Aims/hypothesis: Islets are thought to be stably present in the adult human pancreas to maintain glucose homeostasis. However, identification of the pancreatic intraepithelial neoplasia (PanIN)-islet complex in mice and the presence of PanIN lesions in adult humans suggest that similar remodelling of islet structure and environment may occur in the human pancreas. To identify islet remodelling in a clinically related setting, we examine human donor pancreases with 3D histology to detect and characterise the human PanIN-islet complex.

Article Synopsis
  • - Small cell lung cancer (SCLC) has poor clinical outcomes due to the lack of effective long-term treatments, with the transcriptional factor FOXM1 playing a key role in cell proliferation and tumor progression.
  • - Research indicates that FOXM1 is significantly upregulated in SCLC, correlating with pathways linked to cancer growth and DNA damage repair, and its depletion enhances sensitivity to chemotherapy.
  • - High levels of FOXM1 expression in SCLC patients are associated with advanced disease, worse overall survival rates, and shorter progression-free survival after standard treatment, suggesting FOXM1 could serve as a valuable prognostic biomarker and therapeutic target.

Intraportal islet transplantation has been clinically applied for treatment of unstable type 1 diabetes. However, in the liver, systematic assessment of the dispersed islet grafts and the graft-hepatic integration remains difficult, even in animal models. This is due to the lack of global and in-depth analyses of the transplanted islets and their microenvironment.

Article Synopsis
  • The study analyzed treatment outcomes for patients with lung adenocarcinoma and EGFR mutations who received first-line tyrosine kinase inhibitors (TKIs) and followed up with systemic chemotherapy.
  • A total of 4,889 patients were involved, with results showing no significant overall survival (OS) advantage for afatinib compared to other TKIs, although afatinib did benefit certain subgroups like active smokers and patients with worse performance status.
  • Additionally, when chemotherapy was administered after first-line TKIs, pemetrexed was linked to better OS outcomes compared to other agents, especially gemcitabine.

Electrocardiogram lead selection for intelligent screening of patients with systolic heart failure.

Sci Rep

January 2021

Department of Electrical and Computer Engineering, College of Electrical and Computer Engineering, National Chiao-Tung University, Hsinchu, Taiwan.

Electrocardiogram (ECG)-based intelligent screening for systolic heart failure (HF) is an emerging method that could become a low-cost and rapid screening tool for early diagnosis of the disease before the comprehensive echocardiographic procedure. We collected 12-lead ECG signals from 900 systolic HF patients (ejection fraction, EF < 50%) and 900 individuals with normal EF in the absence of HF symptoms. The 12-lead ECG signals were converted by continuous wavelet transform (CWT) to 2D spectra and classified using a 2D convolutional neural network (CNN).

Article Synopsis
  • - Atrial fibrillation (AF) can lead to serious health issues, and modern pacemakers are used to detect episodes of high heart rates (AHREs) as an indicator of AF.
  • - This study examined whether the inflammatory biomarker hs-CRP might predict future AHREs in patients with pacemakers and relatively normal heart function, collecting data from 171 patients over several years.
  • - Findings showed that higher levels of hs-CRP were significantly associated with an increased risk of AHREs, suggesting that inflammation might be involved in the development of AF in these patients.

The idea of protocol biopsy is to detect subclinical pathologies, including rejection, recurrent disease, or infection for early intervention and adjustment of immunosuppressants. Nevertheless, it is not adopted by most clinicians because of its low yield rate and uncertain long-term benefits. This retrospective study evaluated the impact of protocol biopsy on renal function and allograft survival.
