The presenting features of retinoblastoma in developing countries and their correlation with disease stage and patient survival are poorly known and they may be useful as background information for planning early diagnosis initiatives. Therefore, we undertook a retrospective review of 508 patients (467 evaluable, 296 unilateral) treated in Argentina from 1988 to 2008. Patients presented at an older age than reported from high-income countries [mean age 24 mo (range, 0 to 165 mo), 31 mo for unilateral (range, 0 to 165 mo), and 13.

Pyelonephritis is rarely considered in the differential diagnosis of acute kidney injury. Acute non-obstructed bacterial pyelonephritis is an infrequent and rarely considered cause of rapidly progressive acute kidney injury. A diagnostic challenge thus develops as it is difficult to clinically differentiate acute kidney injury secondary to ischemic or toxic acute tubular necrosis or papillary necrosis versus acute interstitial nephritis secondary to drugs or infectious pyelonephritis.

Transbronchial needle aspiration of central and peripheral nodules.

Monaldi Arch Chest Dis

October 2001

New York Hospital of Queens, Pulmonary Section, Flusing, NY, USA.

Wang and Terry first described the technique of sampling of mediastinal lymph nodes using a flexible bronchoscope (FB) in 1983. Since then, the scope of transbronchial needle aspiration (TBNA) has increased enormously, and its value for diagnosis of central and peripheral lung lesions, even in the absence of endobronchial disease, is now recognized. Improvements in the diagnostic yield of TBNA aspirates, and increasing knowledge of predictors of a positive aspirate, has reduced the need for mediastinoscopy, and occasionally thoracotomy, with benefits in terms of reduced healthcare costs and improved patient welfare.
