Background: According to the literature, about one third of patients with brain ischemic symptoms lasting <24 h, which are classified as Transient ischemic attacks (TIAs) according to the traditional "time-based" definition, show the presence of acute ischemic lesions at neuroimaging. Recent evidence has shown that the presence of acute ischemic lesions at neuroimaging may impact on the outcome of patients with transient ischemic symptoms treated with dual antiplatelet treatment (DAPT). This uncertainty is even more compelling in recent years as short-term DAPT has become the standard treatment for any non-cardioembolic TIA or minor ischemic stroke.

Article Synopsis
  • Protocadherins, especially Protocadherin 9 (PCDH9), are important for cell-cell interactions and have been linked to Autism Spectrum Disorder (ASD) and Major Depressive Disorder (MDD).
  • Knockout (KO) of PCDH9 in mice leads to abnormal neuronal development, characterized by larger presynaptic terminals and increased excitatory synapse activity in the hippocampus.
  • The findings suggest that PCDH9 plays a critical role in regulating excitatory synapse morphology and function, influencing glutamatergic transmission and potentially contributing to neurodevelopmental disorders.

Article Synopsis
  • - Short-term dual antiplatelet treatment (DAPT) shows better effectiveness compared to single antiplatelet treatment (SAPT) for preventing secondary strokes in patients with mild to moderate strokes and high-risk TIAs, according to a study of 2016 patients.
  • - Patients treated with DAPT had a higher likelihood of regaining pre-stroke neurological function within 90 days and showed more early neurological improvement compared to those on SAPT.
  • - The study suggests that DAPT might be a safer and more effective option than SAPT in real-world settings, even for patients not fitting the criteria of major clinical trials.

Department of Translational Medicine, "Federico II" University, 80131 Naples, Italy; Telethon Institute of Genetics and Medicine, 80078 Pozzuoli, Italy; Scuola Superiore Meridionale (SSM, School of Advanced Studies), Genomics and Experimental Medicine Program, University of Naples Federico II, 80138 Naples, Italy. Electronic address:

Background: Mucopolysaccharidosis type VI (MPS VI) is due to a deficiency of the lysosomal enzyme arylsulfatase B (ARSB) that results in multi-organ accumulation of glycosaminoglycans (GAGs). Limitations of current treatments prompted the development of a liver-directed gene therapy clinical trial for MPS VI.

Methods: We report the long-term follow-up of patients with MPS VI who discontinued enzyme replacement therapy (ERT) and received a single intravenous infusion of high-dose (6 × 10 genome copies/kg) recombinant adeno-associated virus serotype 8 (AAV8) vector expressing ARSB under the control of a liver-specific promoter (ClinicalTrials.

Article Synopsis
  • Surface dyslexia and dysgraphia are key traits of semantic variant primary progressive aphasia (svPPA), particularly in English, but pose challenges in languages like Italian that have clearer spelling-pronunciation relationships.
  • Regularization errors occur when individuals with svPPA fail to recognize exceptions in language rules, such as incorrect inflection of irregular verbs.
  • The Italian Verb Inflection Test (IVIT) effectively identifies svPPA by demonstrating higher regularization errors in verb inflection compared to other types of Alzheimer's disease, offering a viable diagnostic tool for Italian speakers.

Esophageal pressure as estimation of pleural pressure: a study in a model of eviscerated chest.

BMC Anesthesiol

November 2024

Dipartimento Area Emergenza Urgenza, Fondazione IRCCS Ca' Granda Ospedale Maggiore Policlinico, Milan, Italy.

Background: Transpulmonary pressure is the effective pressure across the lung parenchyma and has been proposed as a guide for mechanical ventilation. The pleural pressure is challenging to directly measure in clinical setting and esophageal manometry using esophageal balloon catheters was suggested for estimation. However, the accuracy of using esophageal pressure to estimate pleural pressure is debated due to variability in the mechanical properties of respiratory system, esophagus and esophageal catheter.

Article Synopsis
  • Myeloproliferative neoplasms are disorders involving stem cells, where chronic inflammation plays a significant role in complications like blood clots, and the neutrophil-to-lymphocyte ratio (NLR) is becoming a potential prognostic tool for assessing these conditions.
  • A study of 473 patients with essential thrombocythemia showed that a high NLR at diagnosis (≥4) increased the risk of both arterial and venous thrombotic events, alongside other factors such as diabetes and hypertension.
  • The NLR is an accessible inflammatory marker that could be used alongside traditional cardiovascular risk assessments to identify patients at higher risk for thrombosis in essential thrombocythemia.

This large multicenter study of 37 magnetic resonance imaging scanners aimed at characterizing, for the first time, spatial profiles of inaccuracy (namely, Δ-profiles) in apparent diffusion coefficient (ADC) values with varying acquisition plan orientation and diffusion weighting gradient direction, using a statistical approach exploiting unsupervised clustering analysis. A diffusion-weighted imaging (DWI) protocol (b-value: 0-200-400-600-800-1000 s mm) with different combinations of acquisition plan orientation (axial/sagittal/coronal) and diffusion weighting gradient direction (anterior-posterior/left-right/feet-head) was acquired on a standard water phantom. For each acquisition setup, Δ-profiles along the 3 main orthogonal directions were characterized by fitting data with a second order polynomial function ().

Alcohol use disorder's complexity arises from genetic and environmental factors, with alcohol metabolism genes and neurotransmitter pathways being critical. This study aims to analyze synaptic plasticity gene expression changes in individuals with AUD in order to study their contribution to AUD development and to identify potential biomarkers of treatment response. RNA was extracted from whole peripheral blood (20 patients, 10 healthy controls), before and after treatment (Qiagen AllPrep RNA/DNA Mini Kit), and the gene expression of 84 genes related to neuroplasticity was studied using the RT2 Profiler for Human Synaptic Plasticity RT-PCR Array (PAHS-126ZA, Qiagen), comparing AUD patients to control and responders to non-responders.

Although allogeneic hematopoietic cell transplantation (HCT) represents a curative approach for many patients with hematological diseases, post-transplantation relapse occurs in 20-50% of cases, representing the primary cause of treatment failure and mortality. Alloreactive donor T cells are responsible for the graft versus leukemia (GvL) effect, which represents the key mechanism for the long-term curative effect of HCT. However, the downside is represented by graft versus host disease (GvHD), largely contributing to transplant-related mortality (TRM).
