Introduction: Macrophages exhibit marked phenotypic heterogeneity within and across disease states, with lipid metabolic reprogramming contributing to macrophage activation and heterogeneity. Chronic inflammation has been observed in human benign prostatic hyperplasia (BPH) tissues, however macrophage activation states and their contributions to this hyperplastic disease have not been defined. We postulated that a shift in macrophage phenotypes with increasing prostate size could involve metabolic alterations resulting in prostatic epithelial or stromal hyperplasia.

The guidelines from the European and American Societies for Gastrointestinal Endoscopy discourage endoscopic retrieval of drug bags in body stuffers. However, recent evidence challenges this stance, demonstrating successful bag retrieval without fatal outcomes. We present two distinct cases illustrating varying outcomes of intervention.

Article Synopsis
  • The importance of germline genetic testing (GGT) for prostate cancer patients has grown, as it helps tailor treatment plans and manage the disease more effectively.
  • GGT is now considered a standard practice not just for prostate cancer, but also for other cancers like breast and ovarian, emphasizing the need for all patients to have equitable access to this testing.
  • Offering comprehensive GGT allows for better decision-making between patients and doctors, aids in detecting potential future cancers, and facilitates testing for family members who may be at risk.

Introduction: ONWARDS 5 evaluated the effectiveness and safety of insulin icodec (icodec) titrated with a dosing guide app (icodec with app) versus once-daily insulin analogs in insulin-naive adults with type 2 diabetes. The insulin glargine U300 (glargine U300) stratum was too small to enable a robust post hoc efficacy comparison. Augmentation methodology was applied to increase the glargine U300 group size using real-world data (RWD), to facilitate efficacy comparisons of icodec with app versus glargine U300, and to demonstrate the potential of the augmentation methodology to strengthen underpowered treatment comparisons (AUGMENT study).

Elective outpatient middle meningeal artery embolization for chronic subdural hematoma is safe.

J Neurointerv Surg

January 2025

Neurological Surgery, Radiology, Mechanical Engineering, Neurology, Stroke & Applied Neuroscience Center, University of Washington, Seattle, Washington, USA

Background: Middle meningeal artery embolization (MMAE) is a safe and effective treatment for chronic subdural hematoma (cSDH); however, the appropriate level of postoperative care is unknown.

Objective: To evaluate whether elective MMAE for cSDH could be safely performed in an outpatient setting.

Methods: This was a multicenter, retrospective study of patients with cSDH who underwent elective MMAE.

Traumatic craniomaxillofacial (CMF) injuries are associated with various symptoms/concerns that affect patients' quality of life. The assessment of outcomes from the patient perspective has been limited by the absence of patient-reported outcome (PRO) measures tailored to this patient population. To address this need, we employed a mixed methods, multi-step process to first identify the most important symptoms/concerns and then use this information to construct a PRO symptom battery.

Short-Term Complications of Concomitant Pelvic Organ Prolapse and Rectal Prolapse Repair: A Systematic Review and Meta-Analysis.

Int Urogynecol J

December 2024

Division of Urogynecology, University of Chicago Pritzker School of Medicine, NorthShore University HealthSystem, 9650 Gross Point Road, Suite 3900, Skokie, IL, 60076, USA.

Introduction And Hypothesis: The objective was to evaluate and compare the short-term postoperative complications of concomitant pelvic organ prolapse (POP) and rectal prolapse repair with isolated apical prolapse repair or rectopexy.

Methods: This systematic review and meta-analysis were conducted in accordance with PRISMA guidelines. A comprehensive literature search was performed using Web of Science, PubMed, Embase, and Scopus for studies published up to April 2024.

Article Synopsis
  • The study investigates the impact of tumor characteristics on the feasibility of endoscopic management (EM) for upper tract urothelial carcinoma (UTUC) by creating a standardized assessment score known as the Upper TRACT Endometry Score.
  • Utilizing expert opinions through a modified Delphi method, the score incorporates factors like tumor radius, architecture, count, and location, validating it with a retrospective database of patients undergoing EM for UTUC.
  • Results indicate a significant correlation between the Upper TRACT Endometry Score and the intensity of procedures required for patients, suggesting its potential for improving patient counseling and standardizing the assessment in future cases.

Artificial intelligence models have been increasingly used in the analysis of tumor histology to perform tasks ranging from routine classification to identification of molecular features. These approaches distill cancer histologic images into high-level features, which are used in predictions, but understanding the biologic meaning of such features remains challenging. We present and validate a custom generative adversarial network-HistoXGAN-capable of reconstructing representative histology using feature vectors produced by common feature extractors.

Polygenic risk for alcohol use disorder affects cellular responses to ethanol exposure in a human microglial cell model.

Sci Adv

November 2024

Department of Neuroscience and Cell Biology and The Child Health Institute of New Jersey, Rutgers Robert Wood Johnson Medical School, New Brunswick, NJ 08901, USA.

Polygenic risk scores (PRSs) assess genetic susceptibility to alcohol use disorder (AUD), yet their molecular implications remain underexplored. Neuroimmune interactions, particularly in microglia, are recognized as notable contributors to AUD pathophysiology. We investigated the interplay between AUD PRS and ethanol in human microglia derived from iPSCs from individuals with AUD high-PRS (diagnosed with AUD) or low-PRS (unaffected).

As more multi-ancestry GWAS summary data become available, we have developed a comprehensive trans-ancestry pathway analysis framework that effectively utilizes this diverse genetic information. Within this framework, we evaluated various strategies for integrating genetic data at different levels-SNP, gene, and pathway-from multiple ancestry groups. Through extensive simulation studies, we have identified robust strategies that demonstrate superior performance across diverse scenarios.
