The IMPACT (Informing the Pathway of Chronic Obstructive Pulmonary Disease Treatment) trial demonstrated a significant reduction in all-cause mortality (ACM) risk with fluticasone furoate/umeclidinium/vilanterol (FF/UMEC/VI) versus UMEC/VI in patients with chronic obstructive pulmonary disease (COPD) at risk of future exacerbations. A proportion of cfDNA is derived from bacteria and viruses, creating opportunities for the diagnosis of infection via metagenomic sequencing. Recurrent cholangiocarcinoma carries a dismal prognosis, with surgery providing the only potential cure. In those with unresectable disease, chemotherapy is the typical therapeutic mainstay but, given the myelosuppressive effects of this regimen, a pre-existing cytopenia may necessitate dose delay or reduction and contribute to poorer patient outcomes. We present a case of unresectable recurrent cholangiocarcinoma in which chronic gastrointestinal bleeding precluded chemotherapy administration, and which was successfully managed by portal vein stenting and ectopic variceal embolization.

Allogeneic hematopoietic cell transplantation (allo-HCT) is a curative therapy for relapsed/refractory and high-risk non-Hodgkin lymphoma (NHL).
