Epidemiological and clinical studies have shown that a low plasma high‑density lipoprotein cholesterol (HDL-C) level is a strong predictor of cardiovascular disease (CVD). Lecithin:cholesterol acyltransferase (LCAT) is a key enzyme in the formation, maturation and function of HDL. Therefore impaired LCAT function may enhance atherosclerosis because of defective cholesterol transport.

[Surgical treatment results of congenital heart defects in children with Down's syndrome].

We made a retrospective study of patients with Down's syndrome that were surgically treated for correction or palliation for their congenital heart disease between january 1996 to december of the 2000 in the National Institute of Cardiology "Ignacio Chávez". We analyzed these variables: age at the surgical moment, sex, congenital heart defect, pulmonary arterial pressure, type of surgery, time of stay in the intensive care unit, complications and mortality. In this period they were surgically treated 37 patients.

The protective role of estrogens against peripheral vascular and coronary disease in women is well documented; however, it is not present in diabetic women. Estrogens reduce tension development through non-genomic mechanisms that include changes in calcium concentrations in endothelial and smooth muscle cells, and regulation of nitric oxide synthase (NOS) in endothelial cells. Insulin increases endothelin-1 (ET-1) release from endothelial cells modulating smooth muscle calcium levels and elevating force generated by femoral and coronary arteries.

The INC has been a pioneer in producing cardiac bioprostheses in Mexico. Since the 1970's it has produced and implanted more than 2,500 bioprostheses, first made of duramater and later on, made of cattle pericardium. Nowadays, it is working on the production of a new generation of bioprostheses of cattle pericardium as well as of pigs with or without support; to these, a system to reduce calcification has been added, as a result of research being performed at the Applied Biotechnics Department.

It was not until 1947, in Boston (USA), when a cadaveric kidney was transplanted into a female patient with septic shock, renewing natural diuresis in two months. In 1950, different European and American medical teams had already proceeded with renal transplants in humans from cadaver donors with positive results, followed by an absolute success with the first renal transplant in univitelline twins in 1954.

The World Health Organization (WHO) and several other international committees on the study of hypertension define that a patient with blood pressure between 140/90-150/95 mmHg must be treated. In the first stage of hypertension it is possible to modify it with life style changes, but if high blood pressure persists, then drug therapy must be evaluated carefully. Drug therapy management is individualized and based upon age, race, previous history of hypertension, and risks for target systemic organs.

Pharmacological therapeutics in acute myocardial infarction. The acute myocardial infarct is one of the main causes of death among the adult population of the industrialized countries, and this pattern is starting to be paralleled in many developing countries. The mortality rate due to coronary diseases has decreased 30% in the last decade, thanks to the new therapeutic strategies and the guidelines developed by the American college of Cardiology and the American Heart Association, aimed at physicians, nurses, and health personnel in charge of patients with suspected myocardial infarct or with an established myocardial infarct.

Bidirectional cavopulmonary shunt is an alternative palliative procedure for patients with congenital cyanotic heart disease, specially those patients less than "ideal" candidates for a Fontan's procedure. We present our results with this shunt in patients with tricuspid atresia. Twenty patients with tricuspid atresia were operated on with this shunt, with these associated defects: 20 atrial septal defect, 17 ventricular septal defect, 10 pulmonary stenosis, 1 pulmonary atresia and 1 transposition of the great arteries.

Percutaneous balloon coronary angioplasty had a slow start in Mexico, and until recently only a relatively small number of procedures have been performed. Since March 1991, the Instituto Nacional de Cardiología has state-of-the-art fluoroscopic imaging and easy access to a variety of balloon coronary angioplasty catheters and accessories. Under these conditions, an increasing number of PTCAs have been performed (221 procedures in an initial 15 month period).
