Curr Treat Options Cardiovasc Med

August 2017

Vascular Medicine and Peripheral Interventions Program, Director of Vascular Medicine, Department of Cardiology, Hofstra Northwell School of Medicine, Northwell Health, Long Island Jewish Medical Center, 270-05 76th Avenue, Suite O-4000, New Hyde Park, NY, 11040, USA.

Imaging modalities to detect and diagnose vascular disease have become increasingly popular in recent years, owing in large part to their availability and accessibility. The American College of Cardiology Foundation published a two-part Appropriate Use Criteria (AUC) guidance document for both peripheral vascular imaging and physiologic testing several years ago. In the years since their publication, a number of important studies have challenged previously held beliefs about appropriateness of vascular diagnostic testing.

Article Synopsis
  • - The study examines how systemic inflammation may impact brain injury in newborns with congenital heart disease undergoing cardiac surgery, focusing on specific proteins in their serum before and after surgery.
  • - Researchers analyzed the serum levels of proteins like phosphorylated neurofilament-heavy subunit (pNF-H) and S100B, finding them significantly elevated in newborns compared to healthy cord blood, indicating potential brain injury and inflammation.
  • - Findings suggest that inflammation and issues like tissue hypoxia during surgery correlate with increased brain injury markers and prolonged mechanical ventilation, highlighting the risks associated with cardiopulmonary bypass.

Pseudomembranous tracheitis caused by in the setting of high grade T-cell lymphoma.

Respir Med Case Rep

March 2017

Northwell Health, Department of Pulmonary, Critical Care and Sleep Medicine, 410 Lakeville Rd., New Hyde Park, NY 11040, United States.

Pseudomembranous tracheitis (PMT) is a rare condition most commonly caused by fungal or bacterial infection that is characterized by a pseudomembrane that partially or completely covers the tracheobronchial tree. PMT is most commonly found in immunocompromised patient populations, such as post-chemotherapy, AIDS, post-transplant and hematological malignancies. Due to its rarity, PMT is often not included in the differential diagnosis.

The microtubule inhibitor vincristine is currently used to treat a variety of brain tumors, including low-grade glioma and anaplastic oligodendroglioma. Vincristine, however, does not penetrate well into brain tumor tissue, and moreover, it displays dose-limiting toxicities, including peripheral neuropathy. Mebendazole, a Food and Drug Administration-approved anthelmintic drug with a favorable safety profile, has recently been shown to display strong therapeutic efficacy in animal models of both glioma and medulloblastoma.

Article Synopsis
  • Pulmonary regurgitation and stenosis (PS) present challenges in patients with congenital heart defects, prompting a study comparing referral criteria for surgical (SPVR) and transcatheter pulmonary valve replacement (TPVR) methods between 2013 and 2015.
  • The study found that SPVR referrals were primarily based on CMR volumetric criteria, while TPVR referrals often focused on patient symptoms and occasional CMR abnormalities.
  • After the procedures, both groups showed improvements in heart function and volume, with the SPVR group experiencing significant gains in left ventricular output, though overall biventricular function ended up similar in both groups after one year.

Infants of diabetic mothers (IDMs) with hypertrophic cardiomyopathy are recognized to have impaired myocardial performance, but less is known about ventricular function in IDMs without hypertrophy. We hypothesized that in asymptomatic newborns with normal two-dimensional echocardiographic evaluations, pulsed wave tissue Doppler imaging (TDI) would suggest a subclinical decrease in the cardiac function of IDMs compared to infants of non-diabetics (nIDMs). This is a retrospective cohort study of asymptomatic neonates ≥36 weeks gestation, at 0-7 days of life, with normal standard echocardiograms.

Rb-82 PET/CT left ventricular mass-to-volume ratios.

Int J Cardiovasc Imaging

August 2017

Division of Nuclear Medicine and Molecular Imaging, Northwell Health, 270-05 76th Avenue, New Hyde Park, NY, 11040, USA.

Left ventricular (LV) mass:volume ratios indexed to body size (Mi/Vi) provide risk stratification for cardiac events. We sought to determine whether Rb-82 PET mass and volume indices are similar to MRI normal values for low likelihood subjects, and whether abnormal indices are related to abnormal myocardial blood flow (MBF). Data were analyzed retrospectively for 194 patients referred for rest/stress Rb-82 PET.

Transthoracic echocardiography is the most common noninvasive method of evaluating pulmonary hypertension (PH) in infants. Identification of reliable, quantitative indices of myocardial function may enhance the diagnostic value of echocardiography in this population. We hypothesized that pulsed wave tissue Doppler imaging (TDI) and tricuspid annular plane systolic excursion (TAPSE) would be reproducible measurements and would suggest decreased ventricular function, in infants with PH.
