Article Synopsis
  • Personalized medicine in acute myeloid leukemia (AML) faces challenges, particularly with the new prognostic scoring algorithm, Knowledge Bank (KB), developed from genetic mutation findings in trials.
  • A study involving 167 AML patients examined the accuracy of the KB algorithm in predicting overall survival (OS) and found discrepancies, especially in younger patients with adverse risk categories and older patients with favorable risk categories.
  • The study suggests that the emergence of new therapies and improved outcomes from treatments like allogeneic stem cell transplantation highlight the need for continuous evaluation and adaptation of predictive models in AML.

Do Glucocorticoid Injections Increase the Risk of Knee Osteoarthritis Progression Over 5 Years?

Arthritis Rheumatol

August 2022

Université de Paris, INSERM, UMR-S 1132 BIOSCAR, and Rheumatology Department, AP-HP, Lariboisière Hospital, Paris, France.

Objective: Recent findings have demonstrated that intraarticular (IA) glucocorticoid injections can be deleterious for knees with osteoarthritis (OA). This study was undertaken to assess, in a real-life setting, the risk of knee OA progression in patients who received IA glucocorticoid injections over a 5-year follow-up period.

Methods: We used marginal structural modeling with inverse probability of treatment weighting to determine the causal association between IA glucocorticoid injections and the 5-year risk of disease progression in patients with symptomatic knee OA from the Knee and Hip Osteoarthritis Long-term Assessment cohort.

Because of the function and anatomical environment of the rectum, therapeutic strategies for local advanced rectal cancer (LARC) must deal with two challenging stressors that are a high-risk of local and distal recurrences and a high-risk of poor quality of life (QoL). Over the last three decades, advances in screening tests, therapies, and combined-modality treatment options and strategies have improved the prognosis of patients with LARC. However, owing to the heterogeneous nature of LARC and genetic status, the patient may not respond to a specific therapy and may be at increased risk of side-effects without the life-prolonging benefit.

Article Synopsis
  • Core-binding factor acute myeloid leukemia (CBF AML) is typically seen as having a favorable prognosis, but new findings suggest that additional genetic abnormalities can adversely affect this outlook.
  • A study involving 24 patients revealed that while many achieve complete remission, relapse rates are significant, and outcomes differ greatly between those who undergo transplantation and those who do not.
  • The study highlights that new treatments and monitoring methods may improve CBF AML prognosis, challenging the previous notion of consistently favorable outcomes associated with this type of leukemia.

Besides cytoplasmic lipase-dependent adipocyte fat mobilization, the metabolic role of lysosomal acid lipase (LAL), highly expressed in adipocytes, is unclear. We show that the isolated adipocyte fraction, but not the total undigested adipose tissue (ATs), from obese patients has decreased LAL expression compared with that from nonobese people. Lentiviral-mediated LAL knockdown in the 3T3L1 mouse cell line to mimic the obese adipocytes condition did not affect lysosome density or autophagic flux, but it did increase triglyceride storage and disrupt endoplasmic reticulum cholesterol, as indicated by activated SREBP.

Loss of intestinal sympathetic innervation elicits an innate immune driven colitis.

Mol Med

January 2019

Amsterdam UMC, Tytgat Institute for Liver and Intestinal Research, University of Amsterdam, Meibergdreef 69, 1105 BK, Amsterdam, The Netherlands.

Article Synopsis
  • The sympathetic nervous system is important for controlling innate immune responses, and its reduction in inflammatory bowel disease affects immune function in the intestines.
  • Researchers examined how activation of adrenergic receptors impacts cytokine production in macrophages and studied the effects of sympathetic nerve damage in mice lacking certain immune cells.
  • Results showed that nerve damage led to increased inflammation and histological damage in the intestines, indicating that the sympathetic nervous system helps regulate immune cell activity in response to microbial threats.
