Providers' Beliefs and Values: Understanding Their Approach to Gender-Affirming Care.

Objectives: This qualitative study explored the beliefs and values influencing healthcare providers' delivery of gender-affirming care (GAC) to transgender and gender-diverse (TGD) youth amidst current social and political dynamics.

Methods: The study PI conducted 43 semi-structured interviews with providers across states with varying GAC legislation. Responses from 41 providers were analyzed in this paper.

Emergence of an Industrial Innovation Ecosystem: Renewable Energy in India.

Theories and studies of ecosystem emergence focus on macro level explanations such as government investments in research and development or those at the organizational level such as displacement of an older technological system by a new one through competition between technologies. However, mechanisms by which such shifts occur are underemphasized. This article draws on complexity theory to develop a theoretical framework to describe how emergence is generated through top down 'rules' that constrain the behavior of the system, directing it towards a desired outcome.

Complexity Control in Artificial Self-Organizing Systems: The Case of Bottom-Up versus Top-Down Intervention When Managing Pandemic Contagion.

We model an adaptive agent-based environment using selfish algorithm agents (SA-agents) that make decisions along three choice dimensions as they play the multi-round prisoner's dilemma game. The dynamics that emerge from mutual interactions among the SA-agents exhibit two collective-level properties that mirror living systems, thus making these models suitable for societal/biological simulation. The properties are: emergent intelligence and collective agency.

The articles in this special issue examine the contributions of Jeffrey A. Goldstein to the understanding of emergence as a formal group of processes. Applications include work teams, organizations, ecologies of organizations, and societies.

Hypo-osmotic stress shifts transcription of circadian genes.

Cells respond to hypo-osmotic stress by initial swelling followed by intracellular increases in the number of osmolytes and initiation of gene transcription that allow cells to adapt to the stress. Here, we have studied the genes that change expression under mild hypo-osmotic stress for 12 and 24 hours in rat cultured smooth muscle cells (WKO-3M22). We find shifts in the transcription of many genes, several of which are associated with circadian rhythm, such as per1, nr1d1, per2, dbp, and Ciart.

Universal receptive system as a novel regulator of transcriptomic activity of Staphylococcus aureus.

Our previous studies revealed the existence of a Universal Receptive System that regulates interactions between cells and their environment. This system is composed of DNA- and RNA-based Teazeled receptors (TezRs) found on the surface of prokaryotic and eukaryotic cells, as well as integrases and recombinases. In the current study, we aimed to provide further insight into the regulatory role of TezR and its loss in Staphylococcus aureus gene transcription.

Stakeholder perspectives on barriers and facilitators to hypertension control in urban Haiti: a qualitative study to inform a community-based hypertension management intervention.

Background: Uncontrolled hypertension is the leading modifiable risk factor for cardiovascular disease mortality and remains high in low-middle income countries like Haiti. Barriers and facilitators to achieving hypertension control in urban Haiti remain poorly understood. Elucidating these factors could lead to development of successful interventions.

Pica practices, anemia, and oral health outcomes: a systemic review.

Objectives: Pica is a non-nutritive eating behavior. The potential impact of pica on oral health and the association between pica and anemia are understudied. We examined the current evidence on the relationship between pica practices, anemia, and oral health outcomes.

Effect of magnetized water on the fundamental grouting properties of cement grout under varying magnetization conditions.

The development and modification of grouting materials constitute crucial factors influencing the effectiveness of grouting. Given the pivotal role of water in the hydration of cement-based composite materials and construction processes, this study proposes an exploratory approach using green, economical magnetized water technology to enhance the performance of cement grouts. The research systematically investigates the effects of magnetized water on the fundamental grouting properties (stability, rheological behavior, and stone body strength) of cement grouts, prepared under varying magnetization conditions (including magnetic intensity, water flow speed, and cycle times).

Clinicogenomic landscape of pancreatic adenocarcinoma identifies KRAS mutant dosage as prognostic of overall survival.

Nearly all pancreatic adenocarcinomas (PDAC) are genomically characterized by KRAS exon 2 mutations. Most patients with PDAC present with advanced disease and are treated with cytotoxic therapy. Genomic biomarkers prognostic of disease outcomes have been challenging to identify.

Large floods drive changes in cause-specific mortality in the United States.

Flooding greatly endangers public health and is an urgent concern as rapid population growth in flood-prone regions and more extreme weather events will increase the number of people at risk. However, an exhaustive analysis of mortality following floods has not been conducted. Here we used 35.

