Acute Focal Dystonia as a Presentation of Uncontrolled Hyperglycemia.

Hyperglycemia-induced involuntary movements (HIIM) include tremors, hemichorea-hemiballismus (HCHB), and more rarely, dystonia. Presentations may vary, but hyperintensity involving the basal ganglia area on the T1 sequence of MRI brain remains a commonality.  We report the occurrence of focal dystonia with uncontrolled hyperglycemia but no focal abnormalities on MRI.

Navigating recovery while postpartum: examining the relationship between maternal functioning and substance use disorders.

Our current understanding of postpartum people's functioning while managing substance use disorder (SUD) recovery is limited. This study explored whether having a SUD impacts maternal functioning and examined the reliability of the Barkin Index of Maternal Functioning (BIMF) in postpartum people with SUDs. We conducted a prospective, cross-sectional survey ( = 141) by administering the BIMF and a demographic questionnaire with a sample of postpartum people with SUD ( = 48) and a comparative sample of postpartum people without SUD ( = 93).

Evaluation of the Distal Extent of Visualization Using Volar Approaches for Fixation of Distal Radius Fractures.

Purpose: Distal radius fractures may include difficult-to-treat fractures of the volar ulnar corner, colloquially termed as the "critical corner." These fragments are problematic because they are often missed, and their fixation is difficult. We aimed to compare the distal extent of the exposure of the volar surface of the distal radius between the classic Henry approach and the extended flexor carpi radialis (EFCR) approach.

Developing tailored implementation strategies to increase the use of evidence-based practice (EBP) requires accurate identification of predictors of their use. However, known difficulties with measuring EBP use complicates interpretation of the extant literature. In this proof-of-concept study, we examined whether the same predictors of use of cognitive behavioral therapy (CBT) are identified when CBT use is measured with clinician self-report compared to direct observation.

Acceptability and Feasibility of Implementing a Home-Based HIV Pre-Exposure Prophylaxis Program in an Urban Clinic.

Personal and structural barriers to HIV pre-exposure prophylaxis (PrEP) care result in its underutilization and premature discontinuation. A home-based PrEP program comprised of telemedicine visits and/or self-administered lab testing may address some of these barriers. Our objective was to assess the acceptability and feasibility of a home-based PrEP program among stakeholders at an urban HIV and primary care clinic.
