Background: Recently, neoadjuvant chemotherapy with docetaxel/cisplatin/5-fluorouracil (NAC-DCF) was identified as a novel strong regimen with a high rate of pathological complete response (pCR) in advanced esophageal cancer in Japan. Predicting pCR will contribute to the therapeutic strategy and the prevention of surgical invasion. However, a predictor of pCR after NAC-DCF has not yet been developed.

We investigated the effects of a vibratory stimulus on the plantar surface of the hind limb for motor, sensory, and locomotive function using a mouse cast model. The right knee joint of C57BL/6 male mice (7 weeks, 20 g, n = 31) was flexed with aluminum splint and tape for 6 weeks. These mice were randomly divided into 2 groups (control group, n = 11 and vibration group, n = 12).

Heat shock proteins (HSPs), which are molecular chaperones that stabilize numerous vital proteins, may be attractive targets for cancer therapy. The aim of the present study was to investigate the possible anticancer effect of single or dual targeting of HSP90 and HSP70 and the combination treatment with HSP inhibitors and chemotherapeutic agents in bladder cancer cells. The expression of HSP90 and the anticancer effect of the HSP90 inhibitor 17-N-allylamino-17-demethoxygeldanamycin (17-AAG) coupled with cisplatin, docetaxel, or gemcitabine were examined using immunohistochemistry, quantitative real-time PCR, cell growth, flow cytometry, immunoblots and caspase-3/7 assays.

A 69-year-old man with a 4-year history of ulcerative colitis (UC) presented at our hospital with high fever, dysarthria, and right hemiparesis. Computed tomography (CT) of the head revealed a low-density area in the left temporal lobe. Chest CT exposed multiple pulmonary nodules in his right lung.

Background: Although intravenous hydration with isotonic saline is the standard therapy for the prevention of contrast-induced nephropathy (CIN), there is still insufficient evidence concerning the optimal timing to initiate preprocedural intravenous hydration with isotonic saline.

Methods: This study prospectively compared the contrast-induced increases in serum creatinine and cystatin C between 5-hour preprocedural intravenous hydration with isotonic saline (5h-HS) and 20-hour preprocedural intravenous hydration with isotonic saline (20 h-HS) in 122 patients with renal insufficiency (estimated glomerular filtration rate of 15-60 ml/min/1.73 m(2)) undergoing an elective coronary procedure.

Objective: It is difficult to cover a large skin and soft tissue defect with exposure of the ilium. We therefore performed a new reconstruction technique, using a reverse latissimus dorsi muscle flap fed by perforating branches of only the 10th intercostal artery.

Methods: A 45-year-old man had a large traumatic defect located on the hip with exposure of the iliac crest.

This report presents a patient in which balloon occlusion of the posterior descending artery wrapping the apex produced negative U waves in leads V(4-5). This case report demonstrates, for the first time, that negative U waves in the precordial leads can appear by occlusion of the long posterior descending artery.

We report a rare case of a male patient with acute myocardial infarction, in whom spontaneous recanalization of an occlusive lesion in the left anterior descending artery produced new ST-segment elevation in the precordial leads, although the exact mechanism for this phenomenon is unclear.

Object: The use of percutaneous suction aspiration has recently become viewed as an effective management strategy for pyogenic spondylitis unresponsive to conservative treatment. What remains unclear is whether it can be effective for severe pyogenic spondylitis in which abscess formation or marked bone destruction is present. The authors undertook a study to clarify answers to this question.

Study Design: Radiologic study for patients with athetoid cerebral palsy (CP), and a biomechanical study using the finite-element model (FEM).

Objectives: To understand the lumbar disorders of athetoid CP patients and the etiology based on the results from the FEM study.

Summary Of Background Data: Cervical spondylotic myelopathy is a well-identified spinal disorder associated with cerebral palsy, especially in athetoid type.

A 63-year-old female with left ureteral tumor metastasizing to the left iliac lymph node was treated with left total nephroureterectomy. The histopathological diagnosis was squamous cell carcinoma of the ureter, G1 INF beta, pT2, pR0, pV1 and pL0. Three cycles of chemotherapy were performed postoperatively with cis platin and 5-fluorouracil.

Salt enhances responsiveness of terminal aortic vascular bed to sympathetic activity.

Eur J Clin Pharmacol

September 1997

Department of Cardiovascular Medicine, PIA Nakamura Hospital, Hiroshima, Japan.

Objective: Haemodynamic and laboratory examinations were performed in 27 essential hypertensives. The participation of the sympathetic system was estimated from the percentage decrease in regional vascular resistance caused by 1 mg prazosin, during varied salt intake (50-80 mEq sodium per day for 9 weeks, followed by 220-250 mEq sodium per day for 10 days), with or without benidipine.

Results: During low salt intake, terminal aortic and renal resistances were decreased by prazosin, but superior mesenteric resistance remained unchanged.

Lisinopril reverses left ventricular hypertrophy through improved aortic compliance.

Hypertension

September 1996

Department of Cardiovascular Medicine, PIA Nakamura Hospital, Hiroshima, Japan.

We treated with nifedipine or lisinopril 38 essential hypertensive patients with left ventricular hypertrophy. The study had a single-blind crossover design; nifedipine or lisinopril was given for the first 24 weeks, and then patients were crossed over to the other antihypertensive agent for another 24 weeks. Both nifedipine and lisinopril significantly decreased mean arterial pressure to the same extent.

We compared haemodynamic and humoral responses to nifedipine and nilvadipine in 25 elderly hypertensives. The study had a single-blind crossover design. Doppler flowmetry and laboratory examinations were performed before and after nifedipine and nilvadipine.

Lisinopril improves aortic compliance and renal flow. Comparison with nifedipine.

Hypertension

March 1995

Department of Cardiovascular Medicine, PIA Nakamura Hospital, Hiroshima, Japan.

We compared the systemic and regional hemodynamic effects of nifedipine and lisinopril in 26 elderly hypertensive patients with the use of the pulsed Doppler ultrasound technique. Nifedipine is a dihydropyridine calcium antagonist, and lisinopril is an angiotensin-converting enzyme inhibitor. The study had a single-blind crossover design: nifedipine and lisinopril were given for 8 weeks each after washout periods of 4 weeks.

Ten elderly hypertensives (4 men, 6 women) were studied before and after 8-week administration of 20 mg/day slow-release nifedipine. Systemic and regional hemodynamic data were measured using a pulsed Doppler technique. Slow-release nifedipine reduced mean arterial pressure, while cardiac output remained unchanged, resulting in a decrease in total peripheral resistance.

[A case of subfrontal schwannoma].

No Shinkei Geka

November 1992

Department of Neurosurgery, Oita Nakamura Hospital, Japan.

We encountered a rare case of subfrontal schwannoma. A 55-year-old woman had received resection of a left frontal tumor because of hyposmia, at the age of 28 years. On June 10, 1989, she was admitted with the chief complaint of progressive contraction of visual field.

A method of measuring volumetric flow in human arteries with pulsed Doppler technique was developed. Doppler flowmetry was assessed in 11 adult patients whose admission-diagnoses included aneurysm of the aorta or dissections. During surgery, regional blood flows of the left common carotid artery, terminal aorta, common iliac artery, external iliac artery or common femoral artery were measured by Doppler method and electromagnetic flowmeter.
