Diffuse gastroduodenitis resembling ulcerative colitis in respect to macro- and microscopic findings occurs in ulcerative colitis, although it is rare. Reports of gastroduodenitis associated with ulcerative colitis treated with infliximab are rare. A 58-year-old man had tarry stool in March 2011.

Aortic valve replacement using CEP Magna 21 mm bioprosthetic valve was performed because of aortic valve stenosis in a 75-year-old man with maintenance dialysis. In the 39th postoperative month, the bioprosthetic valve malfunction due to calcification was noted, and it was replaced. Judging from the previously reported cases, malfunction of an artificial valve in the 39th month is thought to be relatively early.

The small bowel is the least common site of involvement in the gastrointestinal tract for cytomegalovirus (CMV) infection. CMV enteritis results in a high rate of emergency surgery for bleeding, perforation, or ileus and a high mortality rate. Lower extremity venous ultrasound use in the detection of VTE has been studied and has proven both reliable and efficient. Recently pericardial drainage after cardiac surgery has been done using silicon tubes of small diameter. For more effective drainage, we set one of the drainage tubes circularly, coursing behind the left ventricle, through the transverse sinus, and ending at the right side of the atrium (circular pericardial drainage). As compared to conventional drainage using 28 Fr chloroethilene tubes, drainage time was shorter and no late tamponade had occurred.

We report a case of acute dissection of the ascending aorta with difficult weaning from cardiopulmonary bypass (CPB) due to acute myocardial infarction (MI). Difficult separation from CPB was thought to be due to global coronary artery malperfusion, caused by diffuse left coronary artery sclerosis and the obstructed origin of the right coronary artery arising from the left sinus of Valsalva, resulting in intraoperative MI. In this case, supplemental coronary artery bypass grafting was effective for the separation of the patient from CPB.

Aim: To investigate whether semi-vegetarian diet (SVD) has a preventive effect against relapse of Crohn's disease (CD) in patients who have achieved remission, who are a high-risk group for relapse.
Methods: A prospective, single center, 2-year clinical trial was conducted. Twenty-two adult CD patients who achieved clinical remission either medically (n = 17) or surgically (n = 5) and consumed an SVD during hospitalization were advised to continue with an SVD and avoid known high-risk foods for inflammatory bowel disease.

We experienced anesthetic management of a patient undergoing surgeries for bilateral femoral trochanter fractures before the onset and during the healing stages of takotsubo cardiomyopathy (TCM). While performing the surgery before the onset of TCM, we administered 0.5% intrathecal isobaric bupivacaine 15 mg and the procedure was completed uneventfully.

There have only been a few reports on lansoprazole-associated collagenous colitis. Colonic mucosa of collagenous colitis is known to be endoscopically normal. We present a case of collagenous colitis where the mucosa showed diffuse cloudiness mimicking ulcerative colitis.

Intravascular large B cell lymphoma (IVLBCL) is a rare type of non-Hodgkin lymphoma characterized by a disseminated intravascular proliferation of tumor cells in the lumina of small vessels. Although the kidney is one of the target organs of IVLBCL, it is extremely rare that lymphoma cells are localized only in the kidney. We report here a Japanese patient with kidney-limited IVLBCL.

The five-year event-free survival of nearly 80% in childhood acute lymphoblastic leukemia (ALL) achieved in the 1990 s attested to the effectiveness of risk-directed therapy developed through well-designed clinical trials by 4 groups in clinical study, containing CCLSG, TCCSG, KYCCSG and JACLS. Japanese Pediatric Leukemia/Lymphoma Study Group (JPLSG) was organized in 2003 and includes all four clinical study groups in Japan. For the purpose of finding the standard treatment, JPLSG protocols have been started for three distinct and rare types of ALL, including mature B-ALL, infant ALL and Ph+ALL.

Background: Although nocturnal episodic hypoxemia after major abdominal surgery has been reported, the condition of nocturnal oxygen saturation after lung surgery is largely unknown. This study also compared the postoperative course of nocturnal oxygen saturation after standard lobectomy with posterolateral thoracotomy and lobectomy with video-assisted thoracic surgery.

We report the case of a patient with Marfan's syndrome and a Stanford type B chronic aortic dissection in which replacement of the ascending aorta, aortic arch and descending aorta was accomplished in a single stage via median sternotomy. The patient was a 51-year-old woman with a 70 mm Stanford type B chronic aortic dissection and Marfan's syndrome. Median sternotomy and replacement of the ascending aorta, aortic arch, and descending aorta were performed under deep hypothermic circulatory arrest.

We present a 49-year-old female case of anomalous origin of the left main coronary artery from the pulmonary trunk. Multidetector computed tomography was performed, and 3-dimensional reconstruction of computed tomographic images found that the left main coronary artery originated from left sinus of the pulmonary trunk and the right coronary artery from the right coronary cusp of the aorta. We speculate that this patient's long life may be due to the dominant right coronary artery and rich collateral from the right coronary artery to the left coronary artery.

A female neonate with mitral stenosis due to accessory mitral valve with ventricular septal defect and patent ductus arteriosus is described. She was referred to our hospital because of neonatal asphyxia. Asphyxia was improved by ventilator support, but rapid deterioration of respiration with pulmonary congestion and hemorrhage appeared 8 days after birth.

We studied pulmonary venous (PV) flow patterns using Doppler echocardiography in 26 patients with ventricular septal defect less than 3 years of age. Fifteen patients had moderate or severe symptoms, and the remaining 11 had no significant symptoms. Peak velocity of PV diastolic flow and flow velocity integral of PV diastolic flow in the symptomatic patients were significantly larger than those in either asymptomatic patients or the normal controls.

Serial changes in diameters of ventricular septal defect (VSD), and in the formation patterns of membranous septal aneurysm (MSA), were evaluated using two-dimensional (2-D) echocardiography during the infantile period. We studied 65 patients with VSD, who were classified into three groups by clinical feature.
