The year 2023 marked a significant change for Internal Medicine, as the number of submissions related to the novel coronavirus infection (COVID-19) declined significantly and interest shifted to other disease fields and research areas. Our journal published its first articles on artificial intelligence (AI) and machine learning (ML), and these articles have shown that AI may be useful for the early detection of potential cardiac diseases, while ML can be used to predict the risk of serious illness in patients hospitalized with COVID-19, providing new possibilities for diagnoses and treatment. In addition to touching on the above, the present article also highlights the status of submissions to the journal (including the number of submissions and acceptance rate) in 2023.

Introduction: We describe a case of an adrenal cavernous hemangioma that was surgically resected because of tumor growth and intratumoral hemorrhage.

Case Presentation: A 73-year-old woman presented with an enlarged adrenal tumor and intratumoral hemorrhage during the follow-up of an incidental adrenal tumor. A computed tomography showed that the left adrenal tumor had grown from 23 to 44 mm over 1 year.

α-Actinin-4 (ACTN4) expression levels are correlated with the invasive and metastatic potential of cancer cells; however, the underlying mechanism remains unclear. Here, we identified ACTN4-localized ruffle-edge lamellipodia using live-cell imaging and correlative light and electron microscopy (CLEM). BSC-1 cells expressing EGFP-ACTN4 showed that ACTN4 was most abundant in the leading edges of lamellipodia, although it was also present in stress fibers and focal adhesions.

Extrahepatic portal vein obstruction (EHPVO) is a rare disease with myeloproliferative neoplasm (MPN) as the most common cause. We report that hypersplenic hematologic changes in EHPVO might be eliminated by MPN. Through experience with splenectomy for variceal control with EHPVO, we suspected that spleen might mask MPN-induced thrombocytosis, and that MPN might have a significant influence on excessive thrombocytosis after splenectomy.
