Novel Treatment Options in Patients with Maturity-Onset Diabetes of the Young.

Exp Clin Endocrinol Diabetes

January 2025

Department of Internal Medicine, Gastroenterology and Diabetology, Franziskus Hospital Harderberg, Niels Stensen Hospitals, Georgsmarienhütte, Germany.

Article Synopsis
  • MODY (Maturity onset diabetes of the young) is a common genetic form of diabetes caused by mutations affecting pancreatic beta cells, leading to issues with insulin secretion and has 14 different types.
  • Some types of MODY, like GCK-MODY, usually don’t need medication, whereas others, such as HNF1A-MODY and HNF4A, often require sulfonylureas but may progress to needing insulin therapy over time.
  • Newer treatment options like SGLT2 inhibitors, DPP-4 inhibitors, and GLP-1 receptor agonists are highlighted for their lower risk of hypoglycemia and better effects on body weight compared to traditional therapies.

Diabetic Ketoacidosis in Patients with Maturity-Onset Diabetes of the Young.

Exp Clin Endocrinol Diabetes

August 2024

Department of Internal Medicine, Gastroenterology and Diabetology, Niels Stensen Hospitals, Franziskus Hospital Harderberg, Georgsmarienhütte, Germany.

Article Synopsis
  • * There are 14 known types of MODY, which typically follows an autosomal dominant inheritance pattern, often seen across multiple generations in families.
  • * Recent studies challenge the idea that diabetic ketoacidosis (DKA) excludes a diagnosis of MODY, highlighting new cases where MODY patients have presented with DKA, forming the basis of this review.

Homeostasis Model Assessment of β-Cell Function for Diagnosis of Insulinoma.

J Clin Endocrinol Metab

February 2024

Department of Endocrinology and Diabetology, Medical Faculty and University Hospital Düsseldorf, Heinrich Heine University Düsseldorf, 40225 Düsseldorf, Germany.

Article Synopsis
  • Diagnosis of insulinoma can be evaluated effectively using the homeostasis model assessment of insulin secretion (HOMA-B) based on data from a 72-hour fasting test.
  • In a study of 104 patients, HOMA-B values were significantly higher in those with insulinoma compared to those without, indicating its potential as a reliable diagnostic marker.
  • Results showed HOMA-B had an impressive sensitivity and specificity for diagnosing insulinoma, suggesting it may serve as a comparable or even superior alternative to existing guidelines from various endocrine societies.

For artificial intelligence (AI) based systems to become clinically relevant, they must perform well. Machine Learning (ML) based AI systems require a large amount of labelled training data to achieve this level. In cases of a shortage of such large amounts, Generative Adversarial Networks (GAN) are a standard tool for synthesising artificial training images that can be used to augment the data set.

Institute for Clinical Diabetology, German Diabetes Center (DDZ), Leibniz Center for Diabetes Research at Heinrich Heine University Düsseldorf, Düsseldorf, Germany; German Center for Diabetes Research (DZD), Partner Düsseldorf, München-Neuherberg, Germany; Department of Endocrinology and Diabetology, Medical Faculty and University Hospital, Heinrich Heine University Düsseldorf, Düsseldorf, Germany.

Background And Aims: The dietary glycemic index (GI) and glycemic load (GL) are increasingly recognized as important for the prevention and management of diabetes mellitus. To extend the portfolio of assessment methods for large-scale epidemiological studies, we propose a GI-specific addition to an already established FFQ.

Methods And Results: The German version of the EPIC-FFQ was extended by GI-specific questions for major carbohydrate sources varying notably in GI (breakfast cereals, bread, pasta, rice, potato etc.

Adjustment of Anti-Hyperglycaemic Agents During Bowel Preparation for Colonoscopy in Patients with Diabetes.

Exp Clin Endocrinol Diabetes

September 2022

Second Department of Internal Medicine (Gastroenterology, Hepatology, Diabetology), Klinikum Leverkusen, Leverkusen, Germany.

Objective: Due to the growing diabetes pandemic, the number of colonoscopies performed in patients with diabetes is steadily rising. However, recommendations on adjustments of anti-hyperglycaemic agents (AHG) during bowel preparation for colonoscopy are limited.

Methods: A total of nine articles were revealed on a PubMed search using the search terms "diabetes" and "colonoscopy", "sigmoidoscopy", "endoscopy", "endoscopic intervention", "endoscopic invasive diagnostics", "endoscopic surgery", or "diabetes care in the hospital" and manual screening of the references of the articles reporting on AHG adjustment during bowel preparation.

Background And Aims: It is unclear whether socio-economic status (SES) is associated with glycaemic control in people with recently diagnosed diabetes. The aim was to investigate whether SES is related to haemoglobin A1c (HbA1c) during the first year after diagnosis in people with type 1 and type 2 diabetes and if metabolic, quality of care or mental factors may explain the association.

Methods: In the German Diabetes Study, people with type 1 (n = 274, median age 36 [25th; 75th percentile: 28; 48] years) and type 2 diabetes (n = 424, 54 [47; 60] years) underwent detailed metabolic characterisation within the first year after diagnosis.
