Folliculotropic mycosis fungoides (FMF) is a particular subtype of mycosis fungoides (MF), characterized by an infiltration of neoplastic CD4+ T cells in the epidermis which can spread to all follicular structures, sebaceous glands, sweat glands and hair follicles. Clinically, FMF can exhibit various cutaneous symptoms. However, these symptoms often occur on the scalp, face and neck, which are rarely affected by conventional MF.

An 18-year-old female Japanese patient who suffered from heart failure and severe pulmonary hypertension was referred to our clinic. The etiology of her cardiomyopathy was unclear. The transcoronary ethanol ablation of septum hypertrophy (TASH) results in a necrosis of the myocardium. In contrast to the ischemically induced necrosis, there is not any phagocytosis caused by leukocytes and macrophages, nor does transformation into granulation tissue occur. Even six months after the application of ethanol, the myofibrils can be identified as "ghost cells" surrounded by a web of collagen fibers.
