Current opinion and comparison of surgical procedures for the treatment of primary spontaneous pneumothorax.

Expert Rev Respir Med

February 2022

The Study Group for Pneumothorax and Cystic Lung Diseases, Tokyo, Japan.

Article Synopsis
  • Three-port video-assisted thoracoscopic surgery (VATS) is the standard treatment for primary spontaneous pneumothorax (PSP), but it has a relatively high postoperative recurrence rate.
  • Bullectomy helps reduce recurrences, but additional techniques like pleural covering and chemical pleurodesis are often needed.
  • Newer minimally invasive methods, such as uniportal VATS and needlescopic surgery, offer benefits like less pain and better cosmetic outcomes, and are expected to gain popularity for treating PSP.

Telomere lengths in Barrett's esophagus as a precancerous lesion.

Esophagus

April 2022

Research Team for Geriatric Pathology, Tokyo Metropolitan Institute of Gerontology, 35-2 Sakae-cho, Itabashi-ku, Tokyo, 173-0015, Japan.

Background: We have reported that precancerous conditions and lesions invariably have shorter telomeres and associated chromosomal instability relative to normal tissue.

Methods: Using the Q-FISH method and our original software, Tissue Telo, we estimated telomere lengths in cardiac- and intestinal-type mucosae in 48 cases of Barrett's esophagus (short-segment (SS) n = 18; long-segment (LS) n = 30).

Results: There were no significant differences in telomere length between the cardiac and intestinal types in any of the 48 cases, suggesting that the presence or absence of goblet cells in the columnar segments is unrelated to telomere-dependent chromosomal instability in Barrett's esophagus.

Background: Pleurography (PG) has been described previously but has not gained popularity. PG can determine the exact air leak points in the lung, which is important for treating pneumothorax and pleural fistulas. We believe that the usefulness of PG should be reassessed, and here we describe the method, air leak detection rate, and common complications.

Lymphangioleiomyomatosis (LAM) is a rare pulmonary disease characterised by the proliferation of smooth muscle-like cells (LAM cells), and an abundance of lymphatic vessels in LAM lesions. Studies reported that vascular endothelial growth factor-D (VEGF-D) secreted by LAM cells contributes to LAM-associated lymphangiogenesis, however, the precise mechanisms of lymphangiogenesis and characteristics of lymphatic endothelial cells (LECs) in LAM lesions have not yet been elucidated. In this study, human primary-cultured LECs were obtained both from LAM-affected lung tissues (LAM-LECs) and normal lung tissues (control LECs) using fluorescence-activated cell sorting (FACS).

BMC Pulm Med

March 2021

Department of Respirology, Graduate School of Medicine, Chiba University, 1-8-1 Inohana, Chuou-ku, Chiba, Chiba, 260-8670, Japan.

Article Synopsis
  • Portopulmonary hypertension (PoPH) is characterized by the simultaneous presence of pulmonary arterial and portal hypertension, but there is limited data on the treatment and characteristics of Asian patients with PoPH, prompting this study involving a Japanese cohort.
  • The investigation included an analysis of data from a national research project and a retrospective study at Chiba University Hospital, comparing patients with PoPH to those with idiopathic/heritable pulmonary arterial hypertension (I/H-PAH).
  • Findings indicated that patients with PoPH had higher cardiac outputs and better exercise tolerance, but experienced adverse effects leading to a high rate of medication changes, highlighting the need for further research to enhance treatment strategies for PoPH.

Background: Total pleural covering (TPC) is an innovative surgical procedure in which the entire visceral pleura is wrapped with sheets of oxidized regenerated cellulose (ORC) mesh under video-assisted thoracoscopic surgery. We have previously reported that TPC could successfully prevent pneumothorax recurrence in patients with lymphangioleiomyomatosis (LAM). However, the actual efficacy and preventive effect of TPC on pneumothorax recurrence remains unclear as many LAM patients already had pleural adhesion prior to TPC that was induced by thoracic surgery and/or pleurodesis.

Diabetic polyneuropathy: Bridging the translational gap.

J Peripher Nerv Syst

June 2020

Division of Neurology and Neuroscience and Mental Health Institute, University of Alberta, Edmonton, Alberta, Canada.

Clinical trials for diabetic polyneuropathy (DPN) have failed to identify therapeutic impacts that have arrested or reversed the disorder, despite a long history. This review considers DPN in the context of a unique neurodegenerative disorder that targets peripheral neurons and their companion glial cells. The approach is to examine what cells, cell substructures, and pathways are implicated in causing DPN and how they might be addressed therapeutically.

When the presence of endometriotic lesions are not evident by hematoxylin and eosin staining, CD10 is used to highlight and confirm the presence of endometriotic stroma. However, CD10 is not specific only to the endometrial stroma but is also expressed in many other cells. Recently, interferon-inducible transmembrane protein 1 (IFITM1) was reported as a highly specific immunohistochemical marker of normal endometrial stroma and endometrial stromal neoplasm.

PAX8: A Highly Sensitive Marker for the Glands in Extragenital Endometriosis.

Reprod Sci

August 2020

Department of Obstetrics and Gynecology, Faculty of Medicine, University of Tokyo, 7-3-1 Hongo, Bunkyo-ku, Tokyo, 113-8655, Japan.

In cases of extragenital endometriosis or microscopic endometriosis lesions, pathological diagnosis can be challenging because endometriotic stroma and glands represent only a minor component of fibrotic endometriotic lesions. For better accuracy of diagnosis, the development of a sensitive and specific epithelial marker is beneficial. Previous studies showed that PAX8 is a highly sensitive and specific marker for primary and metastatic Mullerian epithelial tumors.
