Microglia, a type of immune cells of the central nervous system, play a critical role in the pathophysiology of neurodegenerative disorders including Alzheimer's disease (AD). Recently, efforts for drug discovery have focused on modifying the function of microglia to halt AD progression. One such effort targets a multifaceted kinase called receptor-interacting protein kinase 1 (RIPK1) that controls inflammation and cell death.

  • Neutrophil gelatinase-associated lipocalin (NGAL) is a biomarker that may indicate early acute kidney injury (AKI), but its role in gastrointestinal diseases needed further exploration.
  • In a study of 171 patients with various gastrointestinal conditions, urinary NGAL levels were significantly elevated in those with acute pancreatitis and cholangitis, and also increased in inflammatory bowel diseases and liver diseases.
  • The findings suggest that urinary NGAL may be a better indicator of infection severity rather than just AKI, and higher levels of NGAL are associated with worse hospital outcomes and patient prognosis.

A rare cause of esophageal stenosis: Compression due to a thoracic osteophyte.

DEN Open

April 2024

Division of Gastroenterology and Hepatology Graduate School of Medical and Dental Sciences, Niigata University Niigata Japan.

Several cases of esophageal stenosis caused by cervical vertebral osteophytes have been reported; however, few reports of esophageal stenosis caused by thoracic osteophytes are available. We describe the case of an 86-year-old man with esophageal stenosis caused by a thoracic osteophyte near the tracheal bifurcation. An endoscopic ultrasonography examination was scheduled to determine the cause of acute pancreatitis; however, lacerations observed at the bifurcation following endoscope removal during prior esophagogastroduodenoscopy led us to cancel the ultrasonography to avoid potential esophageal perforation.

Article Synopsis
  • The study examines coronary arterial aneurysms (CAAs) in patients with Kawasaki disease (KD), highlighting their impact on patient prognosis and the factors influencing CAA regression.
  • Analyzing data from over 1,000 KD patients across 44 institutions in Japan, the research shows that smaller CAAs are more likely to regress over a 10-year period compared to larger ones.
  • Key findings indicate that early diagnosis (under age 1), more recent onset of KD (2010-2012), and female sex are linked to higher regression rates for CAAs, which can inform better long-term management strategies for these patients.

Objective: The use of folic acid (FA) has been discouraged in cerebral folate deficiency (CFD) because, theoretically, it could inhibit the transport of 5-methyltetrahydrofolic acid (5MTHF) across the blood-cerebrospinal fluid (CSF) barrier. We present the clinical biochemical data of two cases with CFD to support this hypothesis.

Methods: We measured CSF and serum 5MTHF concentrations in a patient with Kearns-Sayre syndrome (KSS) and a patient homozygous for C677T polymorphism before and during folate supplementation therapy.
