Background: Heterogeneity of treatment effects (HTEs) can occur because of either differential treatment compliance or differential treatment effectiveness. This distinction is important, as it has action implications, but it is unclear how to distinguish these two possibilities statistically in precision treatment analysis given that compliance is not observed until after randomization. We review available statistical methods and illustrate a recommended method in secondary analysis in a trial focused on HTE.

Laboratorio de Neurogénesis, Subdirección de Investigaciones Clínicas, Instituto Nacional de Psiquiatría Ramón de la Fuente Muñiz, Calzada México-Xochimilco #101, Col. San Lorenzo Huipulco, Tlalpan, Mexico City C.P. 14370, Mexico.

Unlabelled: Major Depressive Disorder (MDD) is a neuropsychiatric condition whose neurobiological characteristics include alterations in brain plasticity, modulated by Brain-Derived Neurotrophic Factor (BDNF). In animal models, environmental enrichment promotes neuroplasticity and reduces depressive-like behaviors. In humans, we proposed to assess the level of Enriched Environment (EE) using a questionnaire that includes different domains of the EE (cognitive, social, and physical), which we named the EE Indicator (EEI).

Data-Driven Cutoff Selection for the Patient Health Questionnaire-9 Depression Screening Tool.

JAMA Netw Open

November 2024

Department of Epidemiology, Biostatistics and Occupational Health, McGill University, Montréal, Québec, Canada.

Article Synopsis
  • The study investigates how using small datasets to select an optimal cutoff score for the Patient Health Questionnaire-9 (PHQ-9) can lead to inaccurate results.
  • Researchers evaluated whether data-driven methods for cutoff selection resulted in scores that were significantly different from the true population optimal score and if these methods produced biased accuracy estimates.
  • Findings showed that many small studies frequently failed to identify the correct optimal cutoff score, particularly in smaller samples, leading to an overestimation of test sensitivity.

Article Synopsis
  • Idiopathic pulmonary fibrosis (IPF) mainly affects patients over 65, with several recently identified risk factors, though its exact cause is still unknown.
  • The abnormal lung structure in IPF, combined with activated fibroblasts, leads to hypoxia and promotes the production of exosomes that are involved in cell communication.
  • The study explores how hypoxic conditions influence the contents of exosomes and their role in changing fibroblast metabolism during the progression of IPF, a topic that needs further investigation.

Int J Mol Sci

October 2024

Department of Physiology, Facultad de Medicina, Universidad Nacional Autónoma de México, Mexico City 04510, Mexico.

Article Synopsis
  • The study investigates the effects of a high-fat diet (HFD) on pain sensitivity, obesity, and inflammation in adult male Wistar rats, focusing on potential brain dysfunctions.
  • Rats on the HFD showed increased metabolic issues (like insulin resistance), a heightened pain response, and impaired motor coordination compared to control rats.
  • High-fat diet consumption led to significant structural changes in brain regions such as the hypothalamus and cerebellum, indicating potential neuronal damage which might affect sensory-motor functions, suggesting a future avenue for therapeutic strategies.

Article Synopsis
  • A large-scale, cross-national study examined the impact of removing the "excessiveness" requirement for diagnosing generalized anxiety disorder (GAD) among individuals living in challenging circumstances.
  • Data from over 133,000 adults across various income countries revealed that eliminating this criterion raises the global prevalence of GAD from 2.6% to 4.0%, especially in low- and middle-income countries.
  • Non-excessive worriers, while less severe, exhibit similar socio-demographic traits and impairment levels as excessive worriers, indicating they are significant cases deserving diagnosis and treatment.

Effects of selective serotonin reuptake inhibitors on endocrine system (Review).

Biomed Rep

September 2024

Department of Biotechnology, Faculty of Chemistry, Universidad Autónoma de Coahuila, Saltillo Coahuila 25280, México.

Selective serotonin reuptake inhibitors (SSRIs) are typically prescribed for treating major depressive disorder (MDD) due to their high efficacy. These drugs function by inhibiting the reuptake of serotonin [also termed 5-hydroxytryptamine (5-HT)], which raises the levels of 5-HT in the synaptic cleft, leading to prolonged activation of postsynaptic 5-HT receptors. Despite the therapeutic benefits of SSRIs, this mechanism of action also disturbs the neuroendocrine response.
