Granulosa cell tumors (GCT) represent a rare subtype of ovarian cancers. A majority of these tumors express androgen receptor (AR), making them hormonally sensitive. AR positivity not only suggests a potential role of anti-androgen therapy in treating these tumors but also poses a cause for concern: female to male (FTM) transgender patients undergoing exogenous testosterone therapy may be at risk for recurrence, progression, or even incidence of this type of cancer.

Surg Endosc

January 2025

Minimally Invasive Foregut Surgery, Division of General Surgery, Department of Surgery, Oregon Health and Sciences, Portland, OR, USA.

Article Synopsis
  • - The Fundamentals of Leadership Development (FLD) Curriculum aims to enhance diversity, equity, and inclusion (DEI) within leadership skills by assessing short-term impacts through pilot courses and different delivery methods.
  • - A total of 79 SAGES Committee members registered, with a completion rate of 69.7%. Most participants self-identified as underrepresented in medicine, and course engagement was notably higher in the intensive 2-day format compared to the 4-week longitudinal format.
  • - Participants reported increased confidence after completing the modules, but the longitudinal course experienced a decline in weekly participation, showing varying levels of engagement across different delivery methods.

Fillet of toe flap (FTF) leverages the "spare parts" algorithm in reconstructive surgery-utilizing tissue from amputated or otherwise non-salvageable body parts, thus avoiding donor-site morbidity. This study assesses the efficacy of FTF coverage in non-traumatic foot amputations. A retrospective review of patients undergoing foot amputation with FTF coverage between January 2013 to August 2023 was conducted.

Basivertebral nerve ablation technique.

September 2024

University of Alabama Birmingham School of Medicine Department, Division of Vascular and Interventional Radiology, Birmingham, AL.

Chronic low back pain represents a leading cause of global disability, but can often be complex in etiology, with multiple pain generators contributing to patient discomfort. Anterior column low back pain involves the vertebral bodies and discs, with the vertebral body endplates now known to be a significant pain generator. Vertebrogenic low back pain secondary to irritation of the basivertebral nerve presents as axial discomfort, worse with forward flexion, and with concomitant Type I or II Modic changes on MRI.

Thermal ablation is used to treat liver metastasis including those from breast cancer. The ablation is associated with pain, hemorrhage, and biliary structure damage leading to bilomas. Biloma is a collection of bile that can occur inside or outside the biliary system, which could happen as a rare complication of surgery (from procedures like abdominal surgery or diagnostic procedures), trauma, or spontaneously.

The utility of ChatGPT in gender-affirming mastectomy education.

J Plast Reconstr Aesthet Surg

December 2024

Department of Plastic and Reconstructive Surgery, MedStar Georgetown University Hospital, Washington, DC, USA.

Article Synopsis
  • - The integration of AI tools like ChatGPT in medicine can enhance patient education, particularly for gender-affirming mastectomy (GAM), which is significant for many transmasculine and non-binary individuals in their transition.
  • - This study evaluated the quality and readability of ChatGPT's responses to 10 frequent questions about GAM, revealing a readability level suitable for college graduates but also issues with accuracy and comprehensiveness in the information provided.
  • - While physicians found value in ChatGPT's tone and patient-centered approach, they cautioned against over-reliance on it due to instances of misinformation and the need for professional medical advice.

Article Synopsis
  • Spinal cord injuries lead to higher long-term costs and reduced life expectancy, highlighting a significant impact on quality of life.* -
  • Current treatment guidelines are primarily based on limited studies, with a focus on the American Spinal Injury Association impairment scale showing that grades A and B have poorer ambulation recovery compared to grades C and D.* -
  • Early surgical intervention within 24 hours, particularly for cervical spine injuries, may improve motor function, while the effectiveness of using mean arterial pressure goals and steroids for treatment needs more research for better understanding.*

Purpose: Estimating resection weight for patients undergoing reduction mammoplasty (RM) informs surgical planning and outcomes. Existing formulas have limitations that make insurance coverage and accurate surgical planning challenging. We identified the most commonly used scales to evaluate their accuracy in estimating resection weight by applying them to a database of patients to compare calculated and actual resection weights.

The establishment of the Liver Imaging Reporting and Data System (LI-RADS) in 2011 provided a comprehensive approach to standardized imaging, interpretation, and reporting of liver observations in patients diagnosed with or at risk for hepatocellular carcinoma (HCC). Each set of algorithms provides criteria pertinent to the various components of HCC management including surveillance, diagnosis, staging, and treatment response supported by a detailed lexicon of terms applicable to a wide range of liver imaging scenarios. Before its widespread adoption, the variability in the terminology of diagnostic criteria and definitions of imaging features led to significant challenges in patient management and made it difficult to replicate findings or apply them consistently.

Deep-learning auto-segmentation (DLAS) aims to streamline contouring in clinical settings. Nevertheless, achieving clinical acceptance of DLAS remains a hurdle in abdominal MRI, hindering the implementation of efficient clinical workflows for MR-guided online adaptive radiotherapy (MRgOART). Integrating automated contour quality assurance (ACQA) with automatic contour correction (ACC) techniques could optimize the performance of ACC by concentrating on inaccurate contours.

Electronic address:

Article Synopsis
  • Cerebral vasospasm (CV) after tumor resection is uncommon but linked to serious complications, prompting a study into its risk factors.
  • A review of 61 studies found that CV occurs more in males with an average age of 47.3, commonly after resection of posterior fossa tumors, especially schwannomas and pituitary adenomas.
  • Symptoms usually include altered mental status and weakness, often occurring within the first two weeks post-surgery, and the condition can lead to high mortality rates and significant long-term deficits.
