The adjuvant and post-surgical treatment of gynaecological cancers has historically been guided by the estimation of relapse risk based on clinicopathological factors determined at the time of cancer diagnosis. The recent advancement of genomic and molecular characterisation of gynaecological cancers has begun to shift paradigms in the selection of adjuvant treatment strategy. Recent data regarding the predictive and/or prognostic value of molecular tests in the treatment of advanced ovarian cancer as well as early stage endometrial cancer have been the first such examples to enter adjuvant treatment guidelines for these diseases.

Antegrade flow digital artery flaps enjoy a reputation for versatility and reliability with a robust vascular pedicle and the capability of resurfacing small to moderate sized defects in virtually any part of the hand. More than five decades of experience and evolution of surgical techniques with this class of intrinsic hand flaps makes this an indispensable part of the hand surgeons armamentarium when it comes to soft tissue reconstruction in the hand. Variant techniques and variable resurfacing indications are described for these class of flaps in this article.

Pulmonary embolism as a cause of unexplained sinus tachycardia after right ventricular myocardial infarction.

We present the case of a patient who developed new-onset asymptomatic sinus tachycardia after undergoing treatment for a right ventricular myocardial infarction. Even after excluding heart failure, infection and bleeding, the sinus tachycardia persisted. Computed tomography pulmonary angiography showed multiple bilateral pulmonary emboli.
