Purpose Of Review: This expert opinion, prepared by a panel of chest disease specialists, aims to review the current knowledge on practice patterns in real-life management of mild asthma and to address the relevant updates in asthma treatment by The Global Initiative for Asthma (GINA) to guide clinicians for the best clinical practice in applying these new treatment paradigms.

Recent Findings: On the basis of the emerging body of evidence suggesting the non-safety of short-acting β2-agonists (SABA)-only therapy and comparable efficacy of the as-needed inhaled corticosteroids (ICS)-formoterol combinations with maintenance ICS regimens, GINA recently released their updated Global Strategy for Asthma Management and Prevention Guide (2019). The new GINA 2019 recommendations no longer support the SABA-only therapy in mild asthma but instead includes new off-label recommendations such as symptom-driven (as-needed) low-dose ICS-formoterol and "low dose ICS taken whenever SABA is taken.

Objective: Peripheral T-cell lymphomas (PTCLs) are an uncommon and quite heterogeneous group of disorders, representing only 10%-15% of all non-Hodgkin lymphomas. Although both molecular and clinical studies have increased in recent years, we still have little knowledge regarding real-life practice with PTCLs. In this study, we aimed to investigate the clinical characteristics and treatment outcomes of a large population-based cohort of patients presenting with systemic non-cutaneous PTCL.

Article Synopsis
  • Severe IgE-mediated reactions pose significant challenges during oral immunotherapy (OIT) for cow's milk allergy in children, with 73.8% experiencing adverse reactions during the build-up phase.
  • A study of 86 children showed a decrease in reactions from the build-up phase to the maintenance phase, highlighting the importance of monitoring symptoms like cough during food challenges.
  • Key biomarkers, such as higher levels of specific immunoglobulin E (sIgE) and younger age at the onset of OIT, can help identify patients at increased risk for severe reactions.

Objective: Irisin is a myokine thought to be involved in the pathophysiological process of atherosclerosis with its' cardiovascular protective effects. Patients with diabetes mellitus (DM) have lower levels of irisin. Therefore, we investigated whether there is a connection between irisin, DM, coronary collateral circulation (CCC), and SYNTAX scores representing coronary artery disease (CAD) severity.

Background: The coexistence of hyponatremia and atrial fibrillation (AF) increases morbidity and mortality in patients with heart failure (HF). However, it is not established whether hyponatremia is related to AF or not.

Objective: Our study aims to seek a potential association of hyponatremia with AF in patients with reduced ejection fraction heart failure (HFrEF).

Objective: Testicular torsion is a condition that manifests with acute pain and can lead to infertility despite urgent surgical intervention. The aim of this study is to investigate the protective and preventive effects of etomidate, an imidazole derivative, a non-barbiturate general anesthetic agent, without analgesic effect, on testicular ischemia-reperfusion injury.

Materials And Methods: Twenty-four adult male Wistar rats weighing 200-250 g were used in the study.

Objective: Concomitant use of drugs not only enhances the therapeutic effect, but may also lead to undesirable interactions. Drug interactions are frequently seen in intensive care patients. In this study, we aimed to determine the frequency and clinical severity of drug interactions in Medical Intensive Care Unit (MICU) patients.

Objective: Advances in knowledge of neurotrophic factors are now revealing the complex control of energy homeostasis and appetite, as well as the crucial role these factors play in nervous system function. The aim of this study was to assess serum levels of neudesin in adolescents with obesity and to examine the relationship between these levels and metabolic outcomes.

Methods: Adolescents, aged 10-17 years were enrolled.

Background: Defects due to car wheel injury at the anterolateral aspect of the foot are challenging due to the characteristics of the region and the trauma. The aim of this study was to present the results of the patients whose skin defects on the dorsolateral aspect of the foot due to tire injuries were treated with reverse sural artery fasciocutaneous flaps.

Methods: Fourteen patients with a mean age of 26.

Background: The objective of the study is to compare the clinical results of the single-bundle (SB) and double-bundle (DB) anterior cruciate ligament (ACL) repairing techniques using magnetic resonance imaging (MRI).

Methods: Thirty-eight patients were randomized by block randomization into two different groups of ACL reconstruction: DB (n=19) and SB techniques (n=19). MRI evaluation and clinical examination with modified Cincinnati Knee Rating Score and Lysholm knee scores were performed pre-operatively and at the end of a follow-up period of 36 months.

Aim: Our study aimed to investigate the predictive values of inflammation markers in predicting postoperative saphenous vein graft patency in patients who underwent coronary artery bypass grafting (CABG).

Method: We retrospectively analyzed 89 patients who undergone CABG, and 49 patients diagnosed with non-critical coronary artery disease (less than ≤50% stenosis) on coronary angiography were included in the study as a control group. Eighty-nine patients who underwent CABG were divided into two groups according to the presence of 50% or more stenosis in saphenous vein grafts.

Background: A sepsis model was created, induced by cecal ligation and puncture (CLP), in juvenile rat groups. Milrinone (MIL), which is known to have a modulatory effect on pro-inflammatory cytokines, was administered to the designated rat groups in the early period before severe sepsis developed. The study was aimed at investigating the possible protective effects of milrinone on the lung and kidney tissues of rats in the late phase of sepsis.

Retrograde intrarenal surgery (RIRS) is a safe and effective treatment option for upper urinary tract stones smaller than 2 cm. Although several studies have documented perioperative and postoperative complications related to RIRS, there exists limited data regarding the readmission and rehospitalization of patients after RIRS. The aims of the study were to document the rates of readmission and rehospitalization after RIRS and to determine the predictive factors for readmission and rehospitalization.
