- The kidney failure associated with AS leads to severe bone disease because the kidneys fail to produce vitamin D, which is essential for calcium absorption, resulting in low serum calcium levels.
- Decreased phosphate excretion worsens the condition by raising plasma phosphate levels, which increases parathyroid hormone demand to mobilize calcium from bones, leading to secondary hyperparathyroidism and maintaining calcium-phosphorous balance.