Pan-phylum genomes of hornworts reveal conserved autosomes but dynamic accessory and sex chromosomes.

Hornworts, one of the three bryophyte phyla, show some of the deepest divergences in extant land plants, with some families separated by more than 300 million years. Previous hornwort genomes represented only one genus, limiting the ability to infer evolution within hornworts and their early land plant ancestors. Here we report ten new chromosome-scale genomes representing all hornwort families and most of the genera.

Hornworts reveal a spatial model for pyrenoid-based CO-concentrating mechanisms in land plants.

Pyrenoid-based CO-concentrating mechanisms (pCCMs) turbocharge photosynthesis by saturating CO around Rubisco. Hornworts are the only land plants with a pCCM. Owing to their closer relationship to crops, hornworts could offer greater translational potential than the green alga Chlamydomonas, the traditional model for studying pCCMs.

A bipartite bacterial virulence factor targets the complement system and neutrophil activation.

The complement system and neutrophils constitute the two main pillars of the host innate immune defense against infection by bacterial pathogens. Here, we identify T-Mac, a novel virulence factor of the periodontal pathogen Treponema denticola that allows bacteria to evade both defense systems. We show that T-Mac is expressed as a pre-protein that is cleaved into two functional units.

Purpose: This study is to evaluate duration of oocyte cryostorage and association with thaw survival, fertilization, blastulation, ploidy rates, and pregnancy outcomes in patients seeking fertility preservation.

Methods: Retrospective cohort study to evaluate clinical outcomes in patients who underwent fertility preservation from 2011 to 2023 via oocyte vitrification for non-oncologic indications. Primary outcome was thaw survival rate.

Magnetic resonance imaging of hepatocellular carcinoma: a spectrum of uncommon morphologic subtypes, unusual imaging patterns and mimics.

Advances in imaging techniques have evolved, allowing for early noninvasive diagnosis and improved management of high-risk patients with hepatocellular carcinoma (HCC). The hallmark imaging features of HCC on multiphasic cross-sectional imaging can be explained by the multistep process of hepatocarcinogenesis and is seen in 60% of cases. However, approximately 40% of cases do not abide by the classic imaging appearance and may pose a diagnostic challenge for radiologists.

Creating a predictive model and online calculator for high-value care outcomes following glioblastoma resection: incorporating neighborhood socioeconomic status index.

Purpose: Social determinants of health including neighborhood socioeconomic status, have been established to play a profound role in overall access to care and outcomes in numerous specialized disease entities. To provide glioblastoma multiforme (GBM) patients with high-quality care, it is crucial to identify predictors of hospital length of stay (LOS), discharge disposition, and access to postoperative adjuvant chemoradiation. In this study, we incorporate a novel neighborhood socioeconomic status index (NSES) and develop three predictive algorithms for assessing post-operative outcomes in GBM patients, offering a tool for preoperative risk stratification of GBM patients.

Governance Considerations for Point-of-Care Ultrasound: a HIMSS-SIIM Enterprise Imaging Community Whitepaper in Collaboration with AIUM.

Point-of-care ultrasound (POCUS) has emerged as a standard of care across a variety of healthcare settings due to its ability to provide critical clinical information and as well as procedural guidance to clinicians directly at the bedside. Implementation of enterprise imaging (EI) strategies is needed such that POCUS images can be appropriately captured, indexed, managed, stored, distributed, viewed, and analyzed. Because of its unique workflow and educational requirements, reliance on traditional order-based workflow solutions may be insufficient.

Cerebellar Ataxia, Neuropathy, and Vestibular Areflexia as a Presentation of a Novel DNMT1 Mutation.

A 50-year-old woman with a 20-year history of gait instability presented with new-onset vertigo and oscillopsia. Examination revealed bilateral vestibular loss, cerebellar ataxia, sensory neuropathy, a "yes-yes" head tremor, nystagmus and a family history of a similar syndrome. Genetic testing for cerebellar ataxia with neuropathy and bilateral vestibular areflexia syndrome (RFC1) was negative, but whole exome sequencing identified a novel mutation in the DNA methyltransferase 1 (DNMT1) gene, broadening the differential diagnosis for this phenotype.

Generating distant analogies increases metaphor production.

Although a large body of work has explored the mechanisms underlying metaphor comprehension, less research has focused on spontaneous metaphor production. Previous research suggests that reasoning about analogies can induce a relational mindset, which causes a greater focus on underlying abstract similarities. We explored how inducing a relational mindset may increase the tendency to use metaphors to describe topics.
