Preprocedural Computed Tomography Planning for Surgical Aortic Valve Replacement.

Ann Thorac Surg

June 2024

Valve Science Center, Minneapolis Heart Institute Foundation, Minneapolis, Minnesota; Minneapolis Heart Institute, Abbott Northwestern Hospital, Minneapolis, Minnesota. Electronic address:

Background: Selection of transcatheter valve size using preprocedural computed tomography (CT) is standardized and well established. However, valve sizing for surgical aortic valve replacement (SAVR) is currently performed intraoperatively by using sizers, which may result in variation among operators and risk for prosthesis-patient mismatch. This study evaluated the usefulness of CT annulus measurement for SAVR valve sizing.

Right ventricular failure (RVF) is a significant cause of mortality in patients undergoing left ventricular assist device (LVAD) implantation. Although right ventricular assist devices (RVADs) can treat RVF in the perioperative LVAD period, liberal employment before RVF is not well established. We therefore compared the survival outcomes between proactive RVAD placement at the time of LVAD implantation with a bailout strategy in patients with RVF.

Clozapine Withdrawal Catatonia: A Case Report.

Cureus

January 2024

Psychiatry and Behavioral Sciences, West Virginia University, Morgantown, USA.

Catatonia, which is associated with gamma-aminobutyric acid (GABA) hypoactivity, often responds robustly to benzodiazepines. It has been reported to be a consequence of abrupt discontinuation of clozapine, an antipsychotic used for treatment-resistant schizophrenia. Clozapine discontinuation, sometimes necessitated by medical concerns, can carry the risk of adverse outcomes, including catatonia.

Article Synopsis
  • Extended criteria donor (ECD) hearts from donation after brain death (DBD) are often underused for transplants due to cold storage issues; this study explored the impact of an extracorporeal perfusion system on their utilization and outcomes.
  • In a multicenter study with adult recipients, 173 ECD DBD hearts were perfused, achieving an 87% transplantation success rate, with 30-day survival at 97% and only 6.7% experiencing severe primary graft dysfunction (PGD).
  • The outcomes demonstrated that using this perfusion system not only reduced severe PGD but also supported good patient survival rates through two years, suggesting a way to significantly increase heart transplant availability for patients in need.

It is unclear whether extracorporeal CO removal (ECCOR) can reduce the rate of intubation or the total time on invasive mechanical ventilation (IMV) in adults experiencing an exacerbation of chronic obstructive pulmonary disease (COPD). To determine whether ECCOR increases the number of ventilator-free days within the first 5 days postrandomization (VFD-5) in exacerbation of COPD in patients who are either failing noninvasive ventilation (NIV) or who are failing to wean from IMV. This randomized clinical trial was conducted in 41 U.

As the availability of extracorporeal membrane oxygenation (ECMO) expands, so has the need for interfacility transfer to ECMO centers. However, the impact of these transfers is unknown. We hypothesized that interfacility transfers would be associated with increased complications and mortality.

Article Synopsis
  • The study investigates the differences in outcomes between two types of failed transcatheter heart valves (balloon-expandable valves vs. self-expanding valves) during surgical explantation.
  • Analyzing data from 401 patients, it reveals that failed balloon-expandable valves had higher rates of certain complications like endocarditis and paravalvular leaks compared to self-expanding valves.
  • Overall, mortality rates and stroke rates were similar across both groups post-explantation, but concomitant mitral surgery emerged as a significant independent predictor of 1-year mortality for both valve types.

The World Health Organization classification of pituitary tumors provides a framework for pathologists and researchers to classify pituitary adenomas. From the perspective of a practicing pathologist, this classification can be improved by pooling immunohistochemical data in a more standardized way, and by deliberately distinguishing features that assist in classification from those that do not. This article illustrates one general workflow to examine classification features consisting of immunohistochemical stains for anterior pituitary tumors, in order to promote debate and advance an evidence-based framework for classification.
