It has been suggested that intravesical treatment with mitomycin C (MMC) before instillation of bacillus Calmette-Guérin (BCG) improves the antitumor activity of BCG in human bladder cancer. Therefore, we studied the immunological effects of sequential intravesical treatment with MMC and BCG in the guinea pig. Four weekly intravesical instillations with MMC preceded six weekly intravesical BCG instillations.

The urine of superficial bladder cancer patients treated intravesically with BCG was examined for the presence of leukocytes and cytokines. The number of leukocytes was markedly increased 24h and 48h after five or more BCG instillations. Flow cytofluorometric analysis revealed that, in addition to large numbers of granulocytes, monocytes/macrophages and T-lymphocytes were clearly detectable.

In line 10 hepatocellular carcinoma, cellular infiltrates show that there is an ongoing immune response against the tumor. This immune response is changed by intralesional administration of BCG leading to tumor regression and immunity. In the draining lymph node BCG induces a considerable increase in the number of T helper/inducer cells and the T suppressor/cytotoxic cells, as well as an increase in MHC class II antigen expression, but does not alter the ratio of the various leukocyte subpopulations.

We investigated the cellular composition and the major histocompatibility complex (MHC) class II antigen expression in the draining lymph node and the tumour during potentiation of the immune response by intralesional bacillus Calmette-Guérin (BCG) administration in the line 10 hepatocellular carcinoma in the strain 2 guinea-pig. Five days after its injection BCG induced a ninefold increase in the number of draining lymph node cells and an increased MHC class II expression. This increased MHC class II expression was mostly due to the selective increase of B cells in the lymph nodes, and to a lesser extent to the increase of T cells expressing MHC class II antigens.
