: The influence of comorbidity on long-term hospitalization and mortality after COVID-19 in adults (40-59 years) and older adults (≥60 years) is yet to be explored. : This is a Danish population-based cohort study of patients with a first-time positive PCR test for COVID-19 from 1 March 2020, to 28 February 2022 (N = 1,034,103). Exposed cohorts were patients with 1) a Charlson Comorbidity Index (CCI) score of 1-2 and 2) a CCI score ≥3, who were compared to patients without comorbidity (CCI of zero) within the groups of adults (67.

Sports cardiology is an evolving field in cardiology, with several topics remaining controversial. Beyond the several well-known benefits of regular exercise practice, the occurrence of adverse clinical events during sports in apparently healthy individuals, especially sudden cardiac death, and the described long-term adverse cardiac adaptations associated to high volume of exercise, remain challenging. The early identification of athletes with increased risk is critical, but the most appropriate preparticipation screening protocols are also debatable and a more personalized evaluation, considering individual and sports-related characteristics, will potentially optimize this evaluation.

Appendicitis as first presentation of aggressive granulomatosis with polyangiitis and diffuse alveolar haemorrhage.

BMJ Case Rep

November 2024

Serviço de Medicina Intensiva, Unidade Local de Saude de Lisboa Ocidental, Lisboa, Portugal.

Acute respiratory distress syndrome (ARDS) can be associated with a wide multiplicity of causes, including diffuse alveolar haemorrhage (DAH). Vasculitis associated with anti-neutrophil cytoplasmic antibody (ANCA), particularly granulomatosis with polyangiitis (GPA), is one of the most common causes of DAH, primarily affecting small and medium-sized vessels in the lungs and kidneys. Diagnosing GPA can be challenging, and it should be considered a potential cause of DAH, even in the absence of other organ involvement.

Background And Objective: Modelling patient-specific aortic biomechanics with advanced computational techniques, such as Fluid-Structure Interaction (FSI), can be crucial to provide effective decision-making indices to enhance current clinical practices. To effectively simulate Ascending Thoracic Aortic Aneurysms (ATAA), the stress-free configuration must be defined. The Zero Pressure Geometry (ZPG) and the Prestress Tensor (PT) are two of the main approaches to tackle this issue.

Histomorphometric parameters of iliac bone in healthy individuals: Systematic review and meta-analysis.

Bone

January 2025

Nephrology Department, Laboratorio de Fisiopatologia Renal 16 (LIM 16), Hospital das Clinicas, Faculdade de Medicina da Universidade de São Paulo, São Paulo, SP, Brazil.

Despite its invasive character, bone biopsy followed by histomorphometry remains the gold standard for diagnosing and classifying many metabolic bone diseases. However, the interpretation of histomorphometric parameters requires comparison with average values obtained from a proper control group, which are only available for some populations, and reference standards still need to be published. Therefore, our objective was to estimate average values for bone histomorphometric parameters overall, by age, gender, and race (White and Black) categories of healthy adult individuals, based on a systematic review and meta-analysis of clinical studies.

Objectives: To re-evaluate cut-offs for disease activity states according to the Axial Spondyloarthritis Disease Activity Score (ASDAS), and study the impact of sex, age, calendar time, disease and symptom duration on ASDAS and ASDAS cut-offs in a large contemporary cohort.

Methods: Data from 2939 patients with axial spondyloarthritis (axSpA) starting their first tumour necrosis factor inhibitor in nine European registries were pooled and analysed. Receiver operating characteristic analyses were performed to identify cut-offs against external criteria.

  • The aging population and improved survival rates after kidney transplants are leading to more cases of cancer before and after these surgeries, prompting a need for better screening and treatment strategies.
  • Kidney transplant recipients face a higher risk for cancer, yet current guidelines for screening and treatment mainly draw from data on the general public, which may not be fully applicable to them.
  • Effective management of cancer in kidney transplant patients involves balancing cancer treatment with immunosuppression to prevent graft rejection, requiring personalized assessments of risks and benefits due to the rise of new cancer therapies.

  • Researchers aimed to create and validate a prediction score for futile recanalization (FR) in patients with low Alberta Stroke Program Early Computed Tomography Scores (ASPECTS) who underwent endovascular thrombectomy (EVT) for large vessel occlusions (LVO).
  • The study analyzed data from 219 patients with anterior circulation LVO and identified key predictors of FR, resulting in the development of the SNAP score, which considers factors like the site of occlusion and patient age.
  • The SNAP score successfully predicted FR with an area under the curve of 0.79 in both training and validation cohorts, indicating its potential usefulness for guiding expectations in patients undergoing EVT for severe strokes.

Article Synopsis
  • This review focuses on improving the understanding and management of cerebral venous thrombosis (CVT), particularly in light of recent studies and emerging risk factors linked to the pandemic.
  • Key diagnostic tools include contrast-enhanced MRI and susceptibility-weighted imaging, which enhance the detection of vein thrombosis, while anticoagulants like dabigatran are found to be as effective as warfarin for long-term treatment.
  • Despite varying outcomes, a significant number of patients with CVT can experience recovery; about 80% achieve good functional status, though many may still face lingering symptoms affecting their quality of life.

  • The study focuses on improving the detection of KRAS mutations in pancreatic ductal adenocarcinoma (PDAC) using a technique called ARMS-HRMA, which is more sensitive than traditional methods.
  • It analyzed tumor and plasma samples from 88 newly diagnosed PDAC patients, finding G12D and G12C mutations linked to worse patient outcomes, including shorter progression-free and overall survival.
  • The findings suggest ARMS-HRMA could be a quick and affordable way to help determine prognosis and treatment strategies for PDAC patients.
