(group B streptococcus or GBS) is the most common cause of bacterial meningitis in infants, but is rarely the cause in adults. Across all non-pregnant adults it comprises 7% of bacterial meningitis cases, with a mortality rate of 56% in the elderly. Therefore, while rare, GBS should be a part of a patient's differential when initiating antibiotics in adults with chronic illnesses.

Background: An estimated one-third of the world's burden of disease requires surgical treatment. In many high-income nations, a large proportion of critical surgical procedures are performed laparoscopically due to a number of advantages the technique offers. There is forward progress in the global surgery field to increase access to laparoscopic techniques in low and middle-income settings (LMIC), with potential benefits to both patients and surgeons.

September 2020

Astellas Pharma Global Development Inc., 1 Astellas Way, Northbrook, IL 60062, USA.

Background/objectives: Available metrics for characterizing cumulative anticholinergic exposure over time may not be well suited for use across all US data sources. In this review, the properties of existing anticholinergic scales and measures were evaluated to determine their suitability for implementation in observational studies relying on administrative data.

Methods: A targeted literature review was conducted to identify available anticholinergic scales and measures.

Targeting Mobilization of Ferrous Iron in Infection with an Iron(II)-Caged LpxC Inhibitor.

ACS Infect Dis

August 2019

Department of Pharmaceutical Chemistry , University of California, San Francisco , 600 16th Street , San Francisco , California 94158 , United States.

Iron is essential to all life, and competition for this vital nutrient is central to host-pathogen interactions during infection. The opportunistic Gram-negative pathogen utilizes a diverse array of iron-acquisition strategies, including those enabling import of extracellular ferrous iron. We hypothesize that soluble and redox-active ferrous iron can be employed to activate caged antibiotics at sites of infection in vivo.

A group of scientific and public health experts and key stakeholders convened to discuss the state of knowledge on the relationship between adjuvanted monovalent inactivated 2009 influenza A H1N1 vaccines used during the 2009 influenza pandemic and narcolepsy. There was consensus that an increased risk of narcolepsy was consistently observed after Pandemrix (AS03-adjuvanted) vaccine, but similar associations following Arepanrix (AS03-adjuvanted) or Focetria (MF59-adjuvanted) vaccines were not observed. Whether the differences are due to vaccine composition or other factors such as the timing of large-scale vaccination programs relative to H1N1pdm09 wild-type virus circulation in different geographic regions is not clear.

Article Synopsis
  • This study compares the outcomes of robotic versus open approaches to salvage radical prostatectomy for recurrent prostate cancer, finding that robotic surgery results in less blood loss and shorter hospital stays.
  • Although complication rates were similar for both methods, robotic surgery had a lower incidence of anastomotic stricture.
  • The research suggests that robotic salvage radical prostatectomy is associated with better functional outcomes and lower risk of severe incontinence compared to traditional open surgery.

Background: The optimal securement method of endotracheal tubes is unknown but should prevent dislodgement while minimizing complications. The use of an endotracheal tube fastener might reduce complications among critically ill adults undergoing endotracheal intubation.

Methods: In this pragmatic, single-center, randomized trial, critically ill adults admitted to the medical intensive care unit (MICU) and expected to require invasive mechanical ventilation for greater than 24 h were randomized to adhesive tape or endotracheal tube fastener at the time of intubation.

Increased vitamin D receptor expression from macrophages after stimulation with M. tuberculosis among persons who have recovered from extrapulmonary tuberculosis.

BMC Infect Dis

April 2019

Division of Infectious Diseases, Department of Medicine, Vanderbilt University Medical Center, A2209 Medical Center North, 1161 21st Avenue South, Nashville, TN, 37232, USA.

Background: Independent of HIV infection, extrapulmonary TB (EPTB) risk is increased in women, persons of black race or foreign birth, and by genetic variants in vitamin D receptor (VDR), interleukin-1 beta (IL-1β), and toll-like receptor (TLR)-2; functional correlates are unclear. We evaluated macrophage expression of VDR, TLR2, cathelicidin, and TNF-α, and production of IL-1β in HIV-seronegative persons with previous EPTB, previous pulmonary TB, latent M. tuberculosis infection, and uninfected TB contacts.

Characterization of the hemodynamic response function in white matter tracts for event-related fMRI.

Nat Commun

March 2019

Vanderbilt University Institute of Imaging Science, 1161 21st Ave. S, Medical Center North, AA-1105, Nashville, TN, 37232, USA.

Accurate estimates of the BOLD hemodynamic response function (HRF) are crucial for the interpretation and analysis of event-related functional MRI data. To date, however, there have been no comprehensive measurements of the HRF in white matter (WM) despite increasing evidence that BOLD signals in WM change after a stimulus. We performed an event-related cognitive task (Stroop color-word interference) to measure the HRF in selected human WM pathways.

1-Amino-2'-fucosyllactose inhibits biofilm formation by Streptococcus agalactiae.

J Antibiot (Tokyo)

June 2019

Department of Chemistry, Vanderbilt University, 7330 Stevenson Center, Nashville, TN, 37235, USA.

2'-Fucosyllactose (2'-FL) is a ubiquitous oligosaccharide in human milk. Importantly, this carbohydrate promotes the growth of several strains of Bifidobacteria, a class of beneficial gut commensal, and inhibits epithelial binding of pathogens. In light of these protective effects, we elected to evaluate the potential of 2'-FL to serve as an antibacterial agent against Group B Streptococcus (GBS).

January 2019

Division of Clinical Pharmacology, Department of Pharmacology, 2220 Pierce Avenue, Vanderbilt University, 556 Robinson Research Building, Nashville, TN, 37221, USA.

Obesity increases the risk for cardiometabolic diseases. N-acyl phosphatidylethanolamines (NAPEs) are precursors of N-acylethanolamides, which are endogenous lipid satiety factors. Incorporating engineered bacteria expressing NAPEs into the gut microbiota retards development of diet induced obesity in wild-type mice.
