnosed as squamous cell carcinoma (SCC), through a punch biopsy. Sentinel lymph node (SLN) mapping was performed via preoperative lymphoscintigraphy utilizing 99mTc-Phytate. Notably, an SLN was identified in an atypical location, below the anterior superior iliac spine (ASIS) in the subcutaneous tissue, diverging from typical drainage patterns associated with perianal SCC.

We present a case of a 65-year-old woman diagnosed with mixed mucinous-invasive ductal carcinoma, a rare subtype of breast cancer. Utilizing 99mTc-FAPI-46 scintigraphy, we observed a high target-to-background ratio in the breast mass and metastatic axillary lymph nodes. Notably, a benign follicular nodule was also detected in the thyroid which showed absent 99mTc-FAPI uptake.

We present a 74-year-old man with a history of very high-risk prostate cancer (Gleason score: 5+4 in 11 of 14 cores) who was referred for initial staging with 99mTc-HYNIC-PSMA. The scan was reported as miT2m N2(REI, ROB) M1b(uni) M1c(skin?), PRIMARY score: 4, PSMA expression score: 1-3. We incidentally discovered increased uptake in the left skull region on whole-body images.

A 34-year-old woman with a history of renal transplantation and cervical cancer underwent lymphoscintigraphy using 99mTc-phytate before surgery. Initial planar images revealed multiple foci of uptake, which were later precisely localized using SPECT/CT imaging. Notably, uptakes in the right lower quadrant were attributed to the pyelocalyceal system of the transplanted kidney, highlighting a potential pitfall in patients with ectopic or transplanted kidneys.

We present a case of a 69-year-old man diagnosed with prostate adenocarcinoma, who underwent 99mTc-PSMA scintigraphy as part of his initial staging workup. The scan revealed a focal uptake in the posterior abdominal wall, corresponding to a subcutaneous soft tissue lesion. Further evaluation using SPECT/CT imaging localized the lesion, which was subsequently excised and histopathologically diagnosed as an angiolipoma.

We present a case of a 65-year-old woman with a history of melanoma of the left foot who underwent tumor resection with inguinal lymph nodes dissection and received external beam radiation therapy. After some time, she developed nodular lesions in the same region that were confirmed to be due to involvement of the dermis. She was selected as a candidate for isolated limb perfusion procedure.

Traumatic brain injury (TBI) is a complex condition involving mechanisms that lead to brain dysfunction and nerve damage, resulting in significant morbidity and mortality globally. Affecting ~50 million people annually, TBI's impact includes a high death rate, exceeding that of heart disease and cancer. Complications arising from TBI encompass concussion, cerebral hemorrhage, tumors, encephalitis, delayed apoptosis, and necrosis.

A lymphoscintigraphy is a crucial diagnostic tool for visualizing lymph nodes. This scan plays a significant role in determining the treatment and recovery plan for the patients. Due to the small lymph node size, obtaining high-quality images is important to prevent inaccurate results.

Sentinel lymph node (SLN) biopsy offers a minimally invasive approach to staging lymph node involvement in laryngeal squamous cell carcinoma (SCC). Despite its adoption in other cancers, its accuracy in laryngeal SCC remains under investigation. This systematic review and meta-analysis evaluates the diagnostic performance of SLN mapping in laryngeal cancer.

Background: Online adaptive radiotherapy (OART) and rapid quality assurance (QA) are essential for effective heavy ion therapy (HIT). However, there is a shortage of deep learning (DL) models and workflows for predicting Monte Carlo (MC) doses in such treatments.

Purpose: This study seeks to address this gap by developing a DL model for independent MC dose (MCDose) prediction, aiming to facilitate OART and rapid QA implementation for HIT.

We present the case of a 58-year-old man with metastatic medullary thyroid carcinoma refractory to conventional therapies, including peptide receptor radionuclide therapy. Despite multiple interventions, serum calcitonin and carcinoembryonic antigen levels continued to rise. Subsequent evaluation with 99m Tc-FAPI-46 revealed remarkable uptake in metastatic lesions, suggesting a potential role for FAPI-labeled radioisotopes in the management of medullary thyroid carcinoma.

Objectives: This study evaluated the necessity of a ventilation scan in patients suspected of PE with a history of COVID-19 infection.

Methods: This was a cross-sectional study of patients with PCR-confirmed COVID-19 and suspected PE at a tertiary care hospital in 2020. They underwent ventilation/perfusion (V/Q) scintigraphy using single-photon emission computed tomography/computed tomography (SPECT/CT) and CT scans with or without contrast.

Breast cancer lymphoscintigraphy is a crucial tool in pre-operative assessment, typically revealing sentinel lymph node drainage patterns within axillary and extra-axillary regions. However, rare cases challenge conventional understanding. We report a 67-year-old woman with breast cancer, where lymphoscintigraphy revealed focal uptake within the falciform ligament of the liver, an exceedingly rare phenomenon.

Acad Radiol

December 2024

Division of Nuclear Medicine and Molecular Imaging, Department of Medical Imaging, Geneva University Hospital, CH-1211 Geneva 4, Switzerland (H.A., H.Z.); Department of Nuclear Medicine and Molecular Imaging, University of Groningen, University Medical Center Groningen, 9700 RB Groningen, Netherlands (H.Z.); Department of Nuclear Medicine, University of Southern Denmark, Odense, Denmark (H.Z.); University Research and Innovation Center, Óbuda University, Budapest, Hungary (H.Z.). Electronic address:

Background: While magnetic resonance imaging (MRI) remains the gold standard for morphological imaging, its ability to differentiate between tumor tissue and treatment-induced changes on the cellular level is insufficient. Notably, glioma cells, particularly glioblastoma multiforme (GBM), demonstrate overexpression of chemokine receptor-4 (CXCR4). This study aims to evaluate the feasibility of non-invasive Ga-Cixafor™ PET/CT as a tool to improve diagnostic accuracy in patients with high-grade glioma.

Since the 1940s, 131-I radioiodine therapy (RIT) has been the primary treatment for metastatic differentiated thyroid cancer (DTC). Approximately half of these patients respond favorably to RIT, achieving partial or complete remission or maintaining long-term stable disease, while the other half develop radioiodine-refractory DTC (RAI-R DTC). The main genomic alteration involved in radioiodine resistance is the activated mitogen-activated protein kinase (MAPK) pathway, which results in the loss of sodium iodide symporters (NIS).

Equilibrium radionuclide angiography (ERNA) has long been assumed as the preferred method to assess cardiac volumes as well as left ventricular systolic and diastolic indices. ERNA was used to diagnose subtle changes in cardiac function during chemotherapy or early stages of heart failure. Gated myocardial perfusion SPECT (GMPS) was introduced as a more feasible and versatile alternative to ERNA, but the precision of GMPS to assess systolic and diastolic indices has not yet been fully reviewed.
