Background: Ischaemic intestines could be a driver of critical illness through an inflammatory response. We have previously published reports on a biomarker for intestinal injury, plasma Intestinal Fatty Acid Binding Protein (IFABP), and inflammatory biomarkers after out-of-hospital cardiac arrest (OHCA). In this post-hoc study we explored the potential indirect effects of intestinal injury mediated through the inflammatory response on organ dysfunction and mortality.

Despite advances in the treatment and care of severe physical injuries, trauma remains one of the main reasons for disability-adjusted life years worldwide. Trauma patients often suffer from disturbances in energy utilization and metabolic dysfunction, including hyperglycemia and increased insulin resistance. White adipose tissue plays an essential role in the regulation of energy homeostasis and is frequently implicated in traumatic injury due to its ubiquitous body distribution but remains poorly studied.

Article Synopsis
  • - GPs in Norway earn two-thirds of their income through a fee-for-service model, which encourages brief consultations and high service provision but may complicate decision-making as they act as gatekeepers for various treatments.
  • - A study with 33 GPs revealed that while they understand the profitability of different fees and adopt strategies to increase income, the pressure of time constraints can lead them to accommodate patient requests, even if they are unreasonable.
  • - Participants recognized the dual nature of fees as both incentives and compensation, noting that complexities in fee structures can create challenges in accurately interpreting and applying them in practice.

Article Synopsis
  • - The study examines inconsistencies in how healthcare providers and patients identify and count lesions associated with hidradenitis suppurativa (HS), highlighting potential issues in the existing clinical response measurement tool, HiSCR, and the impact of rater training on trial outcomes.
  • - An online survey of 47 members from the HISTORIC collaboration revealed significant variability in lesion classification, with many providers differing on whether lesions should be treated as single or multiple entities based on their morphology.
  • - Findings suggest a need for standardized, consensus-driven guidelines for rater training in HS clinical trials to improve consistency in lesion characterization and potentially enhance trial results.

Background: Shaft fractures of the femur are commonly treated with intramedullary nailing, which can release bone marrow emboli into the bloodstream. Emboli can travel to the lungs, impairing gas exchange and causing inflammation. Occasionally, emboli traverse from the pulmonary to the systemic circulation, hindering perfusion and resulting in injuries such as heart and brain infarctions, known as .

Acute intermittent porphyria (AIP) is an inherited metabolic disorder associated with complications including kidney failure and hepatocellular carcinoma, probably caused by elevations in the porphyrin precursors porphobilinogen (PBG) and delta-aminolevulinic acid (ALA). This study explored differences in modern biomarkers for renal and hepatic damage between AIP patients and controls. Urine PBG testing, kidney injury panels, and liver injury panels, including both routine and modern biomarkers, were performed on plasma and urine samples from AIP cases and matched controls (50 and 48 matched pairs, respectively).
