Heart Rhythm

December 2024

William Harvey Research Institute, Barts and the London Faculty of Medicine and Dentistry, Queen Mary University of London, London, UK; National Institute of Health and Care Research, Barts Biomedical Research Centre, Queen Mary University of London, London, UK. Electronic address:

Background: Atrial fibrillation (AF) prediction improves by combining clinical scores with a polygenic risk score (PRS) for AF (AF-PRS), but there are limited studies of PRS for ventricular arrhythmia (VA) prediction.

Objective: We assessed the value of including multiple PRS for cardiovascular risk factors (CV-PRS) for incident AF and VA prediction.

Methods: We used 158,733 individuals of European ancestry from UK Biobank to build 3 models for AF: CHARGE-AF (AF1), AF1 + AF-PRS (AF2), AF2 + CV-PRS (AF3).

Background: Long-term exposure to aircraft noise has been associated with small increases in cardiovascular disease risk, but there are almost no short-term exposure studies.

Objectives: Research questions were: Is there an association between short-term changes in exposure to aircraft noise and cardiovascular morbidity and mortality? What are the key effect modifiers? Is there variability in risk estimates between areas with consistent versus changing patterns of noise exposure? Do risk estimates differ when using different noise metrics?

Design: Descriptive analyses of noise levels and variability at different times of day, analyses of inequalities in noise exposure and case-crossover analyses of cardiovascular events in relation to aircraft noise exposure.

Setting: Area surrounding London Heathrow airport.

Donor Time to Death and Kidney Transplant Outcomes in the Setting of a 3-Hour Minimum Wait Policy.

JAMA Netw Open

November 2024

National Institute of Health and Care Research Blood and Transplant Research Unit, Newcastle University and Cambridge University, Newcastle upon Tyne, United Kingdom.

Importance: Lengthening waiting lists for organ transplant mandates the development of strategies to expand the deceased donor pool. Due to concerns regarding organ viability, most organ donation organizations internationally wait no longer than 1 to 2 hours for potential donation after circulatory death (DCD), possibly underutilizing an important organ source; UK policy mandates a minimum 3-hour wait time.

Objective: To assess whether time to death (TTD) from withdrawal of life-sustaining treatment (WLST) is associated with kidney transplant outcomes.

January 2025

Research Unit of Clinical Medicine, Department of Psychiatry, University of Oulu, Oulu, Finland; Department of Psychiatry, Oulu University Hospital, Oulu, Finland; Medical Research Center Oulu, Oulu University Hospital and University of Oulu, Oulu, Finland. Electronic address:

Article Synopsis
  • Maternal immune activation (MIA) during pregnancy may contribute to psychiatric disorders in children, but its effects on brain development and sex differences are not fully understood.
  • A study analyzed data from 2,635 mother-child pairs to explore how maternal levels of CRP, a marker of inflammation, correlate with offspring brain structure, focusing on cortical thickness, surface area, and volume.
  • Findings reveal that normal maternal CRP levels show different effects on brain development in boys and girls, while higher CRP levels are linked to changes in brain structure in both sexes, highlighting the importance of sex and inflammation in neurodevelopment.

Immortal time may arise in survival analyses when individuals are assigned to treatment strategies based on post-eligibility information or selected based on post-assignment eligibility criteria. Selection based on eligibility criteria applied after treatment assignment results in immortal time when the analysis starts the follow-up at assignment. Misclassification of assignment to treatment strategies based on treatment received after eligibility results in immortal time when the treatment strategies are not distinguishable at the start of follow-up.

Article Synopsis
  • Early phase clinical trials focus on evaluating the safety and tolerability of new therapies, primarily using investigator and lab results rather than patient feedback.
  • A recent expert roundtable brought together 22 stakeholders to discuss the need for a universal patient-reported outcomes (PRO) model in early phase trials and how to effectively incorporate PROs to assess tolerability and guide treatment decisions.
  • The discussions yielded key recommendations and identified priority areas for further research on integrating patient feedback into early trials to improve safety and symptom management.

Article Synopsis
  • Cerebrovascular events (CVEs) can happen during or after a procedure called TAVR, which replaces a heart valve, and can lead to serious health problems.
  • Even though strokes have become less common since TAVR started, they’re still a major concern because TAVR is being used more often.
  • New devices and treatments are being tested to help prevent CVEs, but current options may not reduce stroke risk enough, and there are concerns about bleeding from the medication used.

Article Synopsis
  • - The review investigates how eHealth interventions can improve outcomes for patients suffering from Post Intensive Care Syndrome (PICS), which includes long-term physical, psychological, and cognitive impairments after critical care. - Thirteen studies were analyzed, showing significant variability in how eHealth was delivered and the outcomes measured, with a focus predominantly on feasibility and limited targeting of cognitive impairments. - Although interventions aimed at psychological recovery report some positive effects, the studies lacked robust data and comprehensive assessments of their acceptability; aspects like usability and perceived usefulness were noted as facilitators, while acceptability remained an unexplored barrier in many cases.

Purpose: The purpose of this paper is to describe the impacts of cancer treatment on sexual health in a sample of people who had been treated for mixed types of cancer; to describe discussions they had with professionals about sexual health that occurred during cancer care; and to consider the extent to which these discussions were sufficient to enable participants to give informed consent for the sexual side effect of cancer treatment.

Method: A cross-sectional, online survey using a convenience sample of people with cancer was recruited via UK cancer charities. Eligibility criteria included having received treatment and follow-up care for any type of cancer in the UK during the previous 10 years.

Given the proven benefits of screening to reduce diabetic ketoacidosis (DKA) likelihood at the time of stage 3 type 1 diabetes diagnosis, and emerging availability of therapy to delay disease progression, type 1 diabetes screening programs are being increasingly emphasized. Once broadly implemented, screening initiatives will identify significant numbers of islet autoantibody-positive (IAb+) children and adults who are at risk for (confirmed single IAb+) or living with (multiple IAb+) early-stage (stage 1 and stage 2) type 1 diabetes. These individuals will need monitoring for disease progression; much of this care will happen in nonspecialized settings.

Brachial artery flow-mediated dilation (BAFMD) is induced by hyperemic wall shear rate (WSR) following forearm ischemia. In older adults, there appears to be a reduced brachial hyperemic WSR and altered stimulus-response relationship compared with young adults. However, it is unclear if an altered forearm microvascular response to ischemia influences brachial hyperemic WSR in older adults.
