KCNJ5 somatic mutations in aldosterone-producing adenoma (APA) are linked to higher left ventricular mass index (LVMI) and worse diastolic function. We previously identified an association between plasma tissue inhibitor of metalloproteinases-1 (TIMP-1) and an aldosterone-induced increase in LVMI and diastolic dysfunction. This study aimed to investigate the association between the presence of KCNJ5 somatic mutation and plasma TIMP-1 in APA patients.

Article Synopsis
  • Extracorporeal cardiopulmonary resuscitation (ECPR) can help improve outcomes for patients experiencing in-hospital cardiac arrest (IHCA), and the RESCUE-IHCA score was created to predict the outcomes of these patients.
  • A study involving 324 ECPR-treated IHCA patients showed that the RESCUE-IHCA score maintained similar performance in predicting mortality compared to the original cohort, with key findings indicating that longer cardiac arrest duration increases mortality risk while certain initial heart rhythms lower that risk.
  • Unlike the original study, factors like age, the timing of resuscitation, disease category, and pre-existing renal issues did not significantly correlate with in-hospital death in this Asian medical center's

Article Synopsis
  • Autonomous cortisol secretion (ACS) is often found in primary aldosteronism (PA) patients, but its impact on cardiovascular health, particularly vascular calcification, is not well understood.
  • In a study of 339 PA patients, those with ACS had significantly higher abdominal aortic calcification (AAC) scores compared to those without ACS, indicating a stronger cardiovascular burden.
  • The research also shows that cortisol enhances vascular calcification in the presence of high aldosterone levels through a mineralocorticoid receptor mechanism, which can be blocked by the drug eplerenone.

To achieve a consensus on optimal blood pressure (BP) targets for older adults remains challenging, necessitating a trade-off between cardiovascular benefits and the risk of impaired organ perfusion. Evidence suggests that age and frailty have a minimal influence on the cardiovascular benefits of intensive BP control in community-dwelling elderly. Nonetheless, an increased incidence of acute kidney injury with intensive BP control has been observed in octogenarians.

Article Synopsis
  • Out-of-hospital cardiac arrest (OHCA) is a significant public health concern, and understanding heart function before the event can help predict outcomes, but the role of echocardiography in this context is not fully explored.
  • This study aimed to determine how left ventricular ejection fraction (LVEF) measured before OHCA correlates with survival after hospital discharge, using data from a large cohort of adult patients in Taiwan.
  • Findings showed that higher pre-arrest LVEF is linked to better survival rates; for instance, patients with LVEF > 60% had more than five times the odds of surviving to discharge compared to those with LVEF < 40%.

Background: The survival trend and factors influencing short- and mid-term mortality in Asian out-of-hospital cardiac arrest (OHCA) survivors should be elucidated. We performed survival analyses on days 3 and 30, hypothesizing decreased survival rates within the initial 3 days post-resuscitation. Additionally, variables linked to mortality at these two timepoints were examined.
