Article Synopsis
  • Lymphovascular invasion (LVI) significantly affects the postoperative prognosis in patients with esophageal squamous cell carcinoma (ESCC) who have undergone neoadjuvant chemotherapy (NAC) and surgery, with separate analysis of lymphatic and venous invasion being infrequently studied.
  • A study involving 427 ESCC patients found that those with venous invasion alone had notably worse survival rates and higher recurrence risks, and those with both lymphatic and venous invasion faced the most severe outcomes.
  • The findings suggest that identifying patients with LVI, particularly those with both types of invasion, could lead to better-targeted postoperative treatments to improve their prognosis and manage the risk of distant metastases.

  • Cricothyrotomy is a procedure used to secure an airway in emergency situations and can also help patients expel sputum; however, its overall safety and effectiveness are still being evaluated.
  • A nationwide survey in Japan involved data from 1,001 patients across 116 healthcare facilities between 2010 and 2021, highlighting that most cricothyrotomies (94.4%) were performed for sputum suctioning post-surgery, primarily after esophagectomy.
  • Results indicated lower complication rates for sputum suctioning compared to emergency airway clearance, suggesting that cricothyrotomy is safer for patients recovering from esophagectomy, but additional research is necessary to confirm its benefits.

Azacitidine and gemtuzumab ozogamicin as post-transplant maintenance therapy for high-risk hematologic malignancies.

Bone Marrow Transplant

August 2024

Hematology Division, Tokyo Metropolitan Cancer and Infectious Diseases Center, Komagome Hospital, Tokyo, Japan.

Disease recurrence remains the principal cause of treatment failure after allogeneic hematopoietic stem cell transplantation. Post-transplant maintenance therapy with azacitidine (AZA) is promising to prevent relapse but the outcomes are unsatisfactory in patients at high risk of recurrence. Herein, we evaluated the outcome in patients who received AZA and gemtuzumab ozogamicin (GO), anti-CD33 antibody-calicheamicin conjugate, as post-transplant maintenance therapy.

The Japanese Breast Cancer Society Clinical Practice Guidelines are published as timely guidance on clinical issues in breast cancer treatment in Japan. In the recent edition of these guidelines, we addressed a new clinical question 34 (CQ 34, systemic treatment part) "Is trastuzumab deruxtecan recommended for patients with unresectable or metastatic HER2-low breast cancer?" and a new future research question 7 (FRQ 7, pathological diagnosis part) "How is HER2-low breast cancer diagnosed for the indication of trastuzumab deruxtecan?". These questions address use of trastuzumab deruxtecan in patients with unresectable or metastatic HER2-low breast cancer who have previously received chemotherapy for metastatic disease.

  • In a study involving nearly 10,000 advanced non-small cell lung cancer (NSCLC) patients, researchers examined the effectiveness of EGFR-targeted therapies specifically in those with poor health performance status (PS) compared to those with better PS.
  • Out of the 1,965 patients with EGFR mutations, those with poor PS (8.7% of total) showed significantly lower one-year survival rates; while EGFR-TKI treatment improved survival for these patients, their overall survival was only 15.5 months, much worse than the 31.5 months for good-PS patients.
  • The findings indicate that poor PS is linked to reduced efficacy of EGFR-TKIs

Progressive multifocal leukoencephalopathy (PML) is a demyelinating disease of the central nervous system (CNS) caused by reactivation of dormant JC polyomavirus (JCPyV). PML was mainly observed in immunocompromised individuals, such as HIV-positive patients, autoimmune disease patients, and cancer patients. Given that the presence of anti-JCPyV antibodies in serum is a risk indicator for PML development, it is essential to monitor anti-JCPyV antibody levels.
