Genomics Inform

October 2024

Stanford Health Care, Stanford, CA, USA.

Article Synopsis
  • Electronic phenotyping uses various data analysis methods, including machine learning and natural language processing, to define patient characteristics, but the current process is slow and labor-intensive.
  • Large language models could automate phenotype definition extraction but have reliability issues and potential risks of generating misleading information.
  • The study aims to create a standard evaluation set for assessing large language models' outputs and to test different prompting methods, showing promising results that still need human validation to ensure accuracy and efficiency in phenotype extraction.

Personal and structural barriers to HIV pre-exposure prophylaxis (PrEP) care result in its underutilization and premature discontinuation. A home-based PrEP program comprised of telemedicine visits and/or self-administered lab testing may address some of these barriers. Our objective was to assess the acceptability and feasibility of a home-based PrEP program among stakeholders at an urban HIV and primary care clinic.

Operative First Web Contracture Management: Current Strategies.

JB JS Open Access

October 2024

OrthoGeorgia, Macon, Georgia.

Article Synopsis
  • First web contractures can arise from various causes such as nonthermal trauma, burns, congenital factors, and Dupuytren contracture.
  • Mild cases may be treated with techniques like contracture release and skin grafting, while severe cases may need more complex solutions, as they create voids requiring additional coverage.
  • Reconstructive options for severe cases include different flap methods, free tissue transfer, and techniques like distraction osteogenesis or pollicization for deeper webbing.

Results from clinical trials show that serotonergic psychedelics have efficacy in treating psychiatric disorders, where currently approved pharmacotherapies are inadequate. Developing psychedelic medicines, however, comes with unique challenges, such as tempering heightened anxiety associated with the psychedelic experience. We conceived a new strategy to potentially mitigate psychedelic effects with defined electromagnetic signals (ES).

The initial step in pre-mRNA splicing involves formation of a spliceosome commitment complex (CC) or E-complex by factors that serve to bind and mark the exon-intron boundaries that will undergo splicing. The CC component U1 snRNP assembles at the 5'-splice site (ss), whereas SF1, U2AF2, and U2AF1 define the 3'-ss of the intron. A PRP40 protein bridges U1 snRNP with factors at the 3'-ss.

Introduction: One of the most common causes of bleeding during pancreaticoduodenectomy is a dissection of the pancreatic head from the superior mesenteric vein and superior mesenteric artery. Knowledge of the anatomical variants of the veins draining the proximal jejunum may allow better control of bleeding during detachment of the uncinate process and pancreatic head from the mesenteric pedicle and division of the mesopancreas. The aim of this systematic review and meta-analysis was to evaluate the anatomical variations of the first jejunal vein (FJV) and jejunal trunk (FJT).

Diabetes is a chronic metabolic disorder that impacts all age groups and affects a large population worldwide. Humans receive glucose from almost every food source, and after absorption from the gut, it reaches the liver, which functions as the distribution center for it. The insulin-responsive glucose transporter type 4 (GLUT-4) is a major carrier of glucose to the various cells (majorly expressed in myocytes, adipocytes, and cardiomyocytes) in a well-fed state.

The Effect of COVID-19 on Lung Cancer Screening.

Cureus

September 2024

Hematology and Oncology, Piedmont Healthcare, Columbus, USA.

Background During the COVID-19 pandemic, many hospitals suspended non-essential medical procedures to reduce transmission and prioritize personal protective equipment (PPE) for COVID-19 patients. Hospitals that continued these procedures faced uncertainty about patient attendance. Multiple factors could explain a decline in patient attendance during the pandemic, including patients' reluctance to risk COVID-19 exposure in the hospital or their own illness requiring self-isolation.

COVID-19 increases the risk for acute ischemic stroke, yet the molecular mechanisms are unclear and remain unresolved medical challenges. We hypothesize that the SARS-CoV-2 spike protein exacerbates stroke and cerebrovascular complications by increasing coagulation and decreasing fibrinolysis by disrupting the renin-angiotensin-aldosterone system (RAAS). A thromboembolic model was induced in humanized ACE2 knock-in mice after one week of SARS-CoV-2 spike protein injection.

The celiacomesenteric trunk (CMT) is a rare anatomical variant where the celiac axis and superior mesenteric artery share a common origin. Despite its rarity, CMT has significant implications across various medical fields, particularly in surgical planning and interventional procedures. We report a case of chronic mesenteric ischemia owing to atherosclerotic stenosis at the CMT bifurcation, necessitating a complex interventional approach.
