Nighttime hypoglycemia in Japanese children with type 1 diabetes mellitus treated with multiple daily injection insulin therapy.

Endocr J

July 2023

The Japanese Study Group of Insulin Therapy for Childhood and Adolescent Diabetes, Kyoto 604-0835, Japan.

Prevention of hypoglycemia is an important strategy for glycemic management in patients with type 1 diabetes mellitus (T1D). Hypoglycemia is difficult to recognize at night while sleeping, particularly when using multiple daily injection (MDI) insulin therapy rather than sensor-augmented insulin-pump therapy. Therefore, it is possible that patients with T1D are at higher risk of nocturnal hypoglycemia when insulin is administered using an MDI regimen.

Background: Although guideline-directed medical therapy (GDMT), including β-blockers, angiotensin-converting enzyme inhibitors (ACEi)/angiotensin receptor blockers (ARBs), and mineralocorticoid receptor antagonists (MRAs), improves survival and quality of life, most patients with heart failure with reduced (HFrEF) and mildly reduced (HFmrEF) ejection fraction are treated with inadequate medications. We investigated the prescription patterns of GDMT in elderly patients with HFrEF and HFmrEF and their characteristics, including the certification of long-term care insurance (LTCI), which represents frailty and disability.

Methods and results: This retrospective cross-sectional study analyzed 1,296 elderly patients with symptomatic HFrEF and HFmrEF with diuretic use (median age 78 years; 63.

Left-sided portal hypertension (LSPH) is a condition of extrahepatic portal hypertension that often results in bleeding from isolated gastric varices (GVs). LSPH is sometimes caused by myeloproliferative diseases, such as essential thrombocythemia (ET). We herein report two cases of GVs with LSPH due to ET that were successfully controlled by gastric devascularization (GDS) or partial splenic embolization (PSE).

Article Synopsis
  • The study examines coronary arterial aneurysms (CAAs) in patients with Kawasaki disease (KD), highlighting their impact on patient prognosis and the factors influencing CAA regression.
  • Analyzing data from over 1,000 KD patients across 44 institutions in Japan, the research shows that smaller CAAs are more likely to regress over a 10-year period compared to larger ones.
  • Key findings indicate that early diagnosis (under age 1), more recent onset of KD (2010-2012), and female sex are linked to higher regression rates for CAAs, which can inform better long-term management strategies for these patients.

Objective: The global standard for chemoradiation therapy (CCRT) for head and neck squamous cell carcinoma is cisplatin 100 mg/m administered once every three weeks, although cisplatin 80 mg/m is also widely used as an alternative treatment to reduce adverse events in Japan. We aimed to assess the long-term survival outcomes and late adverse events associated with CCRT with a 3-weekly cisplatin dose of 80 mg/m.

Methods: A phase 2 study on CCRT with a 3-weekly cisplatin dose of 80 mg/m was performed in 47 patients between April 2015 and December 2016 at four centers in Japan.
