Incidental Finding of Double Posterior Belly of Digastric Muscle in Head and Neck Cancer Patient.

Ann Otol Rhinol Laryngol

December 2024

Department of Otolaryngology-Head and Neck Surgery, University of Nevada, Las Vegas, Las Vegas, NV, USA.

Objectives: The digastric muscles have important roles in swallowing, chewing, speech, and landmark identification during neck dissection. The posterior belly of the digastric muscle (PBDM) is often useful for defining boundaries in surgical neck dissection as it contributes to the carotid, submandibular, and submental triangles. The cadaveric prevalence rate of anatomic variations in the digastrics has been reported to be 31.

Minimally-invasive tracheostomy (MIT): A care bundle for safety improvement in high-risk critically ill patients.

J Clin Anesth

December 2024

Department of Internal Medicine I, Gastroenterology, Hepatology, Endocrinology, Rheumatology, and Infectious diseases, University Hospital Regensburg, Regensburg, Germany. Electronic address:

Background: Detailed reports are scarce on minimally-invasive tracheostomy (MIT) techniques for critically ill patients with challenging anatomy or complex coagulopathies. In such high-risk patients, conventional percutaneous dilatational tracheostomy (PDT) may lead to severe complications.

Methods: Aiming to broaden the scope of MIT for patients previously excluded due to high risks, we developed a new care bundle (MIT technique), specifically designed for intensive care specialists.

Complex Bentall Operation: Clinical Pearls to Standardize the Procedure.

Ann Thorac Surg

September 2024

Division of Cardiothoracic Surgery, Department of Surgery, Westchester Medical Center, New York Medical College, Valhalla, New York; Division of Cardiac Surgery, Department of Surgery, New York Medical College, Valhalla, New York.

Background: A straightforward Bentall operation can be performed safely with low mortality, but some challenging cases require a more complex operation. We discuss here the steps of the Bentall procedure.

Methods: We reviewed specific scenarios, such as acute aortic dissection, native valve or prosthetic valve endocarditis, redo Bentall after aortic root replacement, calcified aortic root, and patients with prior coronary artery bypass grafting, mechanical aortic valve replacement, stentless aortic valve replacement, and prior extensive aortic arch repair with proximalization of neck vessels.

Purpose: This study aimed to evaluate the long-term prognosis of contralateral central neck dissection (CND) in papillary thyroid cancer (PTC) patients with ipsilateral lateral neck metastasis. We compared the actual recurrence rate according to the extent of CND-ipsilateral and contralateral sides.

Methods: A total of 708 PTC patients who underwent total thyroidectomy and concomitant ipsilateral or bilateral CND with ipsilateral lateral neck dissection between January 1997 and December 2022 at Samsung Medical Center were retrospectively analyzed.

Introduction: The implementation of various approaches in hip arthroplasties introduces distinct advantages and complications. Notably, widely adopted methods such as the posterior approach have been linked to elevated rates of posterior hip dislocations and iatrogenic sciatic nerve injuries, while the lateral approach has been associated with superior gluteal nerve injuries. In this study, we propose a refined modification of the McFarland and Osborne approach, aiming to amalgamate the most favorable aspects of prior modifications of the lateral approach to the hip.

Factors Affecting Drain Output in Oral Carcinoma.

J Maxillofac Oral Surg

August 2024

Department of Head and Neck Surgery, Advanced Centre for Treatment, Research and Education in Cancer, Tata Memorial Hospital, Homi Bhabha National Institute, PS Building, ACTREC, Kharghar, Navi Mumbai, Mumbai, Maharashtra 410210 India.

Article Synopsis
  • Advanced oral carcinoma surgery often results in large open areas, leading to complications like seroma and hematoma; closed suction drains help manage these issues by creating negative pressure.
  • A study of 222 patients identified that factors such as type of neck dissection, surgery duration, and blood loss significantly affect postoperative drain volume.
  • Key findings suggest that careful assessment of patient and surgical factors is crucial for determining the optimal timing for drain removal, minimizing complications and enhancing recovery.

Article Synopsis
  • Papillary thyroid cancer (PTC) is the most common type of thyroid cancer, with this case report focusing on two instances of occult thyroid carcinoma (OTC), where cancer spreads to lymph nodes without a detectable primary tumor in the thyroid.
  • The first case involves a 67-year-old female who had swelling in her neck, leading to the discovery of PTC metastasis in lymph nodes after surgeries, while the second case features a 79-year-old male with PTC spreading to his parotid gland, both lacking any primary tumor in the thyroid on final analysis.
  • OTC is rare, and its diagnosis can be complicated, so the authors emphasize the need for more research to create standardized diagnostic and treatment protocols for this condition

Chirurgie (Heidelb)

October 2024

Endokrine Chirurgie, Diakonie-Klinikum Stuttgart, Rosenbergstraße 38, 70176, Stuttgart, Deutschland.

After decades of bilateral "subtotal" and later "total" thyroidectomy, the extent of resection is now determined individually depending on the dominant thyroid condition. The leading indication by far in the study, documentation and quality center (StuDoQ) register of the Surgical Working Group Endocrinology (CAEK) is currently the suspicion of malignancy, which is followed by benign symptomatic nodular goiter, functional disorders and confirmed malignancy. The decision for an intervention as well as the extent of resection must be rigorously established.

The incidence of mixed thyroid carcinoma of poorly differentiated thyroid carcinoma (PDTC) and papillary carcinoma thyroid is very unusual. PDTC exhibits a high degree of dedifferentiation and histopathological confirmation is done based on Turin's criteria. This type of carcinoma has a poor prognosis and the survival rates at five and ten years post-diagnosis are significantly lower compared to well-differentiated thyroid carcinomas.
