As general opinion, conservative treatment is usually suggested for closed humeral shaft fractures with radial nerve palsy; however, some authors advocate early surgical exploration, particularly for spiral fractures of the humerus accompanied with radial nerve palsy. In this article, we present a case of radial nerve palsy after fracture of the proximal-middle third of humeral shaft. A surgical exploration of the nerve revealed that the sharp tip of the bony fragment at fracture site was penetrated into the nerve and separated the nerve as two bands, almost like a ″buttonhole″ injury.

Migration of Kirschner wires (K-wires) postoperatively and with use of cannulated implants intraoperatively is a well-known complication. In this article, we present an extraordinary mechanism causing intraoperative migration of a K-wire. Background: Although the proximal radial artery has been reported as an alternative inflow to prevent steal syndrome, brachiobasilic fistula has been reported to be associated with steal syndrome in 10-20% of cases. We aimed to compare proximal radiobasilic arteriovenous fistula (AVF) with brachiobasilic AVFs on the upper arm in terms of steal syndrome and outcomes.
Material And Methods: We used our institutional operative record database to identify 94 patients in whom brachiobasilic AVF (n=40) and radiobasilic AVF (n=54) were placed between January 2009 and December 2013.

Objectives: This study aims to evaluate the analgesic and functional efficacy of subcutaneous local analgesic infusion (ScLAI) in the early postoperative period (especially on the second postoperative day) in patients undergoing simultaneous bilateral total knee arthroplasty with an intraoperative periarticular injection (PAI) of local analgesic cocktail.
Patients And Methods: Fifteen patients (1 male, 14 females; mean age 62 years; range 52 to 76 years) who underwent simultaneous bilateral total knee arthroplasty (30 knees) and who received the same pre- and intraoperative analgesic protocols were included in this randomized, double-blind, placebo-controlled study. By using a flexible catheter, bupivacaine was administered for ScLAI to either knee (ScLAI group) and placebo infusion was applied to the other one (control group).

Periprosthetic supracondylar femur fractures following total knee arthroplasty are infrequent, but challenging to clinicians. In this article, we present a 74-year-old female case of bilateral supracondylar femoral periprosthetic fractures which were treated with locking plates. The fractures were healed with osseous union, the range of motion was 0-110° for each knee, and there was a 6° residual varus deformity in the left knee.
