species are responsible for most cases of post-operative endophthalmitis. Topical ocular drug was applied for post-operative infection prevention, but the way of delivery encounters many challenges in terms of patient's compliance, drug efficacy, and drug penetration. We used the levofloxacin-loaded chitosan/gelatin/β-glycerophosphate hydrogel sustained releasing system with good anti-bacterial efficacy and biocompatibility, which we had previously designed, for keratitis model to test the preclinical drug efficacy and to determine drug level in the anterior chamber of the eye.

Article Synopsis
  • Systemic inflammation is important in cancer, especially in a type called hepatocellular carcinoma (HCC), and this study looked at 362 patients treated with immunotherapy.* -
  • The study found that patients with higher levels of inflammation (measured by NLR and PLR) had shorter survival times and less successful treatment responses.* -
  • Results suggest that inflammation levels can predict how well HCC patients will do with immunotherapy, but more research is needed to understand why this happens.*

Background: The objective of this study was to evaluate the efficacy and safety of induction chemotherapy (ICT), GOFL (gemcitabine, oxaliplatin plus fluorouracil (5-FU)/leucovorin) versus modified FOLFIRINOX (irinotecan, oxaliplatin plus 5-FU/leucovorin), followed by concurrent chemoradiotherapy (CCRT) in locally advanced pancreatic adenocarcinoma (LAPC).

Methods: Chemo-naive patients with measurable LAPC were eligible and randomly assigned to receive biweekly ICT with either mFOLFIRINOX or GOFL for 3 months. Patients without systemic progression would have 5-FU- or gemcitabine-based CCRT (5040 cGy/28 fractions) and were then subjected to surgery or continuation of chemotherapy until treatment failure.

Reversal of pancreatic desmoplasia by a tumour stroma-targeted nitric oxide nanogel overcomes TRAIL resistance in pancreatic tumours.

Gut

September 2022

Institute of Biomedical Engineering and Frontier Research Center on Fundamental and Applied Sciences of Matters, National Tsing Hua University, Hsinchu, Taiwan

Objective: Stromal barriers, such as the abundant desmoplastic stroma that is characteristic of pancreatic ductal adenocarcinoma (PDAC), can block the delivery and decrease the tumour-penetrating ability of therapeutics such as tumour necrosis factor-related apoptosis-inducing ligand (TRAIL), which can selectively induce cancer cell apoptosis. This study aimed to develop a TRAIL-based nanotherapy that not only eliminated the extracellular matrix barrier to increase TRAIL delivery into tumours but also blocked antiapoptotic mechanisms to overcome TRAIL resistance in PDAC.

Design: Nitric oxide (NO) plays a role in preventing tissue desmoplasia and could thus be delivered to disrupt the stromal barrier and improve TRAIL delivery in PDAC.

Article Synopsis
  • A study compared the survival outcomes of patients with hepatitis C virus (HCV)-related liver cancer who achieved sustained virologic response (SVR) through either direct-acting antivirals (DAA) or interferon (IFN)-based therapy after surgical resection.
  • Results showed that both treatments had a high SVR rate and led to similar reductions in mortality and liver-related complications, regardless of whether patients had cirrhosis or experienced cancer recurrence.
  • The findings suggest that DAA therapy should be recommended for all patients with HCV-related liver cancer following curative surgery, as it is just as effective as IFN-based therapy in improving survival outcomes.

Background: Right ventricular outflow tract obstruction relief is one of the major procedures during the total correction of tetralogy of Fallot (TOF). Pulmonary insufficiency (PI) is usually inevitable after a transannular incision with a patch repair is performed. Therefore, some surgeons advocate to place a monocusp valve within the transannular patch (TAP) in order to decrease the severity of the PI.
