Article Synopsis
  • - Thrombotic microangiopathy (TMA) is a group of disorders where small blood vessel clotting causes organ damage, including conditions like thrombotic thrombocytopenic purpura (TTP) and hemolytic-uremic syndrome (HUS), each with unique causes and impacts on health.
  • - In TMA, inflammation leads to endothelial damage and activates platelet and coagulation processes, often linked to low ADAMTS13 enzyme levels, particularly in cancer patients undergoing chemotherapy, which raises thrombotic risk by increasing the VWF/ADAMTS13 ratio.
  • - Treatment focuses on diagnosing the specific cause and may include therapies to inhibit complement activation, supportive care, and plasmapheresis

Pediatric antiphospholipid syndrome: is it the same as an adult?

J Matern Fetal Neonatal Med

December 2024

Department of Obstetrics, Gynecology and Perinatal Medicine, N. F. Filatov Clinical Institute of Children's Health, I. M. Sechenov First Moscow State Medical University (Sechenov University), Moscow, Russia.

Importance: Antiphospholipid syndrome in neonates and children is a rare, but in some cases life-threatening condition with arterial and/or venous thrombosis and/or non-thrombotic neurological, skin, ophthalmological and other manifestations.

Observations: This review highlights the available information about the features of pediatric APS, including the rare catastrophic form, the differences between pediatric and adult APS, and the role of genetic thrombophilia in APS manifestation.

Conclusions And Relevance: The clinical manifestations and treatment options for APS in children may differ from those in adults, and prescribing therapy can be challenging due to the unique clinical and morphological characteristics of the pediatric patient.

Catastrophic Antiphospholipid Syndrome.

Unlike classic APS, CAPS causes multiple microthrombosis due to an increased inflammatory response, known as a "thrombotic storm". CAPS typically develops after infection, trauma, or surgery and begins with the following symptoms: fever, thrombocytopenia, muscle weakness, visual and cognitive disturbances, abdominal pain, renal failure, and disseminated intravascular coagulation. Although the presence of antiphospholipid antibodies in the blood is one of the diagnostic criteria, the level of these antibodies can fluctuate significantly, which complicates the diagnostic process and can lead to erroneous interpretation of rapidly developing symptoms.

A truncated HIV Tat demonstrates potent and specific latency reversal activity.

Antimicrob Agents Chemother

November 2023

Janssen Infectious Diseases, A Division of Janssen Pharmaceutica NV , Brisbane, California, USA.

A major barrier to HIV-1 cure is caused by the pool of latently infected CD4 T-cells that persist under combination antiretroviral therapy (cART). This latent reservoir is capable of producing replication-competent infectious viruses once prolonged suppressive cART is withdrawn. Inducing the reactivation of HIV-1 gene expression in T-cells harboring a latent provirus in people living with HIV-1 under cART may result in depletion of this latent reservoir due to cytopathic effects or immune clearance.

Educational interventions to integrate surgical staff within medical units during the COVID-19 pandemic: EDUCOVID survey.

Acta Chir Belg

February 2024

Department of Digestive and Hepato-pancreatic-biliary Surgery, AP-HP, Hôpital Henri-Mondor, Paris Est Créteil University, UPEC, Créteil, France.

Background: The SARS-CoV-2 (COVID-19) pandemic required a rapid surge of healthcare capacity to face a growing number of critically ill patients. For this reason, a support reserve of physicians, including surgeons, were required to be reassigned to offer support.

Objective: To realize a survey on the educational programs deployed (face-to-face or e-learning focusing on infective area, basic gestures, COVID clinical management and intensive care medicine), and their impact on behavior change (Kirkpatrick 3) of the target population of surgeons, measured on a five modalities Likert scale.

To describe the frequency and nature of premedication practices for neonatal tracheal intubation (TI) in 2011; to identify independent risk factors for the absence of premedication; to compare data with those from 2005 and to confront observed practices with current recommendations. Data concerning TI performed in neonates during the first 14 days of their admission to participating neonatal/pediatric intensive care units were prospectively collected at the bedside. This study was part of the Epidemiology of Procedural Pain in Neonates study (EPIPPAIN 2) conducted in 16 tertiary care units in the region of Paris, France, in 2011.

Nebulized Antibiotics for Healthcare- and Ventilator-Associated Pneumonia.

Semin Respir Crit Care Med

April 2022

INSERM U1070 Université de Poitiers, UFR de Médecine Pharmacie and Service des Urgences Adultes & SAMU 86, Centre Hospitalier Universitaire de Poitiers, Poitiers, France.

Global emergence of multidrug-resistant and extensive drug-resistant gram-negative bacteria has increased the risk of treatment failure, especially for healthcare- or ventilator-associated pneumonia (HAP/VAP). Nebulization of antibiotics, by providing high intrapulmonary antibiotic concentrations, represents a promising approach to optimize the treatment of HAP/VAP due to multidrug-resistant and extensive drug-resistant gram-negative bacteria, while limiting systemic antibiotic exposure. Aminoglycosides and colistin methanesulfonate are the most common nebulized antibiotics.

Does nonviolent communication education improve empathy in French medical students?

Int J Med Educ

October 2021

TRAJECTOIRES Team, Lyon Neuroscience Research Center, INSERM, U1028, CNRS, UMR5292, University of Lyon, France.

Article Synopsis
  • The study aimed to assess how nonviolent communication (NVC) training affects medical students' empathy, using various implicit and explicit measurement methods.
  • 312 French medical students were divided into an intervention group (123 students who received NVC training) and a control group (189 students) to compare empathy skills before and after training.
  • Results showed a significant improvement in empathy scores for the intervention group, indicating that NVC training can enhance empathic abilities among medical students, suggesting its potential value in medical education.

Nebulized Colistin in Ventilator-Associated Pneumonia and Tracheobronchitis: Historical Background, Pharmacokinetics and Perspectives.

Microorganisms

May 2021

Multidisciplinary Intensive Care Unit, Department of Anaesthesiology and Critical Care, La Pitié-Salpêtrière Hospital, Assistance Publique Hôpitaux de Paris, Medicine Sorbonne University, 75012 Paris, France.

Clinical evidence suggests that nebulized colistimethate sodium (CMS) has benefits for treating lower respiratory tract infections caused by multidrug-resistant Gram-negative bacteria (GNB). Colistin is positively charged, while CMS is negatively charged, and both have a high molecular mass and are hydrophilic. These physico-chemical characteristics impair crossing of the alveolo-capillary membrane but enable the disruption of the bacterial wall of GNB and the aggregation of the circulating lipopolysaccharide.

Department of digestive and oncological surgery, Bicêtre hospital, Paris Sud university hospital Group, Assistance Publique-Hôpitaux de Paris, 63, rue Gabriel-Péri, 94275 Le Kremlin Bicêtre, France; Paris Sud Faculty of Medicine, Paris Saclay university, 94275 Le Kremlin-Bicêtre, France. Electronic address:

The most widely practiced (standard) treatment of non-metastatic rectal cancer is based on proctectomy with mesorectal excision (partial or total according to the location of the tumor and commonly called TME). Surgery is preceded by CAP50-type chemoradiotherapy (capecitabineand 50 Grays radiation) and performed 6-8 weeks after the end of chemoradiotherapy. The development of new endoscopic, surgical, radiation-based and chemotherapeutic modalities leads surgeons to envisage customized treatment to find the best compromise between functional and oncologic results according to the locoregional extension of the tumor.
