J Infect Chemother

September 2019

The Antimicrobial Stewardship Committee of Japanese Society of Chemotherapy, Japan; Department of Medical Technology, Kumamoto Health Science University, Kumamoto, Japan.

Article Synopsis
  • Antimicrobial stewardship programs (ASPs) are essential for ensuring proper use of antimicrobials in healthcare, and these programs rely on multidisciplinary teams known as antimicrobial stewardship teams (ASTs).
  • Despite recent changes in Japan's medical reimbursement to support ASPs, challenges like understaffing and weak enforcement of AST recommendations still exist.
  • This committee report offers guidance on enhancing ASTs based on data from a survey of 1,358 healthcare facilities, aiming to improve structural and financial support for these teams.

Article Synopsis
  • - Enhanced surveillance from April 2013 to March 2018 in Japan identified 1277 cases of invasive pneumococcal disease (IPD) in adults, revealing a rise in infections caused by the serotype 12F.
  • - Compared to non-12F IPD patients, those with 12F IPD were generally younger and had fewer comorbidities, with significant differences in vaccination rates (4% vs. 14%) and immunocompromised conditions (19% vs. 30%).
  • - Despite the clinical differences, the mortality rates were similar between the two groups, but 12F IPD victims were younger and had a higher incidence of bacteremia without focus, indicating the ser

Objective: This study aimed to investigate whether a novel method including prophylactic pancreatic stent (PS) placement prevents postendoscopic papillary balloon dilation pancreatitis.

Patients And Methods: This study enrolled 200 consecutive patients with bile duct stones measuring up to 8 mm in diameter and retrospectively recruited 113 patients undergoing ordinary endoscopic papillary balloon dilation (EPBD) without PS placement from our previous study. In the novel method, EPBD and PS placement was attempted with a guidewire left in the main pancreatic duct for patients in whom stable guidewire placement in the main pancreatic duct was possible.

In paroxysmal nocturnal hemoglobinuria (PNH), various symptoms due to intravascular hemolysis exert a negative impact on patients' quality of life (QOL). To determine clinical factors related with improvements in QOL in PNH patients treated, we analyzed changes in QOL scales in PNH patients treated with eculizumab based on data collected from post-marketing surveillance in Japan. Summary statistics were obtained using figures from QOL scoring systems and laboratory values, and evaluated by t test.

Infiltration by IgG-positive plasma cells is a common finding in tubulointerstitial nephritis. Indeed, it has been thought that CD138-positive mature plasma cells secrete mainly IgG, and the occurrence of tubulointerstitial nephritis with CD138-positive plasma cells secreting IgM has rarely been reported. Routine immunofluorescence of fresh frozen sections is considered the gold standard for detection of immune deposits.
